Unusual traumatic midthoracic spondyloptosis and its surgical management: case report.

Posttraumatic spondyloptosis develops as a result of complete subluxation of the vertebral bodies and causes complete transection of the spinal cord. Severe trauma-related spondyloptosis of the upper-mid thoracic region is a rare form of spinal trauma. Traumatic midthoracic spondyloptosis is quite rare, and radiology plays an important role in the diagnosis and treatment of this condition. Surgical reconstruction and stabilization are required for early mobilization and rehabilitation of patients with this injury. Here, we report the clinical features, radiographic findings, and management of

[Knife injury to the thoracic aorta and spinal cord. Report on an"almost-mistake"].

We report the surgical treatment of a thoracic knife injury with lesions of the aorta and the myelon. An initially incomplete revision of the wound nearly resulted in a critical situation. Complete revision of a wound is obligatory in a penetrating injury, when a relevant structure is possibly injured.

Spontaneous rupture of an intercostal artery in a patient with neurofibromatosis type 1.

A 48-year-old man with neurofibromatosis type 1 (NF1) presented with a right pleural effusion. A 3D computed tomography (CT) angiogram showed an aneurysm of the right 11th intercostal artery. He had no history of chest trauma so we diagnosed a spontaneous rupture of the aneurysm causing a massive effusion. We opened his pleural cavity and found lacerated pleura and active bleeding in the posterior 11th intercostal space. After controlling the active bleeding, we treated a persistent oozing from the region of the 10th-12th vertebrae with pressure hemostasis by absorbable oxidized cellulose pack

[Respiratory complications after oesophagectomy for cancer].

Surgery is the cornerstone of treatment for resectable tumours of the oesophagus. Recent advances of surgical techniques and anaesthesiology have led to a substantial decrease in mortality and morbidity. Respiratory complications affect about 30% of patients after oesophagectomy and 80% of these complications occur within the first five days. Respiratory complications include sputum retention, pneumonia and ARDS. They are the major cause of morbidity and mortality after oesophageal resection and numerous studies have identified the factors associated with these complications. The mechanisms ar

Comparative outcomes of central venous access via open jugular and ultrasound-guided subclavian approaches in low-weight pediatric patients: a study with secondary outcomes.

BACKGROUND: The necessity of this study stems from the lack of comparative evidence on the outcomes of central venous catheterization via th … n in the subclavian group (41.19 ± 13.8 vs. 33.15 ± 12.12 min; P = 0.04). No cases of pneumothorax, hemothorax, bleeding, arrhythmia, occlusion, infection, or death occurred in either group. Hematoma developed only in the jugular group (6 vs. 0; P = 0.02), and all cases were associated with a procedure duration greater than 35 min (P = 0.041). In the subclav …

The application of focused assessment with sonography for trauma in resource-limited settings: a scoping review.

BACKGROUND: Trauma represents a leading cause of mortality and disability in resource-limited settings. However, access to advanced imaging modalities is severely constrained, limiting timely and accurate diagnosis as well as clinical decision-making. Focused assessment with sonography for trauma (FAST) offers a portable, non-ionizing, and cost-effective bedside imaging solution with significant potential for clinical application. It enables rapid identification of life-threatening hemoperitoneum or hemothorax, thereby guiding immediate surgical intervention or preventing unnecessary patient t…

Lung transplantation studies showed that ECMO can be integrated into perioperative pathways, but hemothorax, transfusion, thromboembolism, and anticoagulation strategy remained central safety issues. Conclusions: Invasive procedures during adult ECMO are feasible in selected patients and experienced centers, but procedural safety varies markedly by proced …

Analysis of risk factors for poor healing and long-duration pain in conservative treatment of rib fractures.

Rib fractures are a common injury following blunt chest trauma, accounting for approximately 10% of all traumatic injuries and up to 50% of blunt chest trauma cases. These fractures are associated with a high risk of complications, such as pneumothorax, hemothorax, and pulmonary infections, and can significantly impact respiratory function. This study analyzes the risk factors for poor healing and long-duration pain in the conservative treatment of rib fractures, providing a reference for clinicians in choosing conservative treatment and formulating treatment plans. A retrospective analysis wa…

Isolated low-grade solid organ injuries in children following blunt abdominal trauma: Is it time to consider discharge from the emergency department?

INTRODUCTION: Acute intervention (AI) for solid organ injury (SOI) is rare in hemodynamically stable children. Pediatric guidelines recommend admission with follow-up laboratories, even for low-grade injuries. METHODS: Data sets from two large multicenter prospective observational studies were used to analyze a cohort of children (age, <17 years) with grade I to III SOI following blunt abdominal trauma. Children with hollow viscus injuries were excluded. Patients were divided into (a) those with or without other major injuries (OMIs) (traumatic brain injury, hemothorax or pneumothorax, pelvic…

Cardiac tamponade after radiofrequency ablation for hepatocellular carcinoma: Case report and literature review.

RATIONALE: Radiofrequency ablation (RFA) is a safe and effective local treatment modality with a low complication rate and is commonly used to treat hepatocellular carcinoma (HCC). The clinical outcome of RFA may be closely related to the location, size, and shape of index tumors, and major complications, including hemorrhage, liver abscess, infarction, visceral organ perforation, hemothorax, pneumothorax, tumoral seeding, and hepatic failure. Cardiac tamponade is a rare and serious life-threatening complication associated with RFA. To date, a review of the medical literature reported 5 cases …
