Long-term Outcomes of Modified 360-Degree Suture Trabeculotomy for Uveitic Glaucoma Compared With Primary Open Angle Glaucoma.

PRCIS: A Kaplan-Meier survival curve analysis showed no significant differences in success rates between uveitic glaucoma (UG) and primary open angle glaucoma (POAG) 120 months after modified 360-degree suture trabeculotomy, which was effective for both groups in the long term. PURPOSE: The aim of this study was to examine the outcomes of modified 360-degree suture trabeculotomy in patients with UG as compared with those with POAG. PATIENTS AND METHODS: This was a retrospective, nonrandomized, and comparative case series study. Modified 360-degree trabeculotomy using a 5-0 nylon suture (S-LO

Energy Dose-Response in Selective Laser Trabeculoplasty: A Review.

PRCIS: A literature review of selective laser trabeculoplasty (SLT) energy dose-response found no definitive relationship between intraocular pressure (IOP) reduction with respect to total or pulse energy, race, pigmentation, or application pattern. PURPOSE: SLT is a safe and effective treatment for lowering IOP. Although evidence is mounting for the advantage of its use as a first-line treatment for IOP reduction, the SLT procedures in use vary widely. The purpose of this literature review was to investigate whether there were any relationships between SLT energy and efficacy for lowering IO

A Comprehensive Review of Pediatric Glaucoma Following Cataract Surgery and Progress in Treatment.

Glaucoma following cataract surgery (GFCS) remains a serious postoperative complication of pediatric cataract surgery. Various risk factors, including age at lensectomy, intraocular lens implantation, posterior capsule status, associated ocular/systemic anomaly, additional intraocular surgery, and a family history of congenital cataract and GFCS, have been reported. However, the optimal surgical approach remains unclear. This review evaluates the diagnostic criteria, classification, risk factors, mechanism, and surgical management, especially the efficacy of minimally invasive glaucoma surgery

Outcomes of combined phacoemulsification, anterior vitrectomy, and sclerectomy in nanophthalmic eyes with glaucoma.

OBJECTIVES: To compare the long-term efficacy and safety of combined phacoemulsification, anterior vitrectomy, and sclerectomy (triple procedure surgery, TS); combined phacoemulsification and anterior vitrectomy (double procedure surgery, DS); and filtering surgery (FS) in nanophthalmos with angle-closure glaucoma (NACG). METHODS: Retrospective cohort study. Forty patients (44 eyes) diagnosed with NACG who underwent TS, DS, and FS were included. All eyes in the TS group and seven (47%) eyes in the DS group also underwent goniosynechialysis during the surgery. The main outcome measures (intrao

Novel combination therapy reduces subconjunctival fibrosis after glaucoma filtration surgery in the rabbit model.

BACKGROUND: Glaucoma filtration surgery (GFS) is limited by subconjunctival, episcleral and scleral fibrosis sealing the trabeculectomy and scarring the filtering bleb. Mitomycin-C (MMC) is commonly applied intraoperatively to the subconjunctival and/or intrascleral space to reduce scarring and promotes GFS success but is associated with postoperative scleral melting and bleb leaks. IP-10 peptide (IP-10p), an ELR-negative CXC chemokine mimetic and inhibitor of fibroblast function, may be an alternative or adjunct to current postoperative GFS treatments. This study sought to determine if IP-10p…

Long-term follow-up of intraocular pressure and pressure-lowering medication in patients following Excimer laser trabeculotomy.

BACKGROUND: Excimer laser trabeculotomy (ELT) is a minimally invasive procedure to lower the intraocular pressure (IOP) via a photo-ablative laser that is applied to the trabecular meshwork. With this procedure, it is possible to improve the outflow of the aqueous humor. Until now, a limited number of studies examining mostly relatively small sample sizes with midterm follow-up exist. We therefore present the analysis of a large ELT cohort in a long-term follow-up. METHODS: We recorded data from 580 patients who underwent ELT or combined ELT with cataract surgery at our institution from Novem

[Status of Selective Laser Trabeculoplasty (SLT)].

Purpose Worldwide, in recent years, selective laser trabeculoplasty (SLT) has developed into a very successful therapy for glaucoma. Material and Methods The article provides a review of the literature from PubMed and clinical experience. Results SLT is equally efficacious to argon laser trabeculoplasty (ALT) and to a first-line glaucoma medication. SLT can be used in most common forms of open-angle glaucoma and ocular hypertension. In contrast to ALT, SLT is repeatable and requires much less energy. SLT has been reported to be also successful in some rare forms of ocular hypertension/glaucoma

Recurrent symptomatic intraocular pressure spikes during hemodialysis in a patient with unilateral anterior uveitis.

BACKGROUND: The relationship between intraocular pressure (IOP) changes and hemodialysis has been evaluated for several decades. However, no … equent painful IOP spikes in the left eye. At the end of hemodialysis, IOP averaged ~40 mmHg. After trabeculectomy with mitomycin C in the left eye, his IOP stabilized in the low-teens (range, 10-14 mmHg) and no painful IOP spikes occurred during hemodialysis over the first postoperative year. CONCLUSION: We present a case of recurrent painful IOP spikes during …

Results of the modified trabeculotomy in the treatment of primary congenital glaucoma.

PURPOSE: To evaluate the effect of trabeculotomy employing probes with curvatures more closely corresponding to variable course of Schlemm's canal. METHODS: Forty-six children with primary congenital glaucoma who underwent trabeculotomy with newly designed probes between 1990 and 1998 were evaluated retrospectively. One set of the newly designed instruments consists of 3 pairs of trabeculotomy probes, the curvatures of which correspond to corneal diameters of 10, 12, and 14 mm. The criteria for surgical success were intraocular pressure (IOP) below 21 mmHg and simultaneous absence of cup to d

[Targetted ablation of the trabecular meshwork with excimer laser in primary open-angle glaucoma].

The trabecular meshwork and the inner wall of Schlemm's canal constitute 75% of the outflow resistance of the aqueous. Attempts to improve the outflow facility with laser beams have not been satisfactory because they produced thermal effects with coagulation of the trabecular meshwork. With the excimer laser tissue can be removed with minimal thermal effects and necrosis. This results in minimal scar formation. With these qualities the excimer laser lends itself very well for ablation of the trabecular meshwork. It should be possible to create an open connection between the anterior chamber an

Side effects of antiglaucomatous drugs on the ocular surface.

An increasing number of studies, both experimental and epidemiologic, have shown that filtering glaucoma surgery has progressively become less effective than initially described. Of a number of risk factors for failure, duration and number of antiglaucoma drugs before surgery seem to play a critical role, and high accumulated antiglaucoma topical treatments significantly reduce success rates. Histopathologic studies have confirmed that topically applied drugs may exert toxic effects to the corneoconjunctival surface and induce chronic infraclinical inflammation, as shown by the presence of imm
