Low Nephron Number and Kidney Disease.

Reduced nephron endowment and acquired nephron loss increase susceptibility to hypertension and chronic kidney disease (CKD). Low birth weight, prematurity, genetic factors, and adverse prenatal exposures can limit nephron development, whereas aging and kidney injury reduce the number of functioning nephrons after birth. Compensatory glomerular hypertrophy and increased single-nephron filtration can initially preserve overall glomerular filtration rate, but sustained glomerular stress may promote podocyte injury, proteinuria, and glomerulosclerosis. These changes can accelerate nephron loss an

Anabolic Androgenic Steroids: Emerging Use Disorder, Mechanisms of Action, and Clinical Considerations for Pharmacotherapy.

Anabolic androgenic steroids (AAS) are synthetic versions of testosterone that are frequently used as performance-enhancing drugs. Over the past few decades, these drugs have become more popular and readily available through online clinics and retail outlets. At the same time, increasing evidence has demonstrated their addictive potential and the likely emergence of an AAS use disorder. In addition to their impact on reward pathways, which is similar to that of other addictive substances, AAS have an addiction-related unique mechanism involving hormonal dysregulations of the hypothalamic-pitui

Utilization of Kentucky Syringe Service Programs in a Justice-Involved Sample of People Who Inject Drugs.

Residents of Kentucky are at-risk for harms related to the opioid epidemic and involvement in the criminal legal system (CLS) further increases those risks. In 2015, Kentucky legalized syringe service programs (SSPS) to provide low-threshold access to harm reduction services including syringe exchange, naloxone distribution, and linkage to care. The Behavioral Model for Vulnerable Populations (Gelbert et al., 2000) was used to investigate factors associated with SSP utilization among people who inject drugs involved in the CLS in Kentucky. One traditional predisposing factor (high school educa…

Building Multidisciplinary Consensus on Inpatient Xylazine Management through Clinical Protocols.

The evolving unregulated drug supply in the United States has led to an unprecedented rise in xylazine-adulterated synthetic opioid use-rela … xylazine. Workgroup participants were organized into three subgroups: (1) toxicology screening and harm reduction; (2) withdrawal management; and (3) inpatient wound care management and linkage to outpatient wound care. We summarize our implementation process and clinical protocol recommendations of each subgroup and highlight important cross-cutting issues rel …

Quantifying the Harms and Benefits from Serosorting Among HIV-Negative Gay and Bisexual Men: A Systematic Review and Meta-analysis.

We conducted a systematic review and meta-analysis of the association between serosorting and HIV infection among HIV-negative men who have sex with men (MSM). Compared to no condomless anal sex (i.e., consistent condom use or no anal sex), serosorting was associated with increased HIV risk (RR = 1.64, 95% CI 1.37-1.96). Compared to condomless discordant anal sex, serosorting was associated with reduced HIV risk (RR = 0.46, 95% CI 0.33-0.65). Serosorting may be an important harm reduction strategy when condoms are not consistently used, but can be harmful if HIV-negative MSM who consistently u…

Reducing injection intensity is associated with decreased risk for invasive bacterial infection among high-frequency injection drug users.

BACKGROUND: Bacterial infection is a major cause of morbidity and mortality for persons who inject drugs (PWID). Injection cessation may hel … es of relapse into injection use, targeting sustained reductions in drug use intensity may be a key harm reduction modality for improving clinical outcomes in this population.

Ventricular tachycardia after ketamine sedation for fracture reduction.

This case report describes a 10-year-old female patient who underwent ketamine sedation for fracture reduction and experienced asymptomatic ventricular tachycardia after the sedation. She had no history of syncope, chest pain, palpations, or light-headedness and had a normal physical examination. This is the first reported case of a patient experiencing ventricular tachycardia after ketamine use for sedation. This case demonstrates a serious and potentially harmful possible adverse effect of ketamine administration.

[Negative pressure therapy- supportive method in chronic wound treatment].

Negative pressure sound therapy is one of the supportive options for chronic wound treatment. Good results have been reported in increasing angiogenesis, improving blood flow in the wounds, decreasing edema, regulating moist environment, granulation tissue stimulation and wound retraction. In addition, there is significant reduction in bacterial burden and in the amount of harmful products (exotoxins, endotoxins, cytokines and matrix metalloproteinases) in the wound. The healing time for chronic wounds is reduced in comparison with other conservative treatments.

Wearable health trackers: A potential candidate for behavioural addiction?

Wearable health trackers are a type of health and fitness self-service technology that is becoming increasingly popular. Effective monitoring of health parameters offered through these devices helps in planning physical activity and weight reduction, with studies reporting their use in rehabilitation for chronic medical illnesses. However, the potential risks and harms associated with these devices remain underexplored. Apart from physical risks, such as increased risk of accidents, psychological issues related to the use of these devices include stress, guilt, low mood, and health anxiety. Wh

Catastrophic associations predict level of change in anxiety sensitivity in response to cognitive-behavioural treatment for panic.

The aim of the present study was to elucidate one of the factors that might be responsible for the maintenance of panic patients' harmful beliefs about anxiety. Specifically, it was hypothesized that harmful beliefs about anxiety, i.e. anxiety sensitivity, is maintained in panic patients by automatic activation of idiographic catastrophic cognitions. To test this prediction, panic patients participated in a one-session cognitive-behavioural treatment of 4-8h to reduce anxiety sensitivity. The strength of automatic catastrophic cognitions in response to idiographic anxiety symptoms, measured wi

Incarceration experiences in a cohort of active injection drug users.

BACKGROUND: Incarceration has been associated with a number of health-related harms among injection drug users (IDU). However, little is known about the prevalence and correlates of incarceration among community-based samples of IDU. METHODS: We examined the prevalence and correlates of recent incarceration among IDU in the Scientific Evaluation of Supervised Injecting (SEOSI) cohort examined between 1 July 2004 and 30 June 2006 using generalised estimating equations (GEE). RESULTS: A total of 902 individuals were included in the analysis, of whom 255 (28.72%) were female and 536 (59.42%) re

[Occupational burden of the skin: the example of hands].

The target body part primarily affected by stress in an occupation are the hands. The hands are involved in 90-95 % of the nearly 18,000 cases of occupation-related skin diseases registered every year. Harmful substances and stressing procedures, such as, e. g., wet work, tensides, organic solvents, lubricating coolants, metals, and even foods, are able to cause irritation of the skin. These noxious substances penetrate the skin and either cause direct damage of the protective skin barrier (irritative contact dermatitis (CD)) or provoke an immunological reaction (allergic CD); both will become

Hypothalamic Hyperphagia in the Hospice Patient: Case Report and Discussion.

We present a case of insatiable appetite and harmful overeating in a hospice patient that showcases the burden of hypothalamic hyperphagia, a rare complication of brain cancer. While confounders exist such as progression of disease and prior tapering of dexamethasone, in this case the initiation of metformin was associated with substantial appetite reduction and resolution of our patient's debilitating food-seeking behaviors. We will explore metformin as a potential cost-effective option for palliation in a hospice setting and explore some of the physiologic mechanisms involved.

Beyond Estrogen: Treatment Options for Hot Flashes.

Nonhormonal medications and complementary and alternative therapies are used by many women seeking relief from bothersome hot flashes. However, health care professionals may be less familiar with these treatment modalities. Although estrogen remains the most effective medication to reduce hot flashes, its potential harmful effects have led investigators to examine other treatments for hot flashes, and many women seek alternative forms of relief. Most of these trials are limited by a significant placebo effect, which frequently equals the effectiveness of the medication being evaluated. Despite

Has Declining Opioid Dispensing to Cancer Patients Been Tailored to Risk of Opioid Harms?

CONTEXT: Opioid prescribing to cancer patients is declining, but it is unknown whether reductions have been tailored to those at highest risk of opioid-related harms. OBJECTIVES: Examine whether declines in opioid dispensing to patients receiving active cancer treatment are sharper in patients with substance use disorder (SUD) or mental health diagnoses. METHODS: We used 2008-2018 national, commercial healthcare claims data to examine adjusted and unadjusted trends in opioid dispensing (receipt of ≥1 fill; average daily dosage; receipt of high-dose opioids; receipt of concurrent opioids and
