Slippery when wet: The effect of a novel zwitterionic coating on silicone ureteral stents.

Silicone ureteral stents provide anti-encrustation properties and patient comfort but are more difficult to insert and remove due to increased surface friction. Applying zwitterionic coatings to silicone stents may reduce friction and facilitate stent placement and removal. This study compared the frictional forces of zwitterionic-coated silicone stents with commercial stents using benchtop models. The static and kinetic frictional forces of five ureteral stents were measured: bare silicone stent without coating (BS), single outer-coated zwitterionic silicone stent (Z1), double-coated (inner a

Comparative evaluation of flexible ureterorenoscopy and mini-percutaneous nephrolithotomy for the management of renal pelvic stones less than 2 cm with high attenuation value in pediatric patients: a prospective randomized study.

This study aimed to evaluate the effectiveness and safety of retrograde intrarenal surgery (RIRS) compared with mini-percutaneous nephrolithotomy (mini-PCNL) in children with renal pelvic stones < 2 cm and density > 1000 HU. A prospective randomized clinical trial was conducted at Beni-Suef University Hospitals including 70 children aged 3–14 years. Eligible participants had a single renal pelvic stone < 2 cm with density > 1000 HU on low-dose non-contrast CT and were randomized equally to undergo RIRS or mini-PCNL (n = 35 each). The stone-free rates at one month were 91.4% for RIRS and 94.3%

Clinical features of hypocitraturic kidney stone formers: an observational and retrospective real-life study in an Italian kidney stone center.

Low urinary excretion of citrate is a risk factor for calcium nephrolithiasis, but clinical features of hypocitraturic stone formers remain unclear. Therefore, the present study explored clinical characteristics of hypocitraturic stone formers. It retrospectively analysed clinical characteristics of hypocitraturic patients in a series of 668 adult stone formers refering to a stone outpatient clinic in Milan. Hypocitraturic stone formers (n = 227, 34%) had lower excretion of calcium, phosphate, uric acid, sodium, and potassium than normocitraturic patients. Considering quartiles of citrate excr

Changes in renal function in patients with chronic kidney disease after ureteroscopy: a systematic review and meta-analysis from EAU endourology.

The impact of ureteroscopy (URS) and retrograde intrarenal surgery (RIRS) on renal function in patients with chronic kidney disease (CKD) remains unclear, particularly given concerns regarding intrarenal pressure and repeated interventions. We performed a systematic review and meta-analysis to evaluate changes in estimated glomerular filtration rate (eGFR) following URS or RIRS in patients with pre-existing CKD. A systematic review and meta-analysis were conducted according to PRISMA guidelines and registered in PROSPERO (2025). Databases were searched in March 2026. Studies reporting pre- and

Worldwide variations in purchase and per-procedure costs of single-use flexible ureteroscopes and flexible navigable access sheaths: the economic impact of reuse strategies on procedural outcomes from a 40-country YAU-EAU endourology international collaboration.

Endourological practice increasingly relies on single-use devices, the costs of which vary considerably across regions and healthcare systems. Flexible single-use ureteroscopes (FURS) and flexible and navigable suction ureteral access sheaths (FANS) represent a substantial proportion of ureteroscopy-related expenditure. To mitigate these costs, some centres have adopted reuse strategies aimed at reducing per-procedure expenses. The aim of this study was to evaluate international variation in the purchase prices and real-world per-procedure costs of FURS and FANS, and to assess the economic imp

Predicting the stone-free status post percutaneous nephrolithotomy: a meta-analysis of machine learning models.

Percutaneous nephrolithotomy (PCNL) is the primary treatment for large or complex renal stones, yet postoperative stone-free status (SFS) remains difficult to predict, contributing to variation in clinical outcomes. Machine learning (ML) models have recently been applied in urology and show promise for predicting treatment success by integrating clinical and radiologic features. This study aims to evaluate the diagnostic performance of ML models in predicting the stone-free status following PCNL. We followed PRISMA-DTA guidelines throughout the entire process. Paeoniflorin alleviates nephrolithiasis by targeting serpine1 and inhibiting the NF-κB signaling pathway.

Paeoniflorin (PF) is a bioactive monoterpene glycoside with well-established anti-inflammatory properties; however, its therapeutic potential and molecular mechanism in nephrolithiasis remain insufficiently defined. In this study, network pharmacology was employed to predict potential targets of PF. In vitro, HK-2 cells exposed to calcium oxalate (CaOx) crystals were treated with PF to evaluate inflammatory responses, crystal adhesion, and NF-κB activation. A glyoxylic acid-induced mouse model was used to assess renal crystal deposition, renal function, and expression of key regulatory targets

Inverse association between the C-reactive protein-to-albumin-to-lymphocyte (CALLY) index and kidney stones in US adults: a population-based study.

The C-reactive protein-albumin-lymphocyte (CALLY) index is an emerging integrated biomarker that simultaneously captures systemic inflammation, immune function, and nutritional status. This study investigates the potential association between the CALLY index and the occurrence of kidney stones. This cross-sectional study utilized data obtained from the 2007–2010 NHANES. Weighted logistic regression and nonlinear regression analysis were applied to analyze the relationship between the CALLY index and kidney stones. The robustness of the results was further examined through subgroup analyses. Pr

The development and internal validation of a rating scale system in predicting renal stone bacteria culture positivity before PCNL: a cross-sectional study.

Targeted preoperative antibiotic therapy can help reduce infections following percutaneous nephrolithotomy (PCNL), but it cannot eliminate them entirely, as kidney stones may serve as a reservoir for infection. Moreover, stone culture positivity (SC+) cannot be measured or treated preoperatively. If SC+ could be predicted before surgery, urologists would have an opportunity to optimize infection control of the individual patient’s risk by adjusting the duration of stone removal and managing intrarenal pressure. This was a cross-sectional study including consecutive patients with renal stones u

Parameter mapping for the use of flexible and navigable suction sheath: an in vitro study.

Retrograde intrarenal surgery (RIRS) is recommended for renal stones up to 20 mm. The Flexible and Navigable Suction Sheath (FANS) lowers intrarenal pressure (IRP), enhancing stone-free rates and reducing infectious risk. However, irrigation-suction parameters remain undefined. This study assesses settings across ureteroscope and FANS combinations to define parameters that maintain low IRP and optimal procedural performance. An in vitro experiment was performed using a kidney model and pressure sensor. Four ureteroscopes (6.3 Fr, 7.5 Fr, 8.4 Fr, 9.5 Fr) were evaluated with three FANS (10/12 Fr

A parsimonious preoperative prediction model for postoperative sepsis following upper urinary tract calculi surgery.

We developed and internally validated a prediction model based exclusively on routinely available preoperative (pre-) parameters to identify patients at high risk of postoperative (post-) sepsis following upper urinary tract calculi surgery. A retrospective cohort of 260 patients treated between May 2024 and April 2025 was analyzed. Candidate predictors were selected using least absolute shrinkage and selection operator (LASSO) regression with the one-standard-error rule (λ.1se), followed by multivariable logistic regression modeling. Model performance was evaluated in terms of discrimination,

Explainable machine learning algorithms for preoperative risk stratification of urosepsis after retrograde intrarenal surgery.

Urosepsis is a severe infectious complication of retrograde intrarenal surgery (RIRS) that can progress to septic shock and become life-threatening. Early detection and prompt intervention are crucial for enhancing the prognosis of patients at risk for urosepsis. This study seeks to develop and verify a prediction model for post-RIRS urosepsis utilizing machine learning (ML) techniques. Electronic health record (EHR) data from 124 urosepsis patients and 1,786 non-urosepsis patients, all of whom had upper urinary tract stones and underwent RIRS, were analyzed. Participants were randomly allocat

A retrospective narrative review of kidney stone composition in patients from two race groups having different stone occurrence profiles: a hypothesis-generating study of stone formation based on stone composition alone.

Studies of kidney stone composition have been ongoing for over 60 years but have failed to establish a compelling relationship between stone composition and stone pathophysiology. As such, a focussed investigation involving a different approach to those employed previously is warranted. The stone anomaly in South Africa provides an ideal scenario for such an investigation. In this country, stones occur in the white population group (W) but historically they have been relatively rare in their black African counterparts (B). Accounting for this anomaly in terms of urine composition has not been

Preoperative prediction of postoperative SIRS and sepsis after ureteroscopic lithotripsy: discriminative performance of large language models.

Postoperative systemic inflammatory response syndrome (SIRS) and sepsis following ureteroscopic lithotripsy (URS) are uncommon but potentially life-threatening complications. No prior study has evaluated large language models (LLMs) for predicting post-URS SIRS/sepsis using exclusively preoperative parameters. This study evaluated the discriminative performance of LLMs in this setting. This retrospective case-control study included 210 patients who underwent URS between January 2024 and December 2025 (42 SIRS/sepsis cases, 168 controls). Risk scores (0-100) were generated using GPT-4o-mini and

In Vitro evaluation of laser-stone interaction.

Laser lithotripsy is the main surgical treatment for urinary stones, using Holmium: Yttrium-Aluminium-Garnet (Ho: YAG) and Thulium Fiber Laser (TFL) lasers. Despite technological advances, the stone fragmentation determinants remain unclear. A better understanding would optimize laser lithotripsy (LL) outcomes (efficacy and safety). We aimed to investigate, in vitro, laser-stone interactions. TFL and Ho: YAG lasers were tested using 270 μm fibers on hard synthetic stone samples under four experimental conditions (dry, wet, immersed in contact and at 2 mm distance). A single pulse (0.5-1 J) was

Moroccan Pinus pinaster bark as a dual in vivo and in silico nephroprotective agent against nephrocalcinosis: toxicity and ADMET insights.

Pinus pinaster Aiton, commonly known as maritime pine, is extensively utilized in traditional Mediterranean medicine for a variety of ailments, particularly renal dysfunctions. Its bark is notably high in polyphenols, which may possess therapeutic properties. This study aimed to assess the protective effects of a hydroalcoholic bark extract from P. pinaster (PPBE) against nephrocalcinosis induced by ethylene glycol (EG) in rats, supplemented by in silico toxicity and ADMET profiling. Nephrocalcinosis was induced by providing rats with EG in their drinking water for 30 days, during which they w

Comparing the outcomes of non-papillary versus papillary mini-percutaneous nephrolithotomy for treatment of multiple renal stones: a prospective randomized study.

Papillary puncture is commonly recommended in percutaneous nephrolithotomy (PCNL) because it is thought to reduce the risk of vascular injury. In patients with multiple renal stones, however, calyceal anatomy and stone distribution can make a strict papillary approach difficult. Non-papillary access may be a practical alternative during mini-PCNL, but its safety and effectiveness remain debated. This study compared non-papillary and papillary access in mini-PCNL for multiple renal stones. This prospective randomized study enrolled 108 patients with multiple renal calculi.
