OBJECTIVE: This study analyzed the clinical and echocardiographic late outcomes of surgical septal myectomy in patients with obstructive hypertrophic cardiomyopathy (OHCM).
METHODS: We examined, retrospectively, 34 consecutive adult patients (age 55.7±15.2 years) with OHCM operated on in our institution from 1988 to 2008. Only four (11.8%) patients had family history of OHCM. Nine (26.5%) patients were in New York Heart Association (NYHA) funcional class IV. Thirty (88.2%) patients had solely OHCM, and four (11.8%) had OHCM associated with coronary insufficiency. The surgical technique used i

Current pharmacotherapy for hypertrophic cardiomyopathy (HCM) is not disease-specific and has suboptimal efficacy, often necessitating interventional treatment. EXPLORER-HCM was a phase 3, randomized, double-blind, placebo-controlled, multicenter clinical trial investigating the effects of mavacamten, a first-in-class selective cardiac myosin inhibitor, in patients with HCM, left ventricular outflow tract obstruction (LVOTO) and New York Heart Association (NYHA) class II or III symptoms. The primary endpoint was defined as either a ≥1.5 ml/kg/min increase in peak oxygen consumption (pVO₂) and

OBJECTIVE: Since April 2010, our institution has repaired complete atrioventricular septal defects (CAVSDs) with a two-patch technique utilizing CorMatrix extracellular material. This material is potentially an attractive patch because of its theorized eventual integration with the host tissue. We sought to analyze initial outcomes of CAVSD repair with CorMatrix.
METHODS: Data were collected on consecutive pediatric (age <18) patients receiving two-patch CAVSD repairs with CorMatrix at a single institution from April 2010 to July 2014. Baseline and perioperative characteristics were evaluated

A 57-year-old woman with a history of pulmonary atresia with ventricular septal defect and hypoplastic pulmonary arteries was referred for a fifth redo sternotomy for right ventricular outflow conduit replacement. Traditional pulmonary valve replacement was aborted during the procedure because of heavy calcification, and the pulmonary valve was replaced with a SAPIEN XT stent valve under direct vision. We believe this is the first application of a balloon-expandable stent valve in the pulmonary valve position under direct vision. It provides a novel therapeutic approach to recurrent pulmonary

BACKGROUND: Conventional risk factors for sudden cardiac death (SCD) justify primary prevention through implantable cardioverter-defibrillator (ICD) implantation in hypertrophic cardiomyopathy (HCM) patients. However, the positive predictive values for these conventional SCD risk factors are low. Left ventricular outflow tract obstruction (LVOTO) and midventricular obstruction (MVO) are potential risk modifiers for SCD. The aims of this study were to evaluate whether an elevated intraventricular pressure gradient (IVPG), including LVOTO or MVO, is a potential risk modifier for SCD and ventricu

OBJECTIVE: Mitral regurgitation (MR) represents an important feature in patients with hypertrophic cardiomyopathy (HCM) due to left ventricular outflow tract (LVOT) obstruction and mitral valve systolic anterior motion (SAM). Mitral valve anatomical variants associated with HCM also contribute to the severity of MR. The aim of this study is to evaluate MR severity and its correlation with different parameters in patients with HCM using cardiac magnetic resonance imaging (cMRI).
PATIENTS AND METHODS: 130 patients with HCM underwent cMRI. Parameters assessed for the quantification of MR severit

Pulmonary arterial hypertension (PAH), documented in a significant portion of hypertrophic cardiomyopathy (HCM) patients, has been shown to adversely impact prognosis. In most HCM patients congestive symptoms are consistently elicited by exercise, thus suggesting the need for a provocative test to assess cardiac hemodynamics during effort. Combining cardiopulmonary exercise test (CPET) with echocardiography, we aimed to evaluate the presence of exercise induced pulmonary arterial hypertension (EiPAH), its role in functional limitation and its prognostic significance in a cohort of patients wit

To assess the value and limitations of single-plane transesophageal echocardiography in the evaluation of prosthetic aortic valve function, 89 patients (69 mechanical and 20 bioprosthetic aortic valves) were studied by combined transthoracic and transesophageal 2-dimensional and color flow Doppler echocardiography. In the assessment of aortic regurgitation, the transthoracic and transesophageal echocardiographic findings were concordant in 71 of 89 patients (80%). In 8 patients, the degree of aortic regurgitation was underestimated by the transthoracic approach; in each case the quality of the

History A 46-year-old woman was admitted to our hospital with decompensated congestive heart failure and pericardial effusion diagnosed at echocardiography. She had no family history of sudden cardiac death. She was born at term and experienced no cardiac events until 4 years of age, at which point she was hospitalized because of three syncopal episodes that were not related to exercise. Over the next 10 years, she experienced two additional episodes of syncope not related to exercise. She had another hospital admission at 12 years of age. Clinical examination did not reveal cyanosis or clubbi

PURPOSE: To evaluate the anatomical variations of dural venous sinuses in children with external hydrocephalus, proposing a radiological grading of progressive anatomic restriction to venous outflow based on brain phase-contrast magnetic resonance venography (PC-MRV); to evaluate the correlation between positional plagiocephaly and dural sinuses patency; and to compare these findings with a control group to ascertain the role of anatomical restriction to venous outflow in the pathophysiology of external hydrocephalus.
METHODS: Brain MRI and PC MRV were performed in 97 children (76 males, 21 f

OBJECTIVE: Chronic pulmonary regurgitation (PR) has deleterious effects on right ventricular (RV) function in repaired tetralogy of Fallot (ToF). However, there are little data regarding right ventricular outflow tract (RVOT) contractile dysfunction in response to chronic PR and on both RV and LV volumes and function.
METHODS: We retrospectively identified consecutive patients with PR who were referred for magnetic resonance imaging quantification of "free PR" detected on echocardiography between 2003 and 2008. Patients had ToF and a transannular patch procedure (n = 30, 25.1 ± 1.2 years) or

BACKGROUND: Arterial level repair is considered the most appropriate procedure for transposition of the great arteries. This report describes our experience with the arterial switch operation over the past decade.
METHODS AND RESULTS: From January 1991 to January 2001, a total of 299 patients underwent an arterial switch operation for transposition of the great arteries or double-outlet right ventricle. Group I (n=169, 56.5%) comprised patients with transposition of the great arteries in whom the ventricular septum was essentially intact. Group II patients (n=130, 43.5%) had transposition of

Microdeletions impacting 15q11.2 breakpoint (BP) 1 to BP2, adjacent to the Prader-Willi critical region, have previously described neuropsychiatric associations, with potential low penetrance presentations of congenital heart disease (CHD) also identified. Four highly conserved genes in the region (NIPA1, NIPA2, CYFIP1, and TUBGCP5) have been proposed to be involved in cardiac development, yet further exploration is needed to better define cardiac phenotypes of patients with copy number variants (CNVs) in the 15q11.2 BP1-BP2 region. This study examines cardiac defects in 50 patients with micro

Cardiomyopathies (CMP) comprise a group of heterogeneous heart muscle diseases that have molecular genetic causes and cannot be adequately explained by other cardiovascular diseases. The diagnosis and treatment of CMPs have made significant progress in recent decades, which is reflected in novel specific cardiomyopathy guidelines of the European Society of Cardiology (ESC) published in 2023. A patient-centered approach combines multimodal diagnostics, such as echocardiography, magnetic resonance imaging, genetic testing and biopsy to enable precise etiological classification and thus personali

BACKGROUND: Transcatheter mitral valve replacement (TMVR) is emerging as a feasible alternative to conventional surgical intervention in patients at high surgical risk, also those with severe mitral annular calcification (MAC). However, the presence of a previously implanted self-expanding transcatheter aortic valve prosthesis introduces considerable technical difficulties, especially due to the risk of left ventricular outflow tract obstruction.
CASE SUMMARY: To our knowledge, it is among the very few reported cases of combined percutaneous interventions with a self-expanding valve in the ao

Subaortic membrane (SAM) is a subtype of left ventricular outflow obstruction, rarely seen in adults. In some cases, SAM may be associated with other congenital defects. The association of patent ductus arteriosus (PDA) and SAM is the rarest, especially in adult patients. The management of these patients is delicate in terms of diagnosis, timing, and the appropriate surgical technique. A 34-year-old patient with a known aortic heart murmur presented with exertional syncope. She was referred to our hospital for aortic valve replacement due to symptomatic aortic stenosis. INTRODUCTION: Severe mitral regurgitation (MR) poses significant therapeutic challenges, especially in patients at high surgical risk or unsuitable for transcatheter edge-to-edge repair (TEER). Transcatheter mitral valve replacement (TMVR) has emerged as a viable alternative, with the EVOQUE Eos system representing a next-generation, fully retrievable, transseptal TMVR platform tailored to address prior limitations in safety, anatomical fit, and procedural control.
AREAS COVERED: This review outlines the design features, procedural considerations, and early clinical performance of the EVOQUE

Mitral valve disease, particularly in the context of extensive mitral annular calcification (MAC), poses significant challenges for traditional surgical management. Transcatheter mitral valve replacement (TMVR) has emerged as a promising alternative for high-risk and inoperable patients, driven by rapid advancements in valve technology, imaging techniques, and procedural strategies. Nevertheless, complications such as paravalvular leak (PVL), left ventricular outflow tract (LVOT) obstruction, and hemolysis remain obstacles to optimal outcomes, particularly in patients with complex annular anat
