Orthodontic-surgical treatment for severe skeletal class II malocclusion with vertical maxillary excess and four premolars extraction: A case report.

BACKGROUND: Patient satisfaction with facial appearance at the end of orthodontic camouflage treatment is very important, especially for skeletal malocclusion. This case report highlights the importance of the treatment plan for a patient initially treated with four-premolar-extraction camouflage, despite indications for orthognathic surgery. CASE SUMMARY: A 23-year-old male sought treatment complaining about his unsatisfactory facial appearance. His maxillary first premolars and mandibular second premolars had been extracted, and a fixed appliance had been used to retract his anterior teeth

Comparison of standard miniplates and locked miniplates in post-traumatic fracture stabilization.

BACKGROUND: Bicortical screws (lag and positional) or miniplates with monocortical screws are generally used for the rigid fixa-tion of the sagittal split ramus osteotomy (SSRO) in maxillofacial surgery. However, in this osteosynthesis method, the plate must be perfectly adapted to the bone to prevent misalignment of the bone segment and occlusal changes. In addition, it is necessary to prevent the position of the condyle in the mandibular fossa from changing after fixation. In recent years, locked miniplate systems have been used to overcome these complications. METHODS: The aim of this stud

Three-dimensional evaluation of soft tissue change gradients after mandibular setback surgery in skeletal Class III malocclusion.

OBJECTIVE: To evaluate whether mandibular setback surgery (MSS) for Class III patients would produce gradients of three-dimensional (3D) soft tissue changes in the vertical and transverse aspects. MATERIALS AND METHODS: The samples consisted of 26 Class III patients treated with MSS using bilateral sagittal split ramus osteotomy. Lateral cephalograms and 3D facial scan images were taken before and 6 months after MSS, and changes in landmarks and variables were measured using a Rapidform 2006. Paired and independent t-tests were performed for statistical analysis. RESULTS: Landmarks in the up

Circumcaval Ureter-the Paradigm Shift in Diagnosis and Management.

INTRODUCTION: Correction of a severe anteroposterior skeletal discrepancy, as described in this case of extreme skeletal class III malocclusion, can be quite challenging and fraught with difficulties. Conventional, single-stage bi-jaw orthognathic surgery with pre-and post-surgical orthodontics is associated with drawbacks such as the risk of relapse and an unsatisfactory overall long-term outcome, with persisting occlusal discrepancies and skeletal abnormalities, especially when the magnitude of skeletal correction required is large. Excessive mandibular setback can restrict tongue space, cau

Usefulness of endoscopy in craniofacial trauma.

To determine the extent of endoscopy utilized in craniofacial trauma, a questionnaire was mailed to 400 surgeons in the United States. A 40% response was obtained; 21.3% of the surgeons stated that they had used endoscopy to treat facial fractures; 33% of them had been in practice for 6 to 10 years, and 47% of those who had used endoscopy in craniofacial trauma were in full-time private practice. In facial fractures, endoscopes were used primarily to treat Le Fort fractures (55.9%), followed by zygomatic arch fractures (52.9%). Lack of accompanying instrumentation, such as appropriate drills,

A more accurate method of predicting soft tissue changes after mandibular setback surgery.

PURPOSE: To propose a more accurate method to predict the soft tissue changes after orthognathic surgery. PATIENTS AND METHODS: The subjects included 69 patients who had undergone surgical correction of Class III mandibular prognathism by mandibular setback. Two multivariate methods of forming prediction equations were examined using 134 predictor and 36 soft tissue response variables: the ordinary least-squares (OLS) and the partial least-squares (PLS) methods. After fitting the equation, the bias and a mean absolute prediction error were calculated. To evaluate the predictive performance of

Impact of orthognathic surgery on velopharyngeal function by evaluating speech and cephalometric radiographs.

BACKGROUND: Maxillary retrognathia is a well-known consequence of abnormal facial growth in patients with cleft lip and palate. Orthognathic surgery improves facial appearance and occlusion but may induce velopharyngeal insufficiency. The aim of this study was to evaluate the effect of orthognathic surgery on velopharyngeal function by using speech analysis and lateral cephalometric radiographs. PATIENTS AND METHODS: We conducted a retrospective study of 47 patients who underwent maxillary advancement ± mandibular setback between 2006 and 2016. A new condyle repositionable plate for sagittal split ramus osteotomy.

In sagittal split ramus osteotomy (SSRO) surgery, variable methods are used to maintain the proximal segment in the anatomic position during fixation to minimize early built-in relapse. However, there is still a chance of condyle malposition. In this study, a new titanium plate, which has a sliding oval hole in its distal part, was used to enable postoperative mobility of the proximal segment. This plate makes it possible to reposition the condyle during the postoperative intermaxillary fixation period. These plates were used successfully in 23 patients who received SSRO setback surgery. All of

Treatment of hemimandibular hyperplasia: the biological basis of condylectomy.

Treatments to correct skeletal deformities in patients with hemimandibular hyperplasia differ, particularly about the age at which the operation is done and the operation itself. To some extent, the differences can be attributed to the unknown biological basis of disease. The aim of the present study was to evaluate clinically the outcome of a rationale for the operation based on condylectomy on the affected side. Histological, radiological, and nuclear methods were used to get a more detailed insight into the reason for the operation. Six patients with hemimandibular hyperplasia were treated

Changes in the calibre of the upper airway and the surrounding structures after maxillomandibular advancement for obstructive sleep apnoea.

Maxillomandibular advancement (MMA) is effective in the treatment of obstructive sleep apnoea. We aimed to assess changes in the calibre of the upper airway, facial skeleton, and surrounding structural position after MMA and their association with improvement in symptoms. Sixteen consecutive adults with moderate-to-severe apnoea were treated by primary MMA. Polysomnography and computed tomography (CT) of the head and neck were done before and at least 6 months after MMA. The calibre of the upper airway, the facial skeleton, and the surrounding structures were measured with image analysis softw

Retrospective analysis on prognosis of oral cancer patients according to surgical approaches for effective cancer ablation: swing approach versus visor approach.

BACKGROUND: For the surgical treatment of oral cancer, it is sometimes necessary to expand intraoral access within the oral cavity. The "swing approach" that involves lip splitting of the mandible and temporary mandibular osteotomy and the "visor approach" that does not split the lower lip and mandible are mainly used. This study analyzed postoperative outcomes such as complications, recurrence rate, and survival rate by these two approaches. The goal of this study is to evaluate the surgical outcomes of patients using these two approaches, to propose effective perioperative management for ora…
