We measured apparent accommodation in 42 pseudophakic eyes (34 patients) after implantation of posterior chamber intraocular lenses. The mean apparent accommodation was 2.03 +/- 1.03 diopters. The mean accommodative power of 16 phakic eyes used as controls was 2.91 +/- 1.29 diopters. The diameter of the pupil appeared to be the most important factor in apparent accommodation--the smaller the pupil, the greater the apparent accommodation. Apparent accommodation was inversely proportional to the pupillary diameter. There was no correlation, however, between apparent accommodation and corrected v

A change in the indications for cataract surgery might be one of the many reasons for the steady increasing number of cataract extractions. … ange, but we do operate more second eye cataracts now, 41% against 25% in 1970, which means that monaphakia is in itself considered a stronger indication for surgery in the second eye than it was 10 years ago. Acknowledgment of the difficulties of being monaphakic, refinements of extraction techniques and increased requirements for better vision among the …

We treated eight neonates who had total, monocular, congenital cataracts with surgery, occlusion, and contact lenses. Visual results in all eight patients were good. In five patients visual acuities improved to 6/9 (20/30) or better in the aphakic eye. In three patients visual acuities improved to 6/24 (20/80) or better. Problems with contact lenses probably accounted for the poorer results in two of these three patients. Binocularity was not demonstrated in any of our patients.

Thirteen keratophakia patients were followed for 13 to 35 months. Eighteen cases of hyperopic keratomileusis or hyperopic keratomileusis using preserved corneal tissue were followed for 2 to 30 months. All 13 patients who underwent keratophakia achieved 20/50 or better acuity. All seven patients having hyperopic keratomileusis achieved 20/60 or better acuity. Eight of the 11 patients who had hyperopic keratomileusis using donor corneal tissue achieved 20/60 or better acuity. The preservation of lenticules after lathing did not adversely affect the correlation between predicted and observed dio

The critical period in which human visual acuity may be lost after visual deprivation can be deduced by studying children with amblyopia which follows uncomplicated unilateral cataract and the subsequent aphakic blur. The visual acuity at the first contact lens fitting increases linearly on a log scale as a function of the age at onset between 3 and 10 years. Correction before 4 months of age produces less visual loss but we cannot specify the function within this period. If the eye is deprived between 6 and 30 months of age finger counting is the best visual acuity achieved. Deprivation comme

The unopened eye maintains a relatively stable spherical contour due to the expansile influence of the intraocular pressure. When the eye is opened this expansile pressure is lost and some degree of collapse of the scleral shell ensues. In eyes with a relatively flaccid sclera an anterior segment incision may induce significant reduction in the volume of the posterior segment of the globe. During intracapsular cataract extraction on such eyes, scleral collapse can cause anterior displacement of the lens and iris when the eye is opened and vitreous loss as soon as the lens is extracted. Scleral

In a series of 553 consecutive retinal detachments, 29% (120) were due to lattice degeneration. Forty-five percent of these were due to atrophic holes in the lattice degeneration and 55% were due to tears caused by traction posterior to or at the end of a patch of lattice. In phakic patients, retinal detachments due to atrophic holes were most common in young myopes. Detachments due to traction tears were seen in older, less myopic patients. The incidence of massive periretinal proliferation was less (5%) in detachments due to lattice degeneration than in detachments not due to lattice degener

PURPOSE: To describe a novel technique for capsular bag reopening and secondary in-the-bag intraocular lens (IOL) implantation in aphakic eyes after vitreoretinal surgery and intraocular tamponade.
METHODS: We enrolled 14 eyes of 14 patients who underwent primary vitreoretinal surgery with silicone oil tamponade for rhegmatogenous retinal detachment between September 2018 and September 2019. The novel technique was used for capsular bag reopening and foldable single-piece IOL implantation. Patients were followed up at least 24 weeks with routine ophthalmic examinations, corneal endothelial ce

PURPOSE: To compare complication rates, visual acuity, and refractive outcomes of secondary intraocular lens (IOLs) implantation.
SETTING: Department of Ophthalmology, Cullen Eye Institute, Baylor College of Medicine, Houston, Texas, USA.
DESIGN: Retrospective case series.
METHODS: All secondary IOLs placed by the anterior segment service were reviewed. Preoperative data, operative reports, and data from each subsequent postoperative visit were evaluated. Patients were divided into 5 groups based on the final IOL position: (1) sulcus with optic capture, (2) sulcus without optic capture, (3)
