Impaired dynamic cerebral autoregulation predicts silent ischemic lesions after carotid artery stenting: A hemodynamic biomarker beyond plaque morphology.

Carotid artery stenting (CAS) induces hemodynamic disturbances that may trigger silent cerebral ischemia (ASILs), though the relative contributions of impaired cerebral autoregulation (dCA) versus plaque characteristics remain unclear. In this prospective cohort study, we evaluated 59 patients with severe carotid stenosis undergoing CAS, assessing dCA via transcranial Doppler (TCD) and transfer function analysis (TFA) pre- and post-procedurally, with postoperative MRI to detect ASILs. ASILs occurred in 30.5% (18/59) of patients and were associated with dyslipidemia (P = 0.010) and low-echo pla

The Evolution of Carotid Surgery.

This review traces the evolution of carotid surgery from its ancient roots to modern practice. The first significant appreciation of the carotid arteries' role in cerebral ischemia was made by Johann Jakob Wepfer in his 1,658 book on apoplexy, which described the separate blood supply to each cerebral hemisphere by the carotid arteries. The 16th-19th centuries saw initial attempts at carotid ligation, while the 20th century marked significant advancements. The first carotid endarterectomy (CEA) by DeBakey, Cooley, and Eastcott in the 1950s paved the way for a new technique in carotid surgery a

Tailored Strategies in Carotid Artery Stenting to Avoid Periprocedural Complications.

Carotid artery stenting (CAS) has been widely accepted as a valuable therapeutic alternative to carotid endarterectomy (CEA) for high-grade carotid stenosis. Because carotid revascularization including CAS is usually performed in patients with minimal or no neurological deficits, utmost care should be taken to avoid periprocedural complications. The major concerns associated with CAS are embolic stroke, hyperperfusion syndrome (HPS), and perioperative myocardial infarction.Plaque characteristics, cerebral blood flow (CBF) in the affected cerebral hemisphere, and concomitant coronary artery dis

Management of distal choroidal artery aneurysms in patients with moyamoya disease: report of three cases and review of the literature.

Prevention of rebleeding plays an important role in the treatment of hemorrhagic moyamoya disease, because rebleeding results in high mortal … r moyamoya disease accompanied with distal choroidal artery aneurysms. Our experience suggests that cerebral revascularization combined with obliteration of the complicated distal aneurysm in the same session is a possible treatment.

Magnetic resonance angiography of cerebral arteries after neonatal venoarterial and venovenous extracorporeal membrane oxygenation.

BACKGROUND AND PURPOSE: In newborns with severe respiratory failure, extracorporeal membrane oxygenation (ECMO) has been proven to improve survival. With venoarterial ECMO, the right common carotid artery is ligated, whereas with venovenous ECMO, this carotid artery remains intact. The authors present the magnetic resonance angiography (MRA) evaluation of cerebral hemodynamics in a case of venoarterial and a case of venovenous ECMO. METHODS: With 2D phase-contrast MRA flow volume (mL/min), measurements are obtained of the brain feeding arteries and draining veins. Time-of-flight and flow-dire

[Intracranial hemorrhage observed in the cases of cervical internal carotid artery occlusion associated with moyamoya disease-like vessels].

We report on two peculiar cases of intracranial hemorrhage due to the rupture of moyamoya disease-like vessels associated with unilateral internal carotid occlusion at its origin. The first case is 44-year-old male showing intraventricular hemorrhage associated with right internal carotid occlusion. The second case is 58-year-old female presenting sub-arachnoid hemorrhage associated with left internal carotid occlusion. Although both cases showed the unique appearance of collateral flow resembling moyamoya disease, they were not classified as moyamoya disease nor unilateral Moyamoya-like state

[An Aicardi-Goutières syndrome associated with a quasi-Moyamoya by a biallelic mutation in SAMHD1].

SAMHD1 is one of seven known genes responsible for Aicardi-Goutières syndrome. It has the particularity to associate to this rare pediatric encephalopathy with autoimmune manifestations, a cerebral vasculopathy type Moyamoya. This condition has only been recently reported, less than fifty times in the literature. Our clinical case is a 11 year old boy from an inbred union whose clinical diagnosis confirmed genetically and followed by a review of current data determined an ad hoc management, presently described. He underwent indirect neurosurgical revascularization by a multiple burr hole techn

Headache attack followed by rapid disease progression in pediatric moyamoya disease--how should we manage it?

CASE REPORT: A 4-year-old female was presented at our hospital with frequent right frontal headache attack. She was diagnosed with moyamoya disease and was conservatively followed up. One year later, the frequency of headache gradually decreased. However, follow-up MR imaging revealed that the disease stage markedly progressed in the right side and cerebral infarction occurred in the temporal lobe with atrophy of the right frontal lobe. She underwent direct and indirect revascularization on the right side. CONCLUSION: Aware of this case, we would like to emphasize that headache may be one sub

Frozen elephant trunk technique in redo aortic arch surgery: a step-by-step approach.

Frozen elephant trunk (FET) is an established treatment modality for complex aortic arch and proximal descending aortic pathologies. While patients with limited life expectancy are more frequently considered for endovascular treatment options, the FET remains a sustainable platform, especially for younger post-dissection and heritable thoracic aortic disease (HTAD) patients. These patients frequently have undergone previous transsternal operations. A substantial portion require concomitant aortic root or other cardiac procedures. In this manuscript we describe our technique of redo valve spari

Crossover Versus Ostial Single-stent Implantation for Ostial Left Circumflex Artery Lesions: The Multicenter CROSS-LCX Registry.

BACKGROUND: To date, the optimal revascularization strategy for patients with ostial left circumflex artery (LCX) lesions has not been established. In this study we sought to assess the cardiovascular outcomes of the crossover stent implantation (CSI) and ostial stent implantation (OSI) for the ostial LCX lesions under long-term follow-up. METHODS: This large-scale, multicenter (n = 12), observational retrospective study included 414 patients (290 [70%] men and 124 women, mean age 64.95 ± 11.73 years) who underwent PCI with CSI or OSI for ostial LCX lesions between 2014 and 2025. Outcomes After Thrombectomy for Primary and Secondary Medium Vessel MCA Occlusions: a Nationwide Registry Study.

BACKGROUND: Medium vessel occlusions (MeVO) can be either isolated events (primary), or secondary to thrombus migration from a large vessel occlusion to a medium-sized vessel. Outcomes following endovascular thrombectomy (EVT) in the middle cerebral artery (MCA) may differ between primary and secondary MeVOs. This study aimed to assess the association between primary/secondary MeVOs and clinical outcomes following EVT in a nationwide patient cohort. METHOD: Patients undergoing EVT were included in two Swedish quality registries. Secondary MeVO was defined as distal migration of a solitary thr

Deep hypothermic circulatory arrest for complex cerebral aneurysms: lessons learned.

OBJECTIVE: Deep hypothermic circulatory arrest is a useful adjunct for treating complex aneurysms. Decreased cerebral metabolism and resultant ischemic tolerance create an environment suitable for devascularizing high-risk lesions. However, the advent of modern imaging modalities, innovative cerebral revascularization strategies, and the emergence of endovascular stenting and coiling limit the number of aneurysms requiring this surgical intervention. We present 66 patients with intracranial aneurysms who underwent surgical clipping under deep hypothermic arrest and attempt to identify patients…

Postoperative posterior MCA territory infarction following single-barrel STA-MCA bypass with hemodynamic territory mismatch in moyamoya disease: A case report.

The success of superficial temporal artery-to-middle cerebral artery (STA-MCA) bypass in moyamoya disease (MMD) depends not only on anastomotic patency but on whether the revascularized territory corresponds to the most hemodynamically compromised region. We report a case of postoperative posterior MCA territorial infarction attributable to hemodynamic territory mismatch. A 47-year-old woman with bilateral MMD presented with progressive left hemiparesis. Preoperative computed tomography (CT) perfusion demonstrated markedly reduced cerebral blood flow (CBF) and diffuse mean transit time (MTT) p

Changes in arterial flow velocity and pulsatility following endarterectomy for symptomatic high degree carotid artery stenosis: insights from the Carotis7T Study.

OBJECTIVE: Patients with symptomatic high degree carotid artery stenosis often exhibit a reduced perfusion pressure in the cerebral vasculature in absence of sufficient collaterals. The aim of this research was to evaluate changes in mean blood flow velocity and pulsatility within the cerebral perforating arteries following carotid endarterectomy (CEA). METHODS: Fifteen patients with symptomatic high degree (>50 %) carotid artery stenosis were included in the monocentre prospective observational Carotis7T study. All patients underwent 7T magnetic resonance imaging (MRI) one day prior to CEA a

Assessment of Hemodynamic Compromise Using Computed Tomography Perfusion in Combination with 123I-IMP Single-Photon Emission Computed Tomography without Acetazolamide Challenge Test.

OBJECTIVES: The acetazolamide challenge test in conjunction with 123I-IMP single-photon emission computed tomography (SPECT) is a known method of assessing cerebrovascular reserve capacity. In this study, we investigated whether CT perfusion in combination with resting state 123I-IMP SPECT could be used instead of the acetazolamide challenge test to evaluate hemodynamic compromise in patients with atherosclerotic occlusive disease. METHODS: Twenty consecutive patients with unilateral internal carotid artery or middle cerebral artery steno-occlusive disease were enrolled. 123I-IMP SPECT was pe
