Focal hand dystonia cured by removal of clinoid meningioma-case report-.

A 40-year-old Asian female presented with an unusual case of focal hand dystonia caused by contralateral clinoid meningioma. Magnetic resonance imaging showed that the tumor compressed the caudate nucleus, lentiform nucleus, cerebral peduncle, internal capsule, and a large portion of the white matter surrounding the basal ganglia. The tumor was gross totally removed via a frontotemporal approach with zygomatic osteotomy, resulting in cure of the focal hand dystonia. Magnetic resonance imaging after surgery showed that the compression of the surrounding brain was released. This case shows that

Combined treatment of advanced stages of recurrent skin cancer of the head.

OBJECT: The authors investigated whether skull base resection and primary free-flap reconstruction in a single-stage surgery is oncologically effective for treating advanced stages of recurrent skin cancer (RSC) of the head. METHODS: Eighteen consecutive patients were surgically treated. Twelve of them underwent an anterolateral skull base resection, which was performed using a pterional craniotomy combined with an orbitozygomatic osteotomy. Six patients underwent a posterolateral skull base resection, which was performed using an asterional craniotomy combined with a retrolabyrinthine petros

Multi-parametric magnetic resonance imaging-based radiomics for differentiation of skull base osteomyelitis from locally advanced nasopharyngeal carcinoma: a multi-center external validation study.

PURPOSE: Skull base osteomyelitis (SBO) and nasopharyngeal carcinoma (NPca) are challenging to differentiate due to overlapping clinical and radiological features. This study aimed to develop and validate a multi-parametric magnetic resonance imaging (MRI)-based radiomics model with high sensitivity, enabling reliable diagnosis of SBO in adult patients presenting with equivocal imaging findings. METHODS: This was a retrospective, multicenter study using institutional data. The training cohort, comprising 63 adult patients from two classes (31 SBO, 32 NPca) with MRI data, was used for model de

The Role of Comprehensive Structural Preservation Strategy in Skull Base Reconstruction Following Endoscopic Endonasal Surgery for Pituitary Neuroendocrine Tumors: A Retrospective Single-Center Study.

OBJECTIVE: This study aims to evaluate and compare the surgical outcomes of endoscopic endonasal surgery (EES) for pituitary neuroendocrine tumors (PitNETs) using two surgical approaches: the comprehensive structural preservation strategy based on transseptal approach (CSP-TSA) and the traditional transnasal approach (TNA). METHODS: We retrospectively analyzed 62 patients who underwent EES for PitNETs at our center between March 2021 and September 2024. Patients were categorized into CSP-TSA (n = 32) and TNA (n = 30) groups based on the surgical approach. Outcomes assessed included tumor rese

The Minimal Clinically Important Difference of the Anterior Skull Base Nasal Inventory-12.

BACKGROUND: The minimal clinically important difference (MCID) is defined as the smallest change in health-related quality of life (QOL) that patients consider meaningful. The MCID is essential for determining clinically significant changes, rather than simply statistically significant changes, in QOL scores. The Anterior Skull Base Nasal Inventory-12 (ASK Nasal-12), a site-specific sinonasal QOL instrument, has emerged as a standard instrument for assessing QOL in patients who have undergone endonasal transsphenoidal surgery. OBJECTIVE: To determine the MCID for the ASK Nasal-12. METHODS: D

Facial reanimation according to the postresection defect during lateral skull base surgery.

The vast majority of benign tumors of the cerebellopontine angle, temporal bone, and parotid gland can be successfully resected without permanent injury to the facial nerve. Malignant tumors or recurrent disease may require facial nerve sacrifice, especially if preoperative facial paresis is present. This article will present case examples of the various methods to reconstruct facial animation after lateral skull base resections that require sacrifice of cranial nerve VII, and the associated mimetic facial musculature. Facial mimetic outcome after reanimation was graded using the House-Brackma

Update on the surgical management of temporomandibular joint-centered tendon sheath giant cell tumor with intradural extension: introducing a cost-effective method in using temporal bone for skull base reconstruction in preventing brain hernia.

Tendon sheath giant cell tumor is an idiopathic proliferative and destructive disease of the synovium. It is rare and tends to arise in large joints, for example, knee and ankle, but it can also arise in temporomandibular joints (TMJs). Because of its destructive nature, immediate treatment upon diagnosis is recommended. Radical resection proved to be an excellent choice for superior local control. However, the unfavorable anatomic location of TMJ and infratemporal fossa tumor with intradural extension make such a resection impractical. Hereby, we reported a case of resection of a TMJ tendon s

Superior eyelid crease approach for transobital neuroendoscopic surgery of the anterior cranial fossa.

Access to the anterior cranial fossa has traditionally required a large exposure of the surgical field, which can be a source of aesthetic and neurologic morbidity. Minimally invasive surgery approaches have been developed to overcome these adverse effects and was, for a long time, represented by endonasal endoscopic surgery. The superior eyelid crease approach of the anterior cranial fossa with supraorbital osteotomy was described in the early 80s as an interesting alternative to the endonasal approach. This keyhole endoscopic approach allows safe and efficient surgery of the orbit, the anter

Treatment of malignant tumors of the skull base with multi-session radiosurgery.

OBJECTIVE: Malignant tumors that involve the skull base pose significant challenges to the clinician because of the proximity of critical neurovascular structures and limited effectiveness of surgical resection without major morbidity. The purpose of this study was to evaluate the efficacy and safety of multi-session radiosurgery in patients with malignancies of the skull base. METHODS: Clinical and radiographic data for 37 patients treated with image-guided, multi-session radiosurgery between January 2002 and December 2007 were reviewed retrospectively. Lesions were classified according to i

Strategy for facial nerve management during surgical removal of benign jugular foramen tumors: Outcomes and indications.

OBJECTIVE: Classical surgical management of jugular foramen (JF) tumors usually requires facial nerve rerouting which results in permanent facial palsy in most patients. The purpose of the article is to study the outcomes of different rerouting techniques, and to discuss their indications. MATERIAL AND METHODS: We retrospectively reviewed 98 patients with JF tumors operated at our center between January 2008 and December 2016 using different surgical approaches with the following procedures for facial nerve management: total anterior rerouting (TR), partial anterior rerouting (PR), and fallop

Endoscopic Approach to Clival Chordomas: The Northwestern Experience.

INTRODUCTION: Chordomas are rare primary bone tumors with a low-grade histology but an aggressive clinical behavior characterized by local invasion and recurrence. When occurring in the skull base, their treatment is limited by proximity to critical neurovascular structures. Open surgical approaches can carry high morbidity, making the development of alternative approaches desirable. We describe our experience with endoscopic endonasal approaches to clival chordomas over 13 years. METHODS: We performed a retrospective chart review of patients diagnosed with clival chordomas and treated with a

Complex tumors of the glomus jugulare: criteria, treatment, and outcome.

OBJECT: Tumors of the glomus jugulare are benign, slow-growing paragangliomas. Their natural history, surgical treatment, and outcome have been well addressed in the recent literature; however, there remains a subgroup of complex tumors--multiple, giant, malignant, neuropeptide-secreting lesions, and those treated previously by an intervention with an adverse outcome--that is high risk, presents surgical challenges, and is associated with treatment controversy. In this article the authors report on a series of patients with complex glomus jugulare tumors and focus on treatment decisions, avoid
