Effect of ocular fixation on positional nystagmus in BPPV patients.

OBJECTIVE: The quantitative suppression rate of positional nystagmus (PN) by ocular fixation (OF) is unknown. This study aims to analyse the effect of OF on the slow phase velocity (SPV) of PN during diagnostic positional manoeuvres (DPMs) with videonystagmography in patients with benign paroxysmal positional vertigo (BPPV). DESIGN: DPMs were performed on 58 patients with BPPV, 33 (56.9%) of whom were women. OF was initiated when PN was most evident, and recording was continued. The mean SPV of three consecutive nystagmus before (F0) and after (F1) OF initiation was calculated. The rate of su

Characteristics of assessment and treatment in Benign Paroxysmal Positional Vertigo (BPPV).

QUESTION: Benign Paroxysmal Positional Vertigo (BPPV) is the most common cause of dizziness presenting to specialist vestibular centres and accounts for approximately 20-30% of referrals to these clinics. In spite of the amount of clinical knowledge surrounding its diagnosis and management, the treatment of BPPV remains challenging for even the most experienced clinicians. This study outlines the incidence of BPPV in a specialised vestibular physiotherapy clinics and discusses the various nuances encountered during assessment and treatment of BPPV. Management of 1000 vestibular schwannomas (acoustic neuromas): clinical presentation.

OBJECTIVE: Despite good knowledge of the key symptoms of vestibular schwannomas and their significance for surgical results, the evolution of symptoms and signs and their relation to tumor extension still need thorough investigation. METHODS: From 1978 to 1993, operations were performed by the same surgeon (M.S.) on 1000 vestibular schwannomas at the Neurosurgical Department of Nordstadt Hospital. The vestibular schwannomas were diagnosed in 962 patients, including 522 female patients (54%) and 440 male patients (46%); the mean age was significantly higher in female patients (47.6 yr) than in

Dose-related effects of alcohol on dynamic posturography and oculomotor measures.

Four healthy male volunteers < 30 years of age participated in a blinded study of placebo versus low (0.45 g/kg lean body weight = LBW), medium (0.80 g/kg LBW) and high (1.05 g/kg LBW) dose ethanol ingestion to investigate its effect upon gaze and posture control. Serial electronystagmography and computerized platform posturography were performed at different points along each subject's blood alcohol concentration (BAC) curve as measured by breath analysis. Smooth pursuit and positional testing revealed subtle abnormalities at sub-intoxicating BACs. In addition, instability on posturography wa

Postural control in Menière's disease and acoustic neurinoma when studied on a linearly oscillating platform.

We investigated the effect of visual control on postural stability on a moving platform in 36 patients with Menière's disease (MD) in 25 patients with operated acoustic neurinoma (AN) and in 19 healthy controls. The force platform was placed on rails and oscillated linearly at frequencies from 0.2 to 5 Hz at a constant velocity of 30 mm/s. In all subjects, the support surface oscillation produced postural instability that differed significantly from the base line stability. The body sway velocity increased almost linearly with the increase of stimulation frequency. The MD patients swayed more

Evaluation of flunarizine in patients with Menière's disease. Subjective and vestibular findings.

In an open multicentre pilot study, 32 patients affected with Menière's disease were treated with the cerebrally effective calcium antagonist flunarizine for 12 weeks. By means of the so-called vestibular index, the therapeutic results could be documented in a rapid and clear manner; the vestibular index takes into account both the subjective statements of the patients and objective pathological vestibular findings in terms of figures. The mean value of this vestibular index of 32 patients with Menière's disease was 9.5 before treatment and only 4.0 at the end of the therapy, which is indicati

Progressive sensorineural hearing loss and cochlear otosclerosis: a prospective study.

The association of otosclerosis with reduced bone conduction is well known but no experimental or valid clinical relationship has been established to confirm this relationship. The purpose of this presentation is to demonstrate a clinical relationship between otosclerosis and sensorineural hearing loss. Experimental proof will await the accumulation and study of temporal bones of those individuals who, in life, exhibited the clinical relationship to be developed in this dissertation.

[A graphical method for the assessment of the sensitivity of the vestibular system (author's transl)].

The authors present a graphical method for the assessment of the sensitivity of the vestibular system. With the aid of this method it is possible, with a high degree of probability, to exclude the existence of disturbances (80%) or confirm the presence of disease.

Meniere's disease study: three-year progress report.

The results of a three-year study of 120 patients with Meniere's disease has revealed multiple but specific etiology for 48 percent of the patients: (1) Allergy-14 percent (2) Congenital or acquired syphilis-7 percent (3) Adrenal pituitary insufficiency-6 percent (4) Hypothyroidism-3 percent (5) Stenosis of the internal auditory canal-3 percent (6) Trauma, acoustic or physical-3 percent (7) Combination of the above-12 percent. Emotional or psychiatric factors are not involved in the etiology of this disease. The vestibular and cochlear aqueduct on both temporal bones of patients with Meniere's
