PURPOSE: To evaluate after-cataract formation, ocular growth, and intraocular lens (IOL) behavior in lensectomized infant rabbit eyes implanted with a downsized heparin-surface-modified poly(methyl methacrylate) (HSM PMMA) IOL with long haptics or a 3-piece AcrySof(R) IOL (Alcon Laboratories, Inc.).
SETTING: St. Erik's Eye Hospital, Stockholm, Sweden.

Despite interest in ultraviolet (UV) damage and UV vision in lower vertebrates, there are few recent publications on human UV sensitivity. We obtained dark-adapted spectra from 4 aphakic and 5 normal eyes at 8.8 degrees off-fovea using the staircase method. We created a standardized model of endophthalmitis in the aphakic rabbit eye using a laboratory strain of Staphylococcus epidermidis of known characteristics (ATCC 155). Eyes were injected with the following number of organisms: 170, 3760, 8750, 170,000 and 460,000. Examinations of VEP were performed in 32 cases of pseudophakia (20 implants to the anterior, 12 to the posterior chamber). The impulses were light flashes and a reversible checkboard pattern. Normal records of VEP were obtained in 93 p.c. of cases with the use of a light flash and in 75 p.c. by applying a checkboard pattern. No correlation was found between the visual acuity and the VEP. The authors observed a shortening of the latency time of the N2 bend after implantation of the lens using the light flashes.

Twenty-eight epikeratophakia procedures have been performed at the University of Minnesota since December 1985. This report describes several technique modifications and compares the refractive and visual outcome of these cases with the national results. There have been 17 aphakic cases (13 adult and four pediatric), seven myopic procedures, and four keratoconus cases. The suction trephine was studied in 31 aphakic and pseudophakic penetrating keratoplasty patients (31 eyes). The postoperative corneal astigmatism averaged 3.9 +/- 2.8 diopters overall. The 15 pseudophakic cases in nonscarred corneas showed a marked reduction in postoperative corneal astigmatism (3.59 diopters) when compared to previous reports. Our results have encouraged us to continue use of the suction trephine in all aphakic or combined keratoplasty procedures.

Many advances have been made in recent years in the treatment of patients with cataracts, particularly in the area of aphakic vision correction. Drainage of subretinal fluid was performed in 84% of a consecutive series of 662 scleral buckling procedures. Complications of drainage occurred in 5.6% of cases, and the ultimate anatomical success rate in the series was 94.2%. Ninety-seven percent of failures were due to massive PVR, but this process was not associated with complications of surgery. Although drainage of subretinal fluid is a potentially hazardous maneuver, complications of this procedure are an uncommon cause of anatomical failure in retinal detachment surgery.

It is the author's opinion that extended wear lenses should be fit with caution on patients who cannot wear or handle daily wear lenses or adapt to aphakic spectacles. Standard protocol after initial fitting should be to examine the anterior segment with the slit lamp for vascular changes every three months indefinitely without final discharge. Patients who cannot keep all of their follow-up appointments or cannot afford long-term professional fees and lens replacement costs should not be fit with extended wear lenses.
