OBJECTIVE: Due to the rarity and unknown pathogenesis of SAPHO syndrome, there is no consensus on its treatment. This study aimed to evaluate the effectiveness of Iguratimod-based therapy-monotherapy or combined with alendronate-for treating SAPHO syndrome patients.
METHODS: We conducted a prospective cohort study at Beijing Jishuitan Hospital from August 2022 to August 2024. Consecutively recruited patients diagnosed with SAPHO syndrome received Iguratimod monotherapy (Group A), combination therapy with Iguratimod and alendronate (Group B), or NSAIDs alone (Group C). Patients were followed f

OBJECTIVES: To assess serum interleukin-6 (IL-6) and interleukin-23 (IL-23) and their correlation with angiogenic cytokines and disease activity in ankylosing spondylitis (AS), psoriatic arthritis (PsA), and SAPHO syndrome.
PATIENTS AND METHODS: We studied 152 spondyloarthritis (SpA) patients: 69 PsA, 61 AS, 22 SAPHO, and 29 controls. We recorded age, sex, disease duration, and treatment. We assessed BASDAI, VAS, and PASI scores. Serum IL-6, IL-23, VEGF, EGF, FGFb, and FGFa levels were determined using ELISA. We estimated ESR and CRP.
RESULTS: Serum IL-6 and IL-23 levels were higher in SpA t

Hyperphosphatemic familial tumoral calcinosis (HFTC) is characterized by enhanced renal phosphate absorption, hyperphosphatemia, and tumor-like extraosseous calcifications due to inactivating mutations in FGF23 or associated proteins. Surgical excision is needed when low phosphate diet and phosphate binders are ineffective. Sporadic reports have supported acetazolamide use. We report on a 7-year-old African American boy who presented with severe HFTC requiring numerous surgical excisions. Tumors continued to appear and others reoccurred despite phosphate restriction and sevelamer carbonate. At

We report two cases, one 23 and one 27 year old man, with several years background of sternal and sternoclavicular joints pain, respectively. The laboratory tests were normal. Based on the clinical, radiologic and scintigraphic findings, SAPHO syndrome was diagnosed. Bone scintigraphy makes it possible to obtain a whole body scan in a single whole body scan, and can be used for differential diagnosis of other osteoarticular diseases and for evolutive control and therapeutic response.

We present a rare case of chronic infectious multifocal osteomyelitis with affectation symmetric in the both femures in a patient with spondyarthropathic secondary Inflammatory Bowel Disease. The diagnosis was confirmed with culture of the bone biopsy. The aim of this work is the revision of the Chronic Infectious Multifocal Osteomyelitis and the Chronic Recurrent Multifocal Osteomyelitis into SAPHO syndrome that both were the possibilities diagnoses.

Eight patients in three families had mental retardation, characteristic facies and hands, and skeletal changes; the clinical features suggested to us that they had a syndrome previously thought to represent two entities described by Lowry and associates and by Coffin and associates, respectively. New findings include skeletal, orodental, and dermatoglyphic abnormalities and histopathologic changes suggesting that the syndrome is a heritable disorder of connective tissue. Severe expression in males and transmission through mildly affected females suggest X-linked or sex-influenced autosomal dom

BACKGROUND: The diagnosis of equine sinonasal disorders has been greatly enhanced by using computed tomographic (CT) imaging.
OBJECTIVES: T … with sinus cysts, PEH, trauma and tumours (less purulent types of sinusitis). More severe grades of hyperostosis were associated with sinus cysts and tumours. Sinonasal tumours (odds ratio [OR] 0.03, 95% confidence interval [CI] 0.01-0.07) and PEH (OR 0.23, 95% CI 0.12-0.43) had a significantly lower likelihood of cure than primary sinusitis.

BACKGROUND/OBJECTIVE: Tricortical pedicle screw (TCPS) fixation has emerged as a biomechanically superior alternative to conventional unicortical/bicortical methods for spinal stabilization in high-risk populations, particularly elderly patients with osteoporotic or diffuse idiopathic skeletal hyperostosis (DISH)-related vertebral fractures. This review evaluates TCPS fixation's biomechanical advantages, clinical outcomes, and surgical considerations to address these challenges.
METHODS: A scoping review was conducted using the PRISMA scoping review checklist. PubMed was searched through Marc…

Craniometaphyseal dysplasia (CMD) is a rare genetic bone disorder, characterized by progressive thickening of craniofacial bones and flared metaphyses of long bones. Craniofacial hyperostosis leads to the obstruction of neural foramina and neurological symptoms such as facial palsy, blindness, deafness, or severe headache. Mutations in ANKH (mouse ortholog ANK), a transporter of small molecules such as citrate and ATP, are responsible for autosomal dominant CMD. Knock-in (KI) mice carrying an ANKF377del mutation (AnkKI/KI ) replicate many features of human CMD. Pyrophosphate (PPi) levels in pl…

BACKGROUND: Both vertebral bodies and posterior elements of the vertebrae (facet joints, FJ) can engage in bone formation in radiographic axial spondyloarthritis (r-axSpA). However, little is known about the specific structural lesions and progression patterns of FJs in r-axSpA.
OBJECTIVES: To identify specific lesions related to r-axSpA and to investigate the distinct progression patterns by comparing the FJ changes of r-axSpA with that of diffuse idiopathic skeletal hyperostosis (DISH), osteoarthritis (OA), and control group (CG).
DESIGN: Single-center, retrospective study. Longitudinal im…

A 25-year-old man presented with Touraine-Solente-Golé syndrome (primary pachydermoperiostosis), with an area of inflammatory dermatosis (12-month evolution) of the scalp at the cranial vertex. The patient presented with arthropathy, clubbing of the digits, diffuse periostosis, pachydermia of the hands and feet, and periosteal hyperostosis of the knee. Facial seborrhea and sebaceous gland hyperplasia were evident (Figure 1A and 1B and Figure 2A and 2B). Examination of the scalp revealed an erythematous pruritic plaque with erosions, crusts, and pustules, on which multiple tufts of 10 to 20 nor…

BackgroundSAPHO syndrome can involve the spine, along with vertebral fractures (VFs), and demonstrates osteoporosis and sarcopenia. However, the relationship between musculoskeletal factors with VFs in SAPHO patients is unclear.PurposeTo quantify bone and muscle loss in SAPHO syndrome and identify the factors associated with VFs.Material and MethodsThe study included 35 patients and 70 age- and sex-matched controls. The bone mineral density (BMD) of the lumbar spine (LS), femoral neck (FN), total hip (TH), the age- and sex-specific T- and Z-scores, and trabecular bone score (TBS) were evaluate
