Neuroethics, confidentiality, and a cultural imperative in early onset Alzheimer disease: a case study with a First Nation population.

The meaningful consideration of cultural practices, values and beliefs is a necessary component in the effective translation of advancements in neuroscience to clinical practice and public discourse. Society's immense investment in biomedical science and technology, in conjunction with an increasingly diverse socio-cultural landscape, necessitates the study of how potential discoveries in neurodegenerative diseases such as Alzheimer disease are perceived and utilized across cultures. Building on the work of neuroscientists, ethicists and philosophers, we argue that the growing field of neuroet

Should we treat depression with drugs or psychological interventions? A reply to Ioannidis.

We reply to the Ioannidis's paper "Effectiveness of antidepressants; an evidence based myth constructed from a thousand controlled trials." We disagree that antidepressants have no greater efficacy than placebo. We present the efficacy from hundreds of trials in terms of the percentage of patients with a substantial clinical response (a 50% improvement or more symptomatic reduction). This meta-analysis finds that 42-70% of depressed patients improve with drug and 21%-39% improve with placebo. The response benefit of antidepressant treatment is 33%-11% greater than placebo. Ioannidis argues tha

The Infectious Diseases Society of America Lyme guidelines: a cautionary tale about the development of clinical practice guidelines.

Flawed clinical practice guidelines may compromise patient care. Commercial conflicts of interest on panels that write treatment guidelines are particularly problematic, because panelists may have conflicting agendas that influence guideline recommendations. Historically, there has been no legal remedy for conflicts of interest on guidelines panels. However, in May 2008, the Attorney General of Connecticut concluded a ground-breaking antitrust investigation into the development of Lyme disease treatment guidelines by one of the largest medical societies in the United States, the Infectious Dis

A theory of international bioethics: the negotiable and the non-negotiable.

The preceding article in this issue of the Kennedy Institute of Ethics Journal presents the argument that "moral fundamentalism," the position that international bioethics rests on "basic" or "fundamental" moral prinicples that are universally accepted in all eras and cultures, collapses under a variety of multicultural and postmodern critiques. The present article looks to the contractarian tradition of Hobbes and Locke -- as reinterpreted by David Gauthier, Robert Nozick, and John Rawls -- for an alternative justification for international bioethics. Drawing on the central themes of this tra

Rethinking Organisational Justice in Healthcare Through Nurses' Voices: Insights From Nancy Fraser's Theory of Justice.

Organisational justice has emerged as a key area of interest given the challenges of protecting the health and well-being of nurses in healthcare organisations. Existing research shows significant conceptual and methodological limitations that must be addressed to achieve a more comprehensive and ethically grounded understanding of nurses' needs in the healthcare system. This article explores how nurses experience their working conditions within public healthcare organisations through a theoretical lens of social justice to unveil the systemic injustices embedded within them. Adopting a critic

Reading Incivility Through Camp: Moral Binaries in Nursing Education.

In the United States context, incivility in nursing education is widely studied as a behavioral problem to be measured and remedied. In contrast, civility has been conceptualized as a professional virtue that is foundational to nursing identity. Together, these civility and incivility orientations sustain a moral binary for nursing education in which civility functions as virtue and incivility as its violation. This paper re-orients the discourse by reading incivility through camp. Drawing on Susan Sontag and Fabio Cleto, camp serves as an analytic lens to show how exaggerated seriousness, aff

Assembling packs: Outreach nurses, disaffiliated persons, and sorcerers.

Nurses working in outreach capacities frequently encounter disaffiliated or 'hard to reach' populations, such as those experiencing homelessness, those who use substances, and those with mental health concerns. Despite best efforts, nurses regularly fail to find meaningful engagement with these populations. Mobilizing the work of Deleuze and Guattari, this paper will critically examine conventional outreach nursing practices as rooted in the royal science of psychiatry, which many 'survivors' of psychiatric interventions reject. The field of Mad Studies offers an understanding of patient resis

Neo-Paleyan biology.

There is a 'Neo-Paleyan' tradition in British evolutionary theorising, which began with Darwin and continues to the present day. This tradition conceives of adaptation in terms of design, and it often puts natural selection in the role of an ersatz designer. There are significant disanalogies between Paleyan conceptions of design and modern conceptions of adaptation and selection, which help to explain why the neo-Paleyan programme is sometimes treated with hostility. These general disanalogies do not suffice to dismiss the most interesting forms of recent neo-Paleyanism, which draw on theoret

Could robots become authentic companions in nursing care?

Creating android and humanoid robots to furnish companionship in the nursing care of older people continues to attract substantial development capital and research. Some people object, though, that machines of this kind furnish human-robot interaction characterized by inauthentic relationships. In particular, robotic and artificial intelligence (AI) technologies have been charged with substituting mindless mimicry of human behaviour for the real presence of conscious caring offered by human nurses. When thus viewed as deceptive, the robots also have prompted corresponding concerns regarding th

Human genetics after the bomb: Archives, clinics, proving grounds and board rooms.

In this paper I track the history of post-1945 human genetics and genomics emphasizing the importance of ideas about risk to the scientific study and medical management of human heredity. Drawing on my own scholarship as it is refracted through important new work by other scholars both junior and senior, I explore how radiation risk and then later disease risk mattered to the development of genetics and genomics, particularly in the United States. In this context I excavate one of the central ironies of post-war human genetics: while studies of DNA as the origin and cause of diseases have been

Can functionality in evolving networks be explained reductively?

Philosophers of biology disagree about an adequate explication of the concept of function. Instead of perpetuating the debate on the level of in principle-arguments, this paper aims first at reconstructing functional talk in the biological research papers of Marom and Braun, which focus on two different kinds of evolving networks, and in discussing the ontological consequences which the authors draw from their results. Marom investigates evolving neural networks controlling Braitenberg vehicles. Braun observes the evolutionary rearrangement or "rewiring" of the genetic network of genetically m

Clever Hans and his effects: Karl Krall and the origins of experimental parapsychology in Germany.

Shortly before the outbreak of World War I, the so-called Elberfeld horses, the counting and speaking animals, were among the most debated subjects of the newborn comparative psychology. Yet, they have left little trace in the historiography of this discipline, mostly as an appendix of the more famous Clever Hans. Their story is generally told as the prelude to the triumph of reductionistic experimental psychology. By paying a more scrupulous attention than has so far being done to the second life of Hans, and to the endeavours of his second master, Karl Krall, this article explores the story

The human autonomous karyotype and the origins of prenatal testing: children, pregnant women and early Down's syndrome cytogenetics, Madrid 1962-1975.

Through their ability to reveal and record abnormal chromosomes, whether inherited or accidentally altered, chromosomal studies, known as karyotyping, became the basis upon which medical genetics was constructed. The techniques involved became the visual evidence that confirmed a medical examination and were configured as a material culture for redefining health and disease, or the normal and the abnormal, in cytological terms. I will show that the study of foetal cells obtained by amniocentesis led to the stabilisation of karyotyping in its own right, while also keeping pregnant women under t

Managing the future: the Special Virus Leukemia Program and the acceleration of biomedical research.

After the end of the Second World War, cancer virus research experienced a remarkable revival, culminating in the creation in 1964 of the United States National Cancer Institute's Special Virus Leukemia Program (SVLP), an ambitious program of directed biomedical research to accelerate the development of a leukemia vaccine. Studies of cancer viruses soon became the second most highly funded area of research at the Institute, and by far the most generously funded area of biological research. Remarkably, this vast infrastructure for cancer vaccine production came into being before a human leukemi

Prenatal diagnosis: the irresistible rise of the 'visible fetus'.

Prenatal diagnosis was developed in the 1970s, a result of a partly contingent coming together of three medical innovations-amniocentesis, the study of human chromosomes and obstetrical ultrasound-with a social innovation, the decriminalization of abortion. Initially this diagnostic approach was proposed only to women at high risk of fetal malformations. Later, however, the supervision of the fetus was extended to all pregnant women. The latter step was strongly favoured by professionals' aspiration to prevent the birth of children with Down syndrome, an inborn condition perceived as a source

Medical science and the Cruelty to Animals Act 1876: A re-examination of anti-vivisectionism in provincial Britain.

The Cruelty to Animals Act 1876 was an important but ambiguous piece of legislation. For researchers it stymied British science, yet ensured that vivisection could continue under certain restrictions. For anti-vivisection protestors it was positive proof of the influence of their campaigns, yet overly deferent to Britain's scientific elite. In previous accounts of the Act and the rise of anti-vivisectionism, scientific medicine central to these debates has been treated as monolithic rather than a heterogeneous mix of approaches; and this has gone hand-in-hand with the marginalizing of provinci

A case study in explanatory power: John Snow's conclusions about the pathology and transmission of cholera.

In the mid-1800s, there was much debate about the origin or 'exciting cause' of cholera. Despite much confusion surrounding the disease, the so-called miasma theory emerged as the prevalent account about cholera's cause. Going against this mainstream view, the British physician John Snow inferred several things about cholera's origin and pathology that no one else inferred. Without observing the vibrio cholerae, however,-data unavailable to Snow and his colleagues-, there was no way of settling the question of what exactly was causing cholera and how, or if, it was passed on. The question then

Robert Koch and the invention of the carrier state: tropical medicine, veterinary infections and epidemiology around 1900.

This paper reassesses Robert Koch's work on tropical infections of humans and cattle as being inspired by an underlying interest in epidemiology. Such an interest was developed from the early 1890s when it became clear that an exclusive focus on pathogens was insufficient as an approach to explain the genesis and dynamics of epidemics. Koch, who had failed to do so before, now highlighted differences between infection and disease and described the role of various sub-clinical states of disease in the propagation and--consequently--in the control of epidemics. Studying pathologies of men and ca

The (dis)appearance of the dying patient in generalist hospital and care home nurses' talk about the patient.

This article explores interview data from a study of 50 Norwegian generalist nurses' focus group accounts of caring for dying patients in the hospital and care home. An eclectic discourse analytic approach was applied to nurses' accounts of the patient and three discursive contexts of reference to the patient were identified: the 'taken as read' patient, the patient paired with particular characteristics and the patient as psychologically present. Talk about the patient falls mainly into the first two contexts, which position the patient in relation to three closely related discursive processe
