A mixed-methods pilot randomized control trial of ultrasound visual biofeedback versus standard intervention for children with cleft palate ± cleft lip: Parents' and children's perspectives.

BACKGROUND: Ultrasound visual biofeedback (UVBF) has the potential to be useful for the treatment of compensatory errors in speakers with cleft palate ± lip (CP±L), but there is little research on its effectiveness, or on how acceptable families find the technique. This study reports on parents' and children's perspectives on taking part in a pilot randomized control trial of UVBF compared with articulation intervention. AIMS: To determine the acceptability of randomization, UVBF and articulation intervention to families. We set feasibility criteria of at least 75% of responses rated as accep

Early language measures associated with later psychosis features in 22q11.2 deletion syndrome.

The 22q11.2 deletion syndrome (22q11DS) is associated with impaired cognitive functions and increased risk for schizophrenia spectrum disorders. Speech and language deficits are prominent, with evidence of decline anteceding emergence of psychosis. There is paucity of data examining language function in children with 22q11DS with follow-up assessment of psychosis spectrum (PS) symptoms. We examined the association between early language measures, obtained clinically, and PS status, obtained on average 10.1 years later, in 166 youths with 22q11DS, with repeated language testing in 48. Participa

Effects of oropharyngeal surgery on velopharyngeal competence.

PURPOSE OF REVIEW: This article addresses the question of whether it is safe to perform pharyngeal surgery on children who are at risk for speech disorders. Patients with orofacial clefts or craniofacial disorders often have speech and resonance disorders that affect their understandability. They are also prone to develop sleep apnea or other diseases that affect Waldeyer's ring. RECENT FINDINGS: There is increasing recognition of sleep disordered breathing and obstructive sleep apnea syndrome (OSAS) in the group of children who have, or are at risk of developing, speech disorders. Often, the

SVM-based identification of pathological voices.

This paper proposed a support vector machine (SVM) based classification method to identify diversified pathological voices. Sound signals were sampled from the pronunciation of a vowel "a" vocalized by 214 subjects, including 181 patients suffered from various dysphonias (such as polypoid degeneration, adductor spasmodic dysphonia, vocal fatigue, vocal tremor, vocal fold edema, hyperfunction, and erythema), and 33 healthy subjects. 25 acoustic parameters were calculated from the sampled data for each subject. The original acoustic dataset was first transformed via principal component analysis

Dual free flap transfer using forearm flap for mandibular reconstruction.

To reconstruct a composite mandibular defect, we have simultaneously transferred a vascularized bone graft or osteocutaneous flap together with a forearm flap. The radial forearm flap, being thin, pliable, and having a long vascular stalk, served as mucosal lining and/or an interpositional flap acting as a vascular bridge. Between 1982 and 1989, we used this procedure in 17 patients with a mandibular defect or deformity which developed following treatment of oral cancer. Our clinical experience has demonstrated that this dual free tissue transfer has many advantages. It is useful for obtaining

[Audiophonological evaluation of 16 children fitted with cochlear implants for sensorineural hearing loss induced by bacterial meningitis].

BACKGROUND: Bacterial meningitis (BM) is the primary etiology of acquired sensorineural hearing loss (SNHL) in children and may compromise language development. Since the 1990 s, cochlear implants (CIs) have become part of the management of children with profound SNHL with encouraging results. The aim of this study was to analyze the audiophonological performance of children before and after cochlear implantation for SNHL following bacterial meningitis. METHODS: Retrospective study of all children fitted with CIs for bilateral severe to profound SNHL after bacterial meningitis in the Robert-D

Artificial intelligence for the recognition of benign lesions of vocal folds from audio recordings.

OBJECTIVE: The diagnosis of benign lesions of the vocal fold (BLVF) is still challenging. The analysis of the acoustic signals through the implementation of machine learning models can be a viable solution aimed at offering support for clinical diagnosis. MATERIALS AND METHODS: In this study, a support vector machine was trained and cross-validated (10-fold cross-validation) using 138 features extracted from the acoustic signals of 418 patients with polyps, nodules, oedema, and cysts. The model's performance was presented as accuracy and average F1-score. The results were also analysed in mal

Clinical variables at age 2 predictive of mental retardation at age 5 in children with pervasive developmental disorder.

This study attempted to find clinical variables evaluated at age 2 that would predict mental retardation (MR, IQ/cognition-adaptation developmental quotient [C-A DQ]<70) at age 5 in 57 children with pervasive developmental disorder (PDD). About two-thirds of subjects had MR at both initial and outcome evaluations. The C-A DQ at initial evaluation was significantly lower in mentally retarded PDD (MRPDD) than in high-functioning (IQ >or= 70) PDD (HFPDD). MRPDD changed less than HFPDD in IQ/C-A DQ between ages 2 and 5. The C-A DQ at age 2 was a potent predictor for MR at age 5 and the total score

Speech representation in an unbiased sample of left-handers.

The incidence of left-handedness in a consecutive series of 520 patients selected only for unilateral damage (the majority were strokes), was found to be within normal limits. Left-handers in this sample did not have a higher incidence of aphasia or apraxia than right-handers, although aphasia and apraxia tended to be less severe in left-handers. Statistical evaluation of most previous studies claiming higher frequencies of aphasia in left-handers did not substantiate those claims, except in severely disruptive pathologies. Previous studies employing the sodium Amytal test indicate bilateral s

Psychiatric sequelae in children treated operatively for hydrocephalus in infancy.

This study reports on the prevalence of psychiatric disorder in 45 children of primary-school age who had been treated operatively for hydrocephalus in infancy. A high rate of disturbance was found; presumably this relates not only to neurological factors but also to the child's reaction to his handicaps and to the attitudes of significant people toward him. The symptomatology was predominantly neurotic, and since emotional disorder can be helped by psychological methods it is argued that early psychiatric intervention could improve the quality of life for these children.

The effects of early middle ear pathology on auditory perception and academic achievement.

Two groups of children were compared to determine the effects of early middle ear pathology on the development of auditory perceptual skills and academic achievement. The conductive loss (CL) group consisted of 15 children, aged seven to nine years, with histories of middle ear pathology. The normal control (NC) group was matched for age, sex, and socioeconomic status and had negative histories of middle ear pathology. Both groups were from a suburban community of primarily mild socioeconomic status. A test battery consisting of 12 auditory perceptual tests as well as measures of academic achi

Sequelae of Venezuelan equine encephalitis in humans: a four year follow-up.

The purpose of this study was the identification of possible sequelae of the infection of human individuals with Virus of Venezuelan Equine Encephalitis (VEE). Special emphasis was laid on exploring neurological, psychological and behavioural aspects and particularly on the search for a possible association of the disease with epileptic phenomena, brain damage and/or mental deficiency. A four-year period of observation was conducted on a sample of children from El Carmelo (Colombia) where an epidemic of VEE took place in 1967. A group of seven children who presented the encephalitic type of th

Abnormal cortical processing of the syllable rate of speech in poor readers.

Children with reading impairments have long been associated with impaired perception for rapidly presented acoustic stimuli and recently have shown deficits for slower features. It is not known whether impairments for low-frequency acoustic features negatively impact processing of speech in reading-impaired individuals. Here we provide neurophysiological evidence that poor readers have impaired representation of the speech envelope, the acoustical cue that provides syllable pattern information in speech. We measured cortical-evoked potentials in response to sentence stimuli and found that good

Labial-lingual double articulations in speakers with cleft palate.

OBJECTIVE: Labial-lingual double articulations (LLDAs) are speech errors involving simultaneous valving at the lips and in the linguapalatal region. This study investigates the frequency of LLDAs occurring for /p/, /b/, and /m/ targets and describes the linguapalatal contact patterns involved in these abnormal articulations. DESIGN: A retrospective study involving analysis of articulatory data from all speakers with cleft palate recorded at a research center over a 10-year period. PARTICIPANTS: Twenty-seven speakers aged 5 to 62 years (median 11 years) with compensatory speech errors associa
