Functional Outcomes 18 Months After Total and Midarm Transplantation: A Case Report.

BACKGROUND: The function reported after arm transplantation is deemed beneficial relative to the marked disability that upper arm amputation causes. OBJECTIVE: We report a 51-year-old man with a Disabilities of the Arm, Shoulder and Hand (DASH) score of 75.83 who underwent bilateral arm transplantation in October 2015. PROCEDURE: The right arm was transplanted at the glenohumeral joint level, including transplantation of the humeral head, joint capsule, and rotator cuff ligaments and tendons. Additionally, neurorrhaphies were performed at the origin of the terminal branches of the brachial p…

Sonographic assessment of subacromial bursa distension during arm abduction: establishing a threshold value in the diagnosis of subacromial impingement syndrome.

PURPOSE: In this study, we aimed to establish a quantitative threshold value in the diagnosis of subacromial impingement syndrome by measuring the thickness of the subacromial bursa during abduction and adduction. MATERIALS AND METHODS: Forty-five patients with subacromial impingement syndrome and 54 healthy individuals underwent dynamic shoulder ultrasonography. The subacromial bursa, between the supraspinatus tendon margin and peribursal adipose tissue, was measured between the acromion and humeral head at its widest part. The subacromial impingement ratio was calculated by dividing the sub…

Quantitative and qualitative analyses of subacromial impingement by kinematic open MRI.

PURPOSE: Quantitative and qualitative kinematic analyses of subacromial impingement by 1.2T open MRI were performed to determine the locatio … oclavicular joint in six and five cases, respectively. The superior RC acted as a depressor for the humeral head against the acromion as the shoulder elevated. The mean elevation angle and distance at the closest position between the RC and the acromion in neutral rotation were 93.5° and 1.6 mm, respectively, while those between the RC and the acromioclavicula …

Bone marrow-derived cells from the footprint infiltrate into the repaired rotator cuff.

BACKGROUND: Cells from the bone marrow are considered important during the rotator cuff repair process, but the kinetics of bone marrow-deri … lls were created. Bilateral supraspinatus tendons were separated from the greater tuberosity of the humeral head to produce a rotator cuff transection model. Drilling into the bone marrow was performed in the greater tuberosity of the right humerus and the supraspinatus tendon was repaired (drilling group), while the supraspinatus tendon was repaired on the le …

Intraosseous foreign body granuloma in rotator cuff repair with bioabsorbable suture anchor.

Biodegradable implants lead to problems such as cyst formation, soft-tissue inflammation, loose implant fragments or local osteolysis. This report represents the first published case of an intraosseous foreign body granuloma in the humeral head after arthroscopic rotator cuff tear fixation with a poly-L: -lactide (PLLA) suture anchor. A 48-year-old female patient presented with pain in her right shoulder. A refixation of her right supraspinatus tendon with a biodegradable suture anchor was performed 11 months ago at an external hospital. Laboratory tests showed normal values for C-reactive pro…

Clinical implications of rotator cuff degeneration in the rheumatic shoulder.

OBJECTIVE: In rheumatoid arthritis (RA) of the shoulder, loss of cartilage and soft tissue degeneration coexists with pain and reduced range … e destruction. RESULTS: Only 19 shoulders showed an intact rotator cuff. Proximal migration of the humeral head and fatty degeneration of the infraspinatus muscle especially showed a significantly strong correlation with increased pain and function loss (R2 = 0.36, P < 0.001). In a multivariate regression analysis, proximal migration and fatty degeneration of …

Effect of coracoacromial ligament resection on glenohumeral stability under active muscle loading in an in vitro model.

PURPOSE: The purpose of this study was to determine whether or not the coracoacromial ligament (CAL) has a relevant effect in stabilizing the humeral head under active rotator cuff and deltoid loading compared to passive loading conditions without muscular stabilization. METHODS: Nine human cadaver shoulders were tested in a dynamic shoulder simulator. Forces of the rotator cuff muscles and the middle deltoid muscle were applied using servohydraulic cylinders, while glenohumeral motion was imposed in closed-loop force-control by a sensor-guided robot. Translational movement was measured with …

Association of a large lateral extension of the acromion with rotator cuff tears.

BACKGROUND: Factors predisposing to tearing of the rotator cuff are poorly understood. We have observed that the acromion of patients with a … der of the acromion was divided by the distance from the glenoid plane to the lateral aspect of the humeral head to calculate the acromion index. This index was determined in a group of 102 patients (average age, 65.0 years) with a proven full-thickness rotator cuff tear, in an age and gender-matched group of forty-seven patients (average age, 63.7 years) with …

The effect of isolated labrum resection on shoulder stability.

The present study was initiated to determine whether glenohumeral instability and dislocation can result from isolated lesions of the glenoi … ection. Total labral debridement increased inferior and anterior translation, but did not allow the humeral head to dislocate. The degree of stability in the cocked-arm position, which is the most prone to dislocation, is not altered. In patients, isolated labral tears, that is, without evidence of capsuloligamentous damage, can probably be safely debrided wit …

Clinical significance of the arthroscopic drive-through sign in shoulder surgery.

PURPOSE: During arthroscopy of the shoulder, the ability to pass the arthroscope easily between the humeral head and the glenoid at the level of the anterior band of the inferior glenohumeral ligament is considered a positive drive-through sign. The drive-through sign has been considered diagnostic of shoulder instability and has been associated with shoulder laxity and with SLAP lesions. The goal of this study was to examine the prevalence of the drive-through sign in patients undergoing shoulder arthroscopy and to determine its relationship to shoulder instability, shoulder laxity, and to SL…

An aneurysm involving the axillary artery and its branch vessels in a major league baseball pitcher. A case report and review of the literature.

Baseball pitchers appear to be prone to aneurysms of the axillary artery and its branches. The cause is probably related to repetitive compression of or tension on the vessels at the level of the pectoralis minor muscle and the humeral head, which is exacerbated by the pitching motion. The incidence of aneurysms of the axillary artery and its branches among pitchers and other athletes is not known, nor is it clear whether pitchers who are at high risk of vascular injury can be identified before irreversible damage to the vessels has occurred. Perhaps patients who have documented compression or…

Pediatric elbow fractures: MRI evaluation.

Magnetic resonance imaging (MRI) was performed in eight patients under the age of 8 years who suffered elbow fractures, to assess possible fracture extension into the distal nonossified epiphysis of the humerus in seven cases and to determine the displacement and location of the radial head in one case. MRI allowed accurate depiction of the fracture line when it extended into the cartilaginous epiphysis. In four cases, MRI findings were confirmed at surgery. In five cases, surgery was obviated because no articular extension of the fracture was seen on MRI (4 cases) or because no displacement w

[Differential diagnosis of epicondylitis humeri radialis].

Intensive amateur sport will lead especially in the untrained to frequent traumatisation. Exact discussion of the symptomatics of the epicondylitis seems to be necessary. Sufficient therapy of various pain symptoms, which are usually described as epicondylitis is only possible after exact differentiation. Paraarticular pain can be caused by the ligamentum-anulare-syndrome as well as by the classic insertion tendinosis. A hypertrophy of the radio-humeral disc can also be the reason of pain after posttraumatic synovitis. Osteoarthritis and rheumatoid arthritis can easily be differentiated from p
