A clinical evaluation of flurbiprofen LAT and piroxicam gel: a multicentre study in general practice.

A prospective, randomized, multicentre, open, crossover study of the comparative efficacy, tolerability and acceptability of two topical nonsteroidal anti-inflammatory drug (NSAID) therapies, flurbiprofen local-action transcutaneous (LAT) patch (40 mg b.d.) and piroxicam gel (3 cm, 0.5% q.d.s), was conducted in general practice in the UK in 137 men and women with soft-tissue rheumatism of the shoulder or elbow (e.g. epicondylitis, tendinitis, bursitis and adhesive capsulitis). Patients received one therapy for 4 days before crossing over to the other NSAID for a further 4 days, followed by 6 d…

Magnetic resonance imaging: Clinical correlation in surgically treated isolated subacromial impingement syndrome.

BACKGROUND: This study aimed to evaluate the relationship between magnetic resonance imaging (MRI) findings and clinical symptoms, including … teoarthritis, acromial spur formation, supraspinatus tendinosis grading, and subacromial-subdeltoid bursitis. RESULTS: No significant associations were identified between AHD, CSA, or SAV with either pain scores or functional performance (p > 0.05 for all comparisons). Supraspinatus tendinosis severity showed moderate correlations with both VAS (ρ + 0.517 …

Therapeutic Role of Concentrated Growth Factors in the Management of Chronic Heel Tendinopathies: A Prospective Interventional Study.

Background Chronic heel tendinopathies, including plantar fasciitis, Achilles tendinopathy, retrocalcaneal bursitis, and Haglund deformity, are among the most common causes of persistent heel pain and functional limitation in adults. Increasing evidence suggests that these conditions represent degenerative failed-healing responses rather than purely inflammatory processes, which explains the limited long-term effectiveness of conventional anti-inflammatory therapies. Concentrated growth factors (CGFs), a second-generation autologous platelet concentrate, provide a dense fibrin matrix enriched …

Diagnosis and management of shoulder pain by New Zealand physiotherapists: a national survey.

BACKGROUND: Physiotherapists commonly manage patients with shoulder disorders. The primary aims of this study were to (1) determine current assessment and management of patients with shoulder disorders by physiotherapists in New Zealand and compare it to evidence-based recommendations; and (2) assess their confidence in making a diagnosis. METHODS: We performed a cross-sectional online survey of New Zealand registered physiotherapists who self-report that they treat patients with shoulder disorders. The survey was distributed electronically via professional physiotherapy networks and social m

Short-Term Outcome Predictors in Patients With Primary Adhesive Capsulitis Treated With Ultrasound-Guided Hydrodilatation With Corticosteroids.

OBJECTIVE: Glenohumeral joint hydrodilatation with corticosteroids has been proposed as an effective secondary therapeutic procedure for primary adhesive capsulitis. However, little is known about which subgroup of patients would benefit from this procedure. This study aimed to identify covariates associated with improved prognosis in patients receiving ultrasound-guided hydrodilatation with corticosteroid injection. DESIGN: This was a cohort study. Data on baseline demographic characteristics, disease status, past medical conditions, and initial ultrasonographic findings were collected. Line

Imaging of knee implants and related complications.

Various types of knee arthroplasty implants are available depending on the number of knee compartments to be replaced and ligament stability. Unlike unicompartmental arthroplasty, the aim of total knee replacement is to maintain normal knee alignment. EOS X-ray imaging in the coronal and sagittal planes is the best modality for measuring the reference axes of the knee, but may by complemented by CT scans in the transverse plane. In addition to implant-related complications such as loosening and polyethylene wear, complications of the knee extensor mechanism are sometimes observed. Conventional

Rheumatic conditions in patients with diabetes mellitus.

Diabetes mellitus (DM) is a chronic systemic disease with a wide range of complications, including complications in the musculoskeletal system. DM is a common disease in nearly all countries and continues to increase in numbers and significance. Approximately 90 % of patients have type 2 DM which is caused by resistance to insulin. Type 1 DM results from an absolute deficiency of insulin due to an autoimmune destruction of insulin-producing beta cells within the pancreas. Diabetic patients suffer from diverse rheumatic conditions that affect their life quality. This article reviews the rheumat

Corticosteroid injections in polymyalgia rheumatica: a double-blind, prospective, randomized, placebo controlled study.

OBJECTIVE: To determine the efficacy and safety of shoulder corticosteroid injections in polymyalgia rheumatica (PMR). METHODS: Twenty consecutive patients with active PMR were randomized into a 7 month, double blind, placebo controlled study. Patients received either bilateral shoulder injections of 40 mg of 6-methylprednisolone acetate or placebo (1 ml saline solution). Responders were treated weekly with the same regimen for a total of 4 bilateral injections and then followed for 6 months. Response was defined as a 70% reduction in visual analog scale (VAS) score for pain and for patient a

Comparison of the analgesic efficacy of intra-articular steroid injections and its combination with suprascapular nerve block for adhesive capsulitis of the shoulder joint: a randomized clinical trial.

BACKGROUND: Adhesive capsulitis is a distressing ailment that progressively limits the active and passive mobility of the shoulder joint. Physical therapy (PT) combined with intra-articular steroid (IAS) injection and suprascapular nerve block (SSNB) has shown improved functional outcomes. We aimed to assess whether providing both IAS injection and SSNB improved outcomes compared with IAS injection alone. METHODS: Ninety-six patients with adhesive capsulitis were randomized into three groups consisting of group IAS, group IAS+SSNB and group PT (control). All patients were assessed on day 0, w

Efficacy of high-intensity laser therapy in patients with adhesive capsulitis: a sham-controlled randomized controlled trial.

Adhesive capsulitis is an idiopathic condition characterized by painful shoulder stiffness, impairs quality of life, and causes disability. The purpose of the study is to evaluate the effects of high-intensity laser therapy (HILT) on pain, disability, and quality of life in patients with adhesive capsulitis. The study was designed as a prospective, double-blinded, and sham-controlled randomized trial. Thirty-six patients diagnosed with adhesive capsulitis were randomized into HILT plus therapeutic exercises, sham-laser plus therapeutic exercises, and control-therapeutic exercises only groups.

A central nervous system-focused treatment approach for people with frozen shoulder: protocol for a randomized clinical trial.

BACKGROUND: Frozen shoulder (FS) is a musculoskeletal condition of poorly understood etiology that results in shoulder pain and large mobility deficits. Despite some physical therapy interventions, such as joint mobilization and exercise, having shown therapeutic benefit, a definitive treatment does not currently exist. The aim of this study will be to compare the effectiveness of a central nervous system (CNS)-directed treatment program versus a standard medical and physical therapy care program on outcomes in participants with FS. METHODS/DESIGN: The study is a two-group, randomized clinica

Analgesic gas for rehabilitation of frozen shoulder: Protocol for a randomized controlled trial.

BACKGROUND: There is little evidence regarding the best way to treat adhesive capsulitis. Physical therapy can reduce pain and improve function and range of motion. However, we lack clear indications on the regimen, techniques or intensity of physical therapy to achieve better results. Intensive physical therapy seems to be confined to the later stages of adhesive capsulitis (chronic stage) given that rehabilitation-induced pain could worsen the outcomes. Here we describe a protocol for a study comparing the efficacy of a standardized program of intensive mobilization under analgesic gas to a
