Retrospective Study of Miscellaneous Small Visceral Artery Aneurysms: Results of a Single Center-6-Year Surveillance.

BACKGROUND: Visceral artery aneurysms (VAAs) are rare with a reported incidence of 0.1 to 2%. However, VAAs are clinically important and can present as life-threatening ruptures that require emergency treatment. The aim of this study was to review our experience with managing VAAs. METHODS: Retrospective analysis of a prospectively collected database was performed for 88 patients diagnosed with visceral artery aneurysm (VAA) over a 15-year period (2006-2021). Aneurysm size was compared over time using the repeated measures mixed effects model. RESULTS: There were 108 VAAs identified in 88 pa

A Rare Case of Acalculous Cholecystitis Secondary to Infectious Mononucleosis.

Infectious mononucleosis (IM) is a syndrome characterized by malaise, headache, lymphadenopathy, pharyngitis, rash, enlarged spleen, and fever. Epstein-Barr virus (EBV) is the most common cause of IM, though other organisms, such as cytomegalovirus (CMV), can cause it as well. Complications include transient hepatitis, splenic rupture, peritonsillar abscess, and other organ damage. This case study illustrates a case of acalculous cholecystitis caused by IM - a rare complication - with 44 EBV-associated acute acalculous cholecystitis (AAC) cases reported, and far fewer cases of CMV-associated A…

Glandular Fever Testing in Patients Presenting With Tonsillitis: A Retrospective Study.

BACKGROUND: Tonsillitis, the inflammation of the palatine tonsils, typically arises from infections and predominantly affects children of primary school age. Most cases stem from viral or bacterial infections. Glandular fever, a subtype, is primarily caused by the Epstein-Barr virus (EBV). This specific type of tonsillitis can lead to severe complications, including splenic rupture, encephalitis, chronic fatigue, and acute acalculous cholecystitis. As a result, early identification is vital to establish proper safety measures and prevent the onset of these potentially dangerous complications i…

Effectiveness of Haemophilus influenzae type b vaccination after splenectomy - impact on selected immunological parameters.

Splenectomy is a surgery indicated in case of splenic rupture after injury, when there are tumors in the spleen, or as a treatment for certain diseases, such as idiopathic thrombocytopenic purpura and spherocytosis. The aims of the study were to assess the immunological response to the Haemophilus influenzae type b (Hib) vaccine and the post-vaccination changes in lymphocyte subsets and cell activation markers in splenectomized patients and healthy volunteers. Blood samples were collected from 25 patients that had undergone splenectomy and from 15 healthy, non-splenectomized volunteers. All pa…

Hemodynamic benefits of celiac artery release for ruptured right gastric artery aneurysm associated with median arcuate ligament syndrome: a case report.

BACKGROUND: It has been reported that median arcuate ligament syndrome is closely associated with gastric or pancreaticoduodenal artery aneurysms. Hemodynamic state plays an important role in the formation of the aneurysms. These aneurysms are treated with open resection or endovascular exclusion. However, whether revascularization of the celiac artery can prevent the aneurysm formation is unknown. This report indicated a possibility that prophylactic revascularization for celiac artery stenosis resulted in decreased shear stress on the collaterals, which may otherwise be susceptible to new an

Multiple systemic embolism in infective endocarditis underlying in Barlow's disease.

BACKGROUND: Systemic embolism, especially septic embolism, is a severe complication of infective endocarditis (IE). However, concurrent embolism to the brain, coronary arteries, and spleen is very rare. Because of the risk of hemorrhage or visceral rupture, anticoagulants are recommended only if an indication is present, e.g. prosthetic valve. Antiplatelet therapy in IE is controversial, but theoretically, this therapy has the potential to prevent and treat thrombosis and embolism in IE. Unfortunately, clinical trial results have been inconclusive. CASE PRESENTATION: We describe a previously

[Hemorrhage from pseudocysts caused by pseudoaneurysms in chronic pancreatitis. Diagnosis and management].

Bleeding pseudocysts respectively pseudoaneurysms represents a seldom complication of chronic pancreatitis in owing to erosion of pancreatic or peripancreatic arteries. The potential rupture into neighbouring organs or in the peritoneal cavity is accompanied with paramount life-threatening risks. During the last years we observed 3 patients with acute intestinal bleeding caused by pseudocysts converted to pseudoaneurysms. The goal of our report is to analyze the diagnostic tools and the management of patients with bleeding pancreatic pseudocysts, also well establish the clinical constellation,

Primary peritoneal hydatid cyst complicated with small bowel obstruction: a case report.

INTRODUCTION AND IMPORTANCE: Peritoneal hydatidosis is a rare manifestation of echinococcosis, typically secondary to the rupture of hepatic or splenic hydatid cysts or accidental intraoperative dissemination. Herein, we report a rare case of small bowel obstruction caused by a primary peritoneal hydatid cyst, emphasizing the extreme rarity of primary peritoneal involvement leading to bowel obstruction. CASE PRESENTATION: We report the case of an 87-year-old male presenting with acute small bowel obstruction. Clinical examination revealed abdominal distension and tympanism. Imaging identified

Hemothorax: When Hemosuccus Pancreaticus Takes an Unexpected Detour to the Chest.

This case highlights an unusual presentation of bilateral pleural effusion associated with chronic pancreatitis, emphasizing that although pleural effusions are typically unilateral and left-sided, they can occasionally be bilateral. In acute or chronic pancreatitis, pleural effusions are generally transient and resolve once the underlying condition is appropriately managed. While most effusions are left-sided, they may rarely occur on the right side or present bilaterally, as observed in this patient. These effusions are typically exudative, with elevated pleural fluid amylase as a key diagno

Early career clinicians euthanize more dogs with nontraumatic hemoabdomen but not gastric dilatation and volvulus than more experienced clinicians.

OBJECTIVE: To determine if clinician experience influenced the euthanasia rate in 2 common surgical emergencies. ANIMALS: 142 dogs with nontraumatic hemoabdomen (NTH) due to suspected ruptured splenic mass and 99 dogs with gastric dilatation-volvulus (GDV) where the owner either elected surgery or euthanasia. PROCEDURES: Medical records were reviewed for dogs that had either NTH or GDV. For each patient, the owner's decision to pursue euthanasia versus surgery was recorded. The primary clinician was categorized as an intern, defined as a clinician with < 12 months experience, or a non-intern

Clinical efficacy of coil embolization in treating pseudoaneurysm post-Whipple operation.

The present study aimed to evaluate the clinical efficacy and safety of angiography and coil embolization for pseudoaneurysm post-pancreatoduodenectomy (PD). A total of 17 patients with gastrointestinal or abdominal hemorrhage after PD were included in the present study. Angiography was performed on the celiac artery, common hepatic artery, splenic artery, gastroduodenal artery and superior mesenteric artery to determine the size of the pseudoaneurysm, the parent artery and its collateral branches. A variety of embolization techniques have been applied to embolize pseudoaneurysm, including the

Treatment of a congenital diaphragmatic hernia with associated wandering spleen: Case report of a 17-year-old girl.

INTRODUCTION: A congenital diaphragmatic hernia (CDH) is rarely diagnosed in adults and can allow passage of abdominal viscera into the chest cavity. A particularly rare association is a wandering spleen due to absence of its diaphragmatic and retroperitoneal attachment which predisposes to elongation of the vascular pedicle with risk of torsion, infarction and rupture. PRESENTATION OF CASE: A 17-year-old girl presented with a two-day history of increasing abdominal pain. Examination identified an abdominal mass. Computer tomography (CT) chest, abdomen and pelvis revealed a significantly enla

Hydatid disease cured by minimally invasive therapy.

Hydatid disease is relatively frequently encountered in general surgery services (4-5% of the total operated cases). The association of the hepatic and pulmonary localizations is quite common, but the involvement of more organs is rare. The asymptomatic rupture of the hydatid cyst in the peritoneal cavity is exceptional. We present the case of a 69-year old female patient who was hospitalized in the 1st Surgical Clinic for irritative dry cough, loss of weight (4 kg in the last 3 months), fatigue, itching and dyspnea. She had been hospitalized in a department of internal medicine for respirator

[Hamartoma of the spleen].

Hamartoma is a rare benign lesion of the spleen. Between 140 and 150 cases seem to have been described so far. Hamartoma of the spleen may appear as a single or multiple lesions which may tend to converge. It appears in all ages, mainly in elderly persons. About 20% of patients were described in paediatric subjects. Half of the patients have no symptoms, so that hamartomas were discovered by chance at autopsy. Other 50% of patients had pain, splenomegaly, haematologic abnormalities (most frequently thrombocytopenia or pancytopenia) and spontaneous rupture with intra-abdominal bleeding. In chil

[Balloon-occluded retrograde transvenous obliteration (B-RTO) for gastric varices: therapeutic results and problems].

PURPOSE: To evaluate the usefulness of balloon-occluded retrograde transvenous obliteration (B-RTO) in the treatment of gastric varices. MATERIALS AND METHODS: Twenty-one patients with ruptured or high-risk gastric varices underwent B-RTO. A 5-7F balloon catheter was advanced into the gastrorenal shunt and/or gastrophrenic shunt from the femoral vein. Five percent ethanolamine oleate iopamidol (EOI) was injected via the balloon catheter or a microcatheter that was advanced through the balloon catheter. EOI was withdrawn via the catheter after stagnation for 30-60 minutes. Contrast enhanced CT

Pathologic features of naturally occurring juvenile polyarteritis in beagle dogs.

Eighteen young Beagle dogs (eight males and 10 females), ages 6-40 months, with canine juvenile polyarteritis syndrome (CJPS), a naturally occurring vasculitis and perivasculitis of unknown etiology, were necropsied, and their tissues were examined by histopathologic and histochemical methods. The condition is characterized by recurring episodes of an acute onset of fever (> 40 C) and neck pain that persist for 3-7 days. The major histopathologic alterations were a systemic vasculitis and perivasculitis. During the febrile, painful period of CJPS, the vascular lesions ranged from a histiocytic

[Venous thrombosis in gastroenterology].

Venous thrombosis involving the digestive tract affects the suprahepatic veins and the terminal part of the inferior vena cava, the portal vein and its roots. The etiology and diagnosis of this condition have made considerable progress. A thrombogenic disease can now be recognized in 90% of cases of involvement of the suprahepatic veins, and 75% of portal involvements. The most frequent causes are primary myeloproliferative syndromes, paroxysmal nocturnal hemoglobinuria, hereditary deficiency in coagulation proteins and circulating anticoagulants. The cause of involvement of the portal vein al
