BACKGROUND: Scaphoid fractures account for 90% of carpal fractures and occur predominantly in young men. Immediate surgical fixation of this fracture has increased.
OBJECTIVE: To compare the clinical effectiveness and cost-effectiveness of surgical fixation with cast treatment and early fixation in adults with scaphoid waist fractures that fail to unite.
DESIGN: Multicentre, pragmatic, open-label, parallel two-arm randomised controlled trial with an economic evaluation and a nested qualitative study.
SETTING: Orthopaedic departments of 31 hospitals in England and Wales recruited from July 2

PURPOSE: Several radiographic carpal alignment indices are used to evaluate the deformities of scaphoid fractures. The purpose of this study was to determine the reliabilities and validities of radiographic carpal alignment indices commonly used to evaluate deformities of scaphoid fractures.
METHODS: Thirty-six patients with a scaphoid fracture were evaluated. Five carpal alignment indices were assessed on lateral plain radiographs, namely, scapholunate angle, radioscaphoid angle, radiolunate angle, radiocapitate angle, and capitolunate angle. Three examiners measured these radiographic indic

Scaphoid fractures occur commonly in the athlete and should be treated with urgency to avoid undesired late complications. Magnetic resonance imaging may be helpful to make a prompt diagnosis so that an appropriate early treatment plan can be initiated. Cast immobilization in acute, nondisplaced scaphoid fractures seems to have an equivalent union rate to surgical modalities. Despite this, limiting the immobilization and time to union period in the athlete will allow earlier restoration of preinjury level function and eventual return to play. Percutaneous techniques with or without arthroscopy

PURPOSE: To use computed tomography to determine whether factors could be identified to predict union for acute scaphoid fractures treated nonoperatively.
METHODS: We used a radiology database at a tertiary care center to identify scaphoid computed tomography scans performed between 2004 and 2010. We noted fracture location, fracture orientation, translation between fragments, humpback deformity, comminution, cysts, and sclerosis. We determined the associations between imaging variables on union rates and time to achieve union with casting alone in a cohort of 219 patients (mean age, 31 y; 83

BACKGROUND: A clinically suspected scaphoid fracture with normal initial plain x-rays is a difficult but common problem. The role of MRI as a diagnostic modality in this situation was analysed based on the experience of 611 consecutive cases.
METHODS: All patients presenting to the emergency department with a history of a fall on an outstretched hand and with tenderness over the anatomical snuff box and scaphoid tubercle but normal plain x-rays were offered MRI as part of the standard protocol. Between January 2003 and November 2005, 611 of a total of 619 such patients underwent the scan and

PURPOSE: Type I lunates have a single distal facet for articulation with the capitate; type II lunates have an additional (medial) hamate facet on the distal articular surface. We retrospectively reviewed a series of patients with scaphoid nonunions to determine if there was an association between lunate morphology and the degree of carpal instability observed. Association between lunate morphology and the location of the scaphoid fracture (proximal or waist) was also investigated.
METHODS: Radiographs were evaluated for 45 patients with established scaphoid nonunions. Lunate morphology, scap

PURPOSE: To clarify quantitatively the 3-dimensional deformity of the carpus in scaphoid nonunion on the basis of fracture location.
METHODS: Three-dimensional computed tomography was used to examine 20 patients with scaphoid nonunion. Displacements of the distal and proximal fragments of the scaphoid, lunate, triquetrum, and capitate were visualized and quantified using a 3-dimensional image-matching technology. Cases were categorized as distal fracture (16 cases) or proximal fracture (4 cases) based on the location of the fracture line relative to the dorsal apex of the scaphoid ridge where

Scaphoid fracture patterns can be divided into volar and dorsal type using three-dimensional computed tomography. Ninety-nine patients underwent this examination plus radiography to determine how often fracture pattern was identifiable by radiography including proximal fragment ratio (PFR) measurement. Oblique radiographs obtained at 45 degrees of pronation demonstrated a volar displacement in 28 of 37 volar type fractures in three-dimensional computed tomography (76%), while as oblique view with 45 degrees of supination could identify the dorsal type in ten of 18 (56%). PFR showed a significa

PURPOSE: The paper holds the research purpose of confirming the long-term results of trans-scaphoid perilunate fracture dislocations (TSPFD) under the treatment of open reduction and internal fixation.
METHODS: Anteroposterial-lateral radiographs of the patient's wrist were taken before and after surgery. We use a dorsal approach for all cases. Postoperative clinical and radiographic assessments were performed routinely. The scapholunate angle (SLA), estradiol angle (RLA), as well as lunotriquetral distance (LTD) assisted in the radiographic assessment. Clinical assessment was performed using

Fractures of the scaphoid are common injuries, accounting for approximately 80 % of carpal fractures. Differentiation between stable and unstable fractures (Herbert classification) cannot be made with conventional X-rays, so evaluation by computed tomography should additionally be performed. Under most circumstances, minimally invasive surgery with cannulated screws is the treatment of choice. A longer cast immobilization after minimal-invasive surgery is not necessary. Conservative treatment still has a place if the fracture is not dislocated nor unstable, but operative treatment can be offer

INTRODUCTION: The object of this study was to assess the risk of injury to tendons, nerves and vessels in percutaneous antegrade scaphoid fracture fixation.
METHODS: Forty cadaveric forearms were used in this study. A guide wire for cannulated headless compression screws was inserted percutaneously in each scaphoid according to established surgical technique.
RESULTS: No nerve or vessel injuries were observed. Tendons however were injured in 5 out of the 40 specimens. This included the extensor pollicis longus tendon in two specimens, the extensor carpi radialis tendon in two specimens and t

We present a report of a patient with an isolated scaphoid dislocation associated with a hyperextension and axial loading injury of the carpus required a careful and extensive clinical and radiographic evaluation leading to surgical intervention to reduce and stabilize the scaphoid and to reduce and hold internally the axial carpal injury. Knowledge of the anatomy and the potential injury patterns of the carpus will aid the hand surgeon with injury recognition and proper treatment.

BACKGROUND: Arthroscopic lunocapitate (LC) fusion can be an alternative surgical treatment for scapholunate advanced collapse (SLAC) or scaphoid nonunion advanced collapse (SNAC) of the wrist. We retrospectively reviewed patients who had arthroscopic LC fusion to estimate clinical and radiological outcomes.
METHODS: From January 2013 to February 2017, all patients with SLAC (stage II or III) or SNAC (stage II or III) wrists, who underwent arthroscopic LC fusion with scaphoidectomy and were followed up for a minimum of 2 years, were enrolled in this retrospective study. Clinical outcomes inclu

PURPOSE: To evaluate the reliability of radiographs (XR) alone versus the combination of XR and computed tomography (CT) in determining scaphoid union following open reduction internal fixation (ORIF) with a headless compression screw.
METHODS: We used our imaging database to identify 32 XR and corresponding CTs over a 6-year period (from 2012 to 2018) that were performed to evaluate scaphoid healing following ORIF. Three hand surgeons evaluated the studies to assess (1) healing, (2) if partially healed, the percentage of healing, and (3) the certainty of healing. Initially, XR were reviewed

We investigated the intraosseous arteries of six normal cadaveric scaphoids using red lead injection and three-dimensional reconstruction with micro-computed tomography. The arterial entrances were generally located around the dorsal ridge, the insertion of the scaphocapitate ligament and the radial part of the radioscaphocapitate ligament. Two to three trunk arteries entered the scaphoid on dorsal ridge at the level of the waist. The distal part of the scaphoid was mainly supplied by arteries from the waist. The blood supply of about 40% of proximal part of the scaphoid was poor. The blood su

BACKGROUND: Management of osseous nonunion presents a significant challenge to reconstructive surgeons. Conventional treatment options include rigid internal fixation with nonvascularised bone graft, local pedicled vascularized bone flap, and microvascular free bone flaps. The medial femoral condyle free flap (MFC-FF) is versatile, with reliable blood supply, good pedicle length, and ability to be harvested in various compositions. It has been well described in the literature for scaphoid reconstruction following recalcitrant proximal pole nonunion. Its application in other areas of the body h

Lesions of the scapholunate ligament are among the most common causes of carpal instability and may progress to scapholunate advanced collapse with progressive wrist osteoarthritis if left untreated. This review presents current arthroscopic treatment options in a structured manner and places them in the context of anatomy, pathomechanics, classification systems, and diagnostics. Wrist arthroscopy is the diagnostic gold standard and enables stage-appropriate treatment. According to the 4R algorithm acute refixations, capsuloligamentous reinforcement techniques, reconstructive procedures using
