Differences in mortality characteristics in neonates with Down's syndrome.

OBJECTIVE: Neonates with Down's syndrome (nDS) may have multiple medical issues that place them at increased risk for mortality during the newborn period. Goal of this study was to determine if there are differences in baseline characteristics, medical complications or procedures performed during hospitalization between nDS who survived versus those who died during initial hospitalization. STUDY DESIGN: Data from 2000 to 2014 were reviewed using the Pediatric Health Information Systems (PHIS) database on all DS patients admitted to the hospital <30 days postnatal life. Baseline demographics,

Atrial Fibrillation in Patients with Congenital Heart Disease.

Advances in surgical techniques have led to the survival of most patients with congenital heart disease (CHD) up to their adulthood. During their lifetime, many of them develop atrial tachyarrhythmias due to atrial dilatation and scarring from surgical procedures. More complex defects and palliative repairs are linked to a higher incidence and earlier occurrence of arrhythmias. Atrial fibrillation (AF) is common in patients who have atrial septal defects repaired after age 55 and in patients with tetralogy of Fallot repaired after age 45. Patients with dextrotransposition of the great arteries

Racial/ethnic disparities in timing of death during childhood among children with congenital heart defects.

BACKGROUND: Infants with congenital heart defects (CHD) have increased risk of morbidity and mortality. Little is known about racial/ethnic differences in timing of death during childhood. Our intent was to investigate racial/ethnic differences in mortality for CHDs during specific time periods in childhood. METHODS: Texas Birth Defect Registry data were used for a retrospective cohort study with 30,015 singleton infants with a CHD, born January 1, 1999, to December 31, 2007, to non-Hispanic (NH) white, NH-black, or Hispanic women. Texas Birth Defect Registry data were linked to Texas death r

One and a half ventricle repair in adults: postoperative hemodynamic assessment using phase-contrast magnetic resonance imaging.

BACKGROUND: One and a half ventricle repair (1½ repair) strategy has been used for patients with a hypoplastic or dysfunctional right ventricle (RV), or both. We sought to assess the postoperative hemodynamics of 1½ repair using phase-contrast magnetic resonance imaging (PC-MRI). METHODS: Ten adults, 9 with Ebstein's anomaly and 1 with tricuspid stenosis, underwent 1½ repair (median age at operation, 42.4 years). The azygos vein was left open in all patients on 1½ repair to prevent severe postoperative central venous hypertension. Postoperative PC-MRI studies were performed to measure blood f

[Evaluation of adult congenital heart diseases].

OBJECTIVES: Data on adult congenital heart diseases (CHD) are limited in Turkey. We evaluated the types and clinical features of CHDs in adult patients followed-up in our center. STUDY DESIGN: This study included 200 adult patients (age >16 years) who were diagnosed as having CHD in our clinic between April 2006 and January 2009. The patients were evaluated in three groups based on the complexity of adult CHD (simple, moderate, or great) according to the most recent ACC/AHA guidelines. RESULTS: There were 121 females (60.5%) and 79 males (39.5%). The mean age was 34.7+/-13.4 years (range 16

Arrhythmia surgery in association with complex congenital heart repairs excluding patients with fontan conversion.

Surgical arrhythmia therapy may be performed for patients failing the catheter ablation approach or incorporated into repair of complex congenital heart disease. Variations in atrial and ventricular anatomy that may limit the catheter approach can be directly addressed surgically assuring lesion depth and continuity of anatomic lines of block. Between July 1992 and August 2002, we performed arrhythmia surgery on 34 patients for refractory atrial (n = 29) or ventricular (n = 5) arrhythmias. Not included in this series are patients who had arrhythmia surgery during Fontan conversion. The majorit

Late results of systemic atrioventricular valve replacement in corrected transposition.

From December 1964 to October 1993, 40 patients (aged 5 months to 70 years, mean 21.8 years, median 13.6 years) with corrected transposition and systemic atrioventricular valve insufficiency underwent replacement (n = 39) or repair (n = 1) of the systemic atrioventricular valve. Thirty-nine patients had situs solitus and 1 had situs inversus. Associated anomalies included Ebstein's malformation of the systemic atrioventricular valve (n = 22), ventricular septal defect (n = 19), and pulmonary stenosis (n = 14). Preoperatively, 16 patients (40.0%) had complete heart block and 27 patients (67.5%)

Ebstein's malformation. surgical treatment and results.

Ebstein's malformation can be defined as an anomaly of the tricuspid valve existing in the setting of a right ventricular dysfunction. The technique introduced by Carpentier in 1980 is based on the concept of mobilization of the restrictive anterosuperior leaflet associated with a longitudinal plication of the inlet component of the right ventricle. From January 1980 to December 1999, 142 patients underwent surgery. The mean age was 25 +/- 15 years (1-65). Cyanosis was present in 48% and associated lesions in 64% of the patients. Patients were classified using a functional approach according t

A radiographic and echocardiographic approach to cyanotic congenital heart disease.

An updated, systematic approach to the diagnosis of cyanosis in the infant or child has been presented. It involves the use of clinical data (to establish the presence of cyanosis), radiographic information (to assess lung parenchyma, pulmonary vascularity, and certain key, border-forming, cardiac structures), and M-mode and cross-sectional echocardiographic data (the source of detailed intracardiac anatomic information). Specific radiographic and echocardiographic features of the 10 most common cyanotic congenital heart lesions have been presented. Using this approach, one should be able to d

Tricuspid valve replacement through a left atriotomy and transseptal approach in a congenitally malformed heart.

More and more children with congenital heart disease are surviving into adulthood. These patients are forcing adult congenital cardiac surgeons to develop innovative approaches to correct their complex anatomy and physiology. This report describes a patient with a congenitally malformed heart necessitating a novel approach to access the tricuspid valve--a left atriotomy and transseptal incision. Three-dimensional preoperative imaging allowed for successful surgical planning.

Classification of ventricular pre-excitation. Vectorcardiographic study.

In a study of 45 cases of ventricular pre-excitation, 19 were classified as type A and 20 as type B according to Rosenbaum's criteria, which depend on the polarity of the major deflections in the right praecordial leads and not, as is commonly thought, on the direction of the delta vector. Six cases that could not be classified as type A or type B were termed intermediate. Vectorcardiograms were recorded from 29, and these showed a wide but continuous range of values for both the delta and the main QRS vectors in all three planes. Any classification based on these features must, therefore, dep

Long-term survival after tricuspid valve replacement. Results with seven different prostheses.

The experience with tricuspid valve replacement (TVR) with seven different prostheses, alone or combined with replacement of other valves, in 73 patients (64 rheumatic and nine nonrheumatic) between 1964 and March, 1975, at the University of Alberta Hospital has been reviewed. Early and late mortality rates in rheumatic patients were 41 and 23 percent, respectively (36 percent being alive after a mean of 5.6 years), compared to 33 and 11 percent, respectively, in the nonrheumatic patients (56 percent being alive after a mean of 2 years). Of all survivors, 88 percent were functionally improved.
