Length Changes of the Medial Patellofemoral Ligament During In Vivo Knee Motion: An Evaluation Using Dynamic Computed Tomography.

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction is associated with high complication rates because of graft overloading from incorrect graft positioning. To improve clinical outcomes, it is crucial to gain a better understanding of MPFL elongation patterns. PURPOSE: To assess MPFL length changes in healthy knees from 0° to 90° of dynamic flexion and their relationship with anatomic parameters of the patellofemoral joint. STUDY DESIGN: Descriptive laboratory study. METHODS: Dynamic computed tomography scans of an active flexion-extension-flexion movement in 115 knees from 63

How to Address the Medial Patellofemoral Ligament, Tibial Tubercle, and Articular Cartilage in Patients With Recurrent Patellar Instability.

Patellofemoral instability is a common pathology especially in the adolescent female population.1,2 Prompt diagnosis and management is critical to prevent future episodes of instability as well as to reduce the risk of cartilaginous injury to the patellofemoral articular surface. Initial management of a first-time patellar dislocation has historically been nonsurgical; however, the presence of intra-articular loose bodies or osseocartilaginous injury may require surgical intervention.3,4 More recent evidence has shown patients with specific risk factors such as skeletal immaturity, an incompet

Unusual Horizontal Patellar Dislocation Reduced without General Anesthesia: A Report of 2 Cases.

CASE: Horizontal patellar dislocation is a rare clinical event, and there is no consensus on its management. We describe 2 relatively elderly patients with 2 different types of horizontal patellar dislocation who were successfully treated with closed reduction without general anesthesia. CONCLUSION: Appropriate and gently managed closed reduction without general anesthesia may be successful in cases of acute horizontal patellar dislocation, provided there are no osteophytes or an injury that requires surgical treatment. With closed reduction and subsequent rehabilitation, patients can regain

[Influencing factors of medial patellofemoral ligament reconstruction for patellar dislocation].

OBJECTIVE: To review the influencing factors of medial patellofemoral ligament (MPFL) reconstruction for patellar dislocation. METHODS: The literature of MPFL reconstruction for patellar dislocation at home and abroad in recent years were summarized and analyzed. RESULTS: The influencing factors such as the location of the femoral insertion point, the tension and the fixed angle of the grafts, the dysplasia of the femoral trochlear before operation, the abnormal tuberositas tibiae-trochlear groove value, the high position of the patellar, and the tilting angle of the patellar, are all the fa

Isolated dislocation of the proximal tibiofibular joint in a long jumper.

Acute traumatic proximal tibiofibular joint dislocation is an exceedingly rare injury. This is a case report in a rare horizontal type joint variant, following a long jump injury. The diagnostic approach when this injury is suspected is described.

[A differential diagnosis of quadricipital amyotrophy syndrome: bilateral disinsertion of the quadriceps tendon].

When it occurs bilaterally, disinsertion of the quadriceps tendons may suggest quadricipital amyotrophy syndrome. Questioning and physical examination of the locomotor system usually lead to a diagnosis of mechanical lesion involving the extensor mechanism of the knee. If necessary, this diagnosis can be confirmed by ultrasonography and MRI of the knee. A search for predisposing factors may result in a specific treatment, but only surgery provides functional improvement.

[Dysplasia of the femoral trochlea].

Dysplasia of the trochlea was studied on a strict profile image of the knee. 1305 radiographs were analysed corresponding to several patello-femoral conditions (major and subjective patello-femoral osteoarthritis) and to control subjects. Two criteria are defined: the depth and the eminence of the trochlea. The depth was often minimal in instabilities that give a radiological characteristic image of intersection between the trochlear end line and the lateral condyle. This picture had a great diagnostic value for patellar instability. The eminence represented the owerhang of the trochlear end l

[Chondropathy of the patella. Clinico-radiologic study].

Totally 165 cases of chondromalacia patellae, taken from a series of 240 patients, are presented. The cause of the chondromalacia lies in imbalances, displacements, dysplasias of patellae and in femoro-tibial axial deviations. Overload is the constant intermediary between the different starting causes and the same final result. The precocity of the clinical-radiological diagnosis is essential for the timely carrying out of a corrective surgery, directed to remove the starting causes before the reversible chondromalacia develops into irreversible arthrosis.

Comparison of outcomes and revision free survival of early arthrofibrosis management after total knee replacement: a national cohort analysis.

PURPOSE: This study examined three common treatment options for arthrofibrosis treatment after total knee arthroplasty (TKA)-manipulation under anesthesia (MUA), arthroscopic lysis of adhesions (LOA), and revision TKA (rTKA)-and evaluated differences in medical outcomes, orthopedic outcomes, and revision free survivorship. METHODS: The TriNetX platform was queried to identify patients with arthrofibrosis after TKA. Cohorts were stratified by treatment and matched based on demographics and comorbidities. Differences in short-term medical complications, long-term orthopedic complications, and r

The tibial tubercle-to-trochlear groove distance changes in standing weight-bearing condition: An upright weight-bearing computed tomography analysis.

BACKGROUND: The tibial tubercle-to-trochlear groove (TT-TG) distance and Insall-Salvati (I/S) ratio are widely used to determine the need for distal realignment in conjunction with medial patellofemoral ligament (MPFL) reconstruction in patients with recurrent patellar dislocation. A TT-TG distance >20 mm and an I/S ratio >1.3 are significant anatomical risk factors for patellar instability. However, these parameters have traditionally been measured using non-weight-bearing (NWB) imaging modalities. As patellar dislocation occurs during weight-bearing actions, these two parameters should be me

Patients With Multiple Sclerosis are at Increased Risk for Postoperative Complications Following Total Hip and Knee Arthroplasty.

BACKGROUND: Multiple sclerosis (MS) is a chronic inflammatory demyelinating disease affecting the central nervous system. Patients with MS are living longer due to improved medical therapy and thus the demand for arthroplasty in this population will increase. The objective of this study is to evaluate MS as a potential risk factor for postoperative complications following total hip arthroplasty (THA) and total knee arthroplasty (TKA). METHODS: Patients with a diagnosis of MS who underwent THA or TKA from 2005 to 2014 were identified in a national insurance database. Rates of death, hospital r

Does form follow function in trochlear dysplasia? Case report of patellar instability in a below-knee amputee.

INTRODUCTION: Patellar instability can be considered as a rapidly increasing musculoskeletal subject of interest. It is now well accepted that multiple factors can produce this pathological condition, isolated, but more often in concert. One major static compound is found to be in trochlear dysplasia. It has been stated that the form of the trochlear surface is primary and genetically bilaterally determined already before use. According to these data, recurrent dislocation of the patella has been described as a hereditary disease with autosomal dominant transmission. Against this assumption, w
