OBJECTIVES: Adult-acquired flat foot secondary to a dysfunctional posterior tibialis tendon (PTT) is often treated by surgical transfer of the flexor digitorum longus tendon (FDLT). In this study, the authors compared normal PTT, stage II dysfunctional PTT and replacement FDLT, aiming to define changes in collagen modification, glycosaminoglycan (GAG) and the expression of matrix and metalloproteinase mRNA.
METHODS: Normal PTTs were obtained from patients with no history of tendon problems. Samples of dysfunctional PTT and replacement FDLT tissue were obtained from patients undergoing surgica

PURPOSE: The aim of this study was to evaluate the feasibility of exploring the axillary nerve (AN) at the 6 o'clock position (blind spot) using the deltopectoral approach, with the interval lateral to the conjoint tendon (CJT) or combined with the axillary approach.
METHODS: Four ANs were dissected combining the deltopectoral approach-medial to the CJ (A), the deltopectoral approach-lateral to the CJT (B) and the axillary approach (C) in 3 sequences: A-B-C, B-A-C, and C-B-A. After the first approach was completed, the proximal and distal margins were marked. Additional exposure with the seco

Total ankle arthroplasty (TAA) is becoming popular in patients with rheumatoid arthritis (RA)-associated ankle joint degeneration. However, ankle wound complications can occur after TAA, which sometimes requires challenging reconstruction due to anatomical complexity of the ankle. Superficial circumflex iliac artery (SCIA) perforator (SCIP) flap has been reported to be useful for various reconstructions, but no case has been reported regarding a chimeric SCIP flap for complex ankle reconstruction. We report a case of complex ankle defect successfully reconstructed with a free quadruple-compone

PURPOSE: To report a series of 6 radiocarpal avulsion amputations in 5 patients.
METHOD: We replanted 6 radiocarpal amputations in 5 patients with proximal musculotendinous and nerve avulsion between 2005 and 2013 and reviewed them retrospectively. All 5 patients were men, age range from 21 to 32 years. Functional results were evaluated through total active motion (TAM) of the fingers, 2-point discrimination, monofilament test, grip strength, and Disabilities of the Arm, Shoulder, and Hand questionnaire. Follow-up was 4 to 10 years.
RESULTS: All replanted parts survived without systemic comp

Detachment with reattachment of the Achilles tendon is a common surgery for debridement of retrocalcaneal exostosis, bursitis, and other insertional pathologic entities. The technique involves a midline skin incision on the posterior Achilles to the tendon. The distal Achilles attachment is removed in a U-shaped manner, leaving the medial and lateral flares, but exposing the posterior spur. This midline approach provides excellent exposure and allows for rapid and efficient surgical debridement. The tendon is reapproximated and repaired with a suture anchor to facilitate fixation to the poster

BACKGROUND: One-stage coverage of multiple or jumping defects in the hand remains a great challenge in reconstructive surgery. A medial sural artery perforator-based conjoined or chimeric flap can be a potential candidate for reconstruction and a versatile donor site for tendon and nerve grafts.
METHODS: Between September 2009 and December 2012, two free conjoined and three free chimeric medial sural artery perforator flaps were transferred to reconstruct multiple soft tissue defects in the hands. Only patients with more than two perforators in the preoperative Doppler analysis were admitted

Free functioning muscle transfer (FFMT) is widely used to reconstruct complex lower limb defects involving muscle deficits. This review explores ways to optimize FFMT outcomes by examining biomechanical considerations, particularly balancing the needs for stable joint positioning (isometric function) and active motion (isotonic function) for specific reconstructive objectives. We discuss knee extension, ankle dorsiflexion, and ankle plantarflexion reconstruction, recommending suitable donor muscles based on their mechanical properties and functional requirements. Important considerations inclu

Failed rotator cuff repairs can pose a clinical challenge to the treating orthopedic surgeon. There are many nonsurgical and surgical options available to address the failed rotator cuff repair. Surgical options include revising the primary repair, partial or nonanatomic repair, tendon transfer, biological augmentation or use of tissue-engineered grafts for reconstruction, or total joint arthroplasty (typically with a modern reverse total shoulder arthroplasty system). The treating surgeon must assess the patient's functional status, health status, and expectations in order to customize the ap…

Minimally invasive plate osteosynthesis is a widely used procedure for the treatment of fractures of the femur and the tibia. For a short time it is also used for the treatment of humeral shaft fractures. Among other advantages, the ambassadors of this technique emphasize the lower risk of nerve injuries when compared to open reduction and internal fixation. We report the case of secondary radial nerve palsy caused by percutaneous fixation of a plate above the antecubital fold. The nerve did not recover and the patient needed a tendon transfer to regain active extension of the fingers. This ca…

OBJECTIVE: In this report, we placed focus on the immunological function of lymph nodes and performed lymph node transfer via a free flap to a site of refractory infection.
CASE AND RESULTS: Case 1 describes a 34-year-old male suffering from compound fractures with severe crush injuries and burns in the right ankle joint. A 20 × 15 cm skin defect was observed around the right malleolus medialis, along with denuded tendons with bacterial infection. After conservative treatment, we transferred a lymph-node-containing free superficial circumflex iliac artery perforator flap to the region, with m

This study was designed to determine gait patterns in children with lumbar and sacral neurologic level spina bifida. We studied a group of 2 … of the absence of the plantar flexion-knee extension couple. We found that gait was not improved by tendon transfers performed either at the hip (posterolateral psoas transfer) or at the ankle (tibialis anterior transfer).

A technique is described for reconstruction of a pincer, by a second toe transfer, in traumatic and congenital deformities, leaving only the wrist. An anterior transfer on the radius allows the wrist mobility to compensate for the limited range of mobility of the second toe. Proximal situation of the toe gives the possibility of having plenty of tendons to balance the transfer. From 1987 to 1992, 8 such operations were performed, 2 in traumatic amputations and 6 in cases of congenital anomalies. The mean age of the congenital patients was 13 months (range, 11 to 21 months) and the 2 traumatic

Children with cerebral palsy frequently develop foot deformities, most commonly equinus contractures, which can be managed with orthotics up to age 5 to 7 years. Plantar flexor lengthening, typically around this age, should be restricted to the offending muscle only, usually with a fascia release of the gastrocnemius. Equinovarus, mainly a problem in children with unilateral cerebral palsy, often responds to plantar flexor lengthening. If further tendon transfers are needed, they should be done when the child is older to avoid overcorrection. Planovalgus mostly improves spontaneously up to age…

Novel and combined tissue transfers from the lower extremity provide new tools to combat soft tissue defects of the hand, foot, and ankle, or fracture nonunion. Flaps can be designed for special purposes, such as providing a gliding bed for a grafted or repaired tendon or for thumb or finger reconstruction. Propeller flaps can cover soft tissue defects of the leg and foot. In repairing severe bone and soft tissue defects of the lower extremity, combined approaches, including external fixators, one-stage vascularized bone grafting, and skin or muscle flap coverage of the traumatized leg and foo

Pollicization of the index finger is a well established procedure for some cases of thumb hypoplasia. We reviewed our experience of 27 "normal" index-finger pollicizations with a mean follow-up of seven years, excluding those cases with more extensive radial hypoplasia and abnormal fingers. Even in such an "ideal" situation, strength was deceptive and the frequently slender aspect of the new thumb with quite extensive dorsal scar formation and the "cleft" appearance of the first web disappointing. In a prospective series of 32 cases, we tried to improve both the function and appearance of the

The dorsal approach, with early section of the second metatarsal bone, allows direct access to the plantar structures of the second toe (arteries, nerves, and flexor tendons). A "multiple set arterial transfer" then becomes possible, improving the blood supply to the toe. Early walking is encouraged on the fifth postoperative day when the patient leaves the hospital. No vascular complications have occurred in any microvascular reconstruction after the fifth postoperative day. Close approximation of the first and third toe of the donor foot not only improves the overall appearance of the foot,

Poliomyelitis is an infectious disease characterized by a loss of motor neurons. Affected individuals usually suffer from many abnormalities predisposing them to degenerative joint disease. We report a case of a young male, with a history of poliomyelitis, distal femoral extension osteotomy and previous tendon transfer, suffering from severe knee pain. The patient underwent total knee arthroplasty with posterior stabilized Triathlon® for the femoral side reconstruction and Total-stabilizer Triathlon® for the tibia with short stem. At 2-year follow-up, his range-of-motion had improved, and he c…

Preliminary soft tissue distraction using an external fixator before centralisation and tendon transfer of the flexor and extensor carpi ulnaris to the little finger metacarpal was used to treat Bayne's type III and IV deformities in 18 hands of 14 patients with radial club hands. Treatment with external fixator was started at a mean age of 8 (range 3-30) months. In 16 of 18 hands, the surgical treatment was completed before 10 months of age. Adequate soft tissue stretching was achieved before centralisation using fractional distraction with the external fixator in the majority of hands. After…
