Perioperative complications of superficial temporal artery to middle cerebral artery bypass for the treatment of complex middle cerebral artery aneurysms.

OBJECT: Only a few studies have reported the risk of ischemic complications occurring when superficial temporal artery (STA) to middle cerebral artery (MCA) anastomosis is performed during surgery for complex MCA aneurysms. SUBJECTS AND METHODS: This is a retrospective study of 10 patients (age 52-73) with MCA aneurysms treated with revascularization surgery. The aneurysms were 10-50mm in size (mean: 21mm). We studied the causes and frequency of ischemic complications by analyzing postoperative magnetic resonance imaging. RESULTS: Postoperative diffusion-imaging confirmed ischemic complicati

Dually supplied T-junctions in arteriolo-arteriolar anastomosis in mice: key to local hemodynamic homeostasis in normal and ischemic states?

BACKGROUND AND PURPOSE: The functional role of arteriolo-arteriolar anastomosis (AAA) between the middle cerebral artery (MCA) and anterior cerebral artery in local hemodynamics is unknown, and was investigated here. METHODS: Blood flow in AAAs was examined using fluorescein isothiocyanate-labeled red blood cells (RBCs) as a flow indicator in 16 anesthetized C57BL/6J mice before and after MCA occlusion up to 7 experimental days. RESULTS: We observed paradoxical flow in AAAs; labeled RBCs entered from both the MCA and anterior cerebral artery sides and the opposing flows met at a branching T-

Endangered cerebral blood supply after closure of left subclavian artery: postmortem and clinical imaging studies.

BACKGROUND: The aim of this study was to assess hemodynamic consequences of increasingly common coverage of the left subclavian artery (LSA) during endovascular stent-graft repair for thoracic aortic disease without surgical revascularization. We considered that critical arteries to examine are the right vertebral artery and posterior communicating arteries (PComA) because their simultaneous insufficiency would drastically endanger posterior cerebral circulation. The existence and the diameters of these arteries were studied in a postmortem anatomic study with review of clinical craniocervical

Perfusion-bypass recipient mismatch as a potential risk factor for perioperative ischemic stroke following single-barrel STA-MCA bypass for symptomatic atherosclerotic carotid occlusive disease.

OBJECTIVE: EC-IC bypass for symptomatic atherosclerotic internal carotid and middle cerebral artery chronic occlusion (AIMCAO) failed to show benefit over medical management in previous randomized controlled trials. We explored how a perfusion-directed bypass targeting strategy impacts perioperative stroke risk for symptomatic AIMCAO. METHODS: We conducted a single-institution, retrospective analysis of consecutive patients undergoing single-barrel STA-MCA bypass for symptomatic AIMCAO from 2019 to 2025. Matched and mismatched cohorts were created based on the congruity of dominant hypoperfus

Optimizing the Workflow of Superficial Temporal Artery Mapping in Extracranial-Intracranial Bypass Surgery Using Mixed Reality: A Proof-of-Concept Study.

BACKGROUND: Doppler ultrasound is the standard for mapping the superficial temporal artery (STA) during extracranial-intracranial (EC-IC) bypass surgery. Mixed reality (MR) offers a novel alternative by providing patient-specific anatomic overlays to better visualize the STA. This study aims to validate MR-guided STA mapping as a reliable preoperative planning tool in EC-IC bypass to the middle cerebral artery. METHODS: In this proof-of-concept study, 7 patients undergoing STA-middle cerebral artery bypass surgery were enrolled. Preoperative computed tomography angiograms were superimposed on

Long-term follow-up of pediatric moyamoya disease treated by combined direct-indirect revascularization surgery: single institute experience with surgical and perioperative management.

Moyamoya disease (MMD) is a rare occlusive cerebrovascular disease that mainly presents in children as cerebral ischemia. Prompt treatment with either a direct or indirect revascularization procedure is necessary for children with MMD in order to prevent repeated ischemic events. We herein present our experience with combined direct and indirect bypass surgery for the treatment of pediatric MMD as well as our uniquely designed perioperative protocol. Twenty-three patients with MMD, aged between 2 and 16 years old (mean 9.36), underwent 38 combined bypass procedures between 2008 and 2015. All p

Analysis of Multiple Intracranial Aneurysms with Different Outcomes in the Same Patient After Endovascular Treatment.

BACKGROUND: Aneurysm recanalization after coiling, with or without stent assistance, is a major issue in the endovascular management of intracranial aneurysms. Multiple intracranial aneurysms with different outcomes after endovascular treatment may represent a useful disease model in which patient-specific risk factors can be balanced to investigate possible features linked to aneurysm recanalization. In the present study, we evaluated the impact of aneurysm-specific, treatment-related, and hemodynamics-related factors on multiple aneurysms and to explore the reason why one aneurysm recanalize

Improvement in cerebral hemodynamic parameters and outcomes after superficial temporal artery-middle cerebral artery bypass in patients with severe stenoocclusive disease of the intracranial internal carotid or middle cerebral arteries.

OBJECT: Both the older and the recent extracranial-intracranial (EC-IC) bypass trials for symptomatic carotid occlusion failed to demonstrate a reduction in stroke recurrence. However, the role of superficial temporal artery (STA)-middle cerebral artery (MCA) bypass in patients with symptomatic intracranial stenoocclusive disease has been rarely evaluated. The authors evaluated serial changes in various cerebral hemodynamic parameters in patients with severe stenoocclusive disease of the intracranial internal carotid artery (ICA) or middle cerebral artery (MCA) and impaired cerebral vasodilato

De novo ivy sign indicates postoperative hyperperfusion in moyamoya disease.

BACKGROUND AND PURPOSE: The ivy sign on fluid-attenuated inversion recovery MRI is a specific finding in moyamoya disease (MMD). This sign indicates decreased cerebral perfusion, dilated pial vasculature, and slow leptomeningeal collateral flow. This study aimed to clarify the characteristics of perioperative changes in the ivy sign in relation to cerebral hyperperfusion, which frequently occurs in MMD of unknown pathogenesis. METHODS: This prospective study included patients with MMD who underwent superior temporal artery-middle cerebral artery single bypass. Fluid-attenuated inversion recov

Treatment of acute cervical internal carotid artery dissection using the Solitaire FR revascularization device.

During treatment of a right internal carotid artery terminus aneurysm, an acute iatrogenic flow limiting dissection was caused in the cervical internal carotid. The true lumen was catheterized using a Mirage 0.008 microwire over an Excelsior SL-10 microcatheter, which was exchanged for a Marksman microcatheter. A 6 mm × 30 mm Solitaire FR revascularization device was then deployed across the dissection as a salvage technique.

High flow extra-cranial to intra-cranial bypass for complex internal carotid aneurysms.

Cerebral revascularisation with extracranial - intracranial (EC-IC) bypass is generally indicated in patients with complex anterior circulation aneurysms who have failed parent artery occlusion. We report on the process and outcome of our early experience of performing high flow bypass in patients with complex anterior circulation aneurysms. We have reviewed patients who have undergone an EC-IC bypass for treatment of complex anterior circulation aneurysms, and report our outcome on graft patency, surgical complications, discharge destination, and obliteration rates. Nine patients that underwe

Extra-intracranial blood shunt mimicking aneurysm rupture: intracranial-pressure-controlled rabbit subarachnoid hemorrhage model.

INTRODUCTION: The achieved degree of delayed cerebral vasospasm (DCVS) in the rabbits most frequently applied cistern magna blood injection model is often mild. The aim of this study was to characterize and evaluate the feasibility of an experimental SAH technique that mimics pathophysiological mechanisms and triggers higher degrees of DCVS. MATERIALS AND METHODS: SAH was induced by extracranial-intracranial (EC/IC) shunting of blood from the subclavian artery into the great cerebral cistern. Intracranial pressure (ICP), arterial blood pressure, heart rate, arterial blood gas analysis, and ne

Intracerebral hemorrhage caused by rupture of a giant aneurysm complicating superficial temporal artery-middle cerebral artery anastomosis for moyamoya disease.

INTRODUCTION: Aneurysm formation at the anastomosis site after extracranial-intracranial (EC-IC) bypass surgery for major arterial occlusion or stenosis due to atherosclerosis has only been reported a few times previously. However, no case describing the formation of a giant aneurysm after EC-IC bypass surgery has been reported to date. Additionally, this complication associated with moyamoya disease is extremely rare, and only one case has been reported so far. CLINICAL REPORT: We report a case of a 51-year-old woman having a rare complication of intracerebral hemorrhage due to rupture of a

Venous outflow as a criterion of impairment of cerebral vascular reserve.

AIM: The purpose of this work is to evaluate if the asymmetry of venous outflow between the two hemispheres is a reliable criterion of impairment of the cerebral vascular reserve among symptomatic patients harbouring a spontaneous atherosclerotic occlusion of internal carotid artery. METHODS: From January 1995 to December 2007, 7 symptomatic patients, affected from occlusion of internal carotid artery, were submitted to a low-flow by-pass between the superficial temporal artery and the middle cerebral artery owing to the presence of an impairment of cerebral vascular reserve diagnosed by TC-D

Transient hyperperfusion after superficial temporal artery/middle cerebral artery bypass surgery as a possible cause of postoperative transient neurological deterioration.

BACKGROUND: This study was designed to investigate the relationship between transient neurological deterioration and changes in cerebral perfusion after superficial temporal artery/middle cerebral artery (STA-MCA) bypass. METHODS: Clinical characteristics of the patients with postoperative transient neurological deterioration were analyzed in 120 STA-MCA bypass procedures for ischemic cerebral diseases, such as atherosclerotic disease and moyamoya disease. Serial brain single-photon emission computed tomography (SPECT) was performed on 25 patients before surgery and on the third and tenth pos

Reconstruction of scalp defect after Moyamoya disease surgery using an occipital pedicle V-Y advancement flap.

Scalp necrosis is an infrequent complication of Moyamoya disease surgery, which is more prevalent in the parietotemporal area. Because scalp vascularity is severely compromised after Moyamoya disease surgery, reconstruction of defects with local scalp tissue is challenging. To cover defects, a flap is needed that is highly vascularized and has great mobility and territory to avoid existing scars. After tracing ipsilateral occipital artery, an advancement flap that was based on occipital artery and vein was designed to fit the defect. The flap was elevated in the subperiosteal layer and advance

Long-term follow-up study after extracranial-intracranial bypass surgery for anterior circulation ischemia in childhood moyamoya disease.

Between May, 1974, and March, 1991, 104 patients with moyamoya disease, all under 16 years old at the time of first surgery, underwent superficial temporal-to-middle cerebral artery anastomosis and/or encephalomyosynangiosis. The mean follow-up period was 9.6 years (range 4.8 to 16.0 years). Hemiplegia was the most frequent symptom before the first operation. Transient ischemic attacks (TIA's) were noted in 57 patients and minor stroke with hemiplegia in 44. The most frequent type of cortical dysfunction was aphasia (21 cases). Postoperatively, the incidence of TIA's and/or completed stroke wi

EC-IC bypass for adult patients with moyamoya disease.

We have performed 10 bypass operations on 7 adult patients with moyamoya disease over the past 6 years. Four had haemorrhagic episodes and three had ischemic episodes. Three patients underwent superficial temporal artery-middle cerebral artery (STA-MCA) bypass on both sides and the other four had the procedure on one side. Six of 7 patients showed angiographical improvement after the bypass operation while one patient did not. This patient underwent the second severe stroke and became vegetative during the course of follow-up. Another patient died of myocardial infarction during the follow-up

Tomographic cerebral blood flow measurements in patients with ischemic cerebrovascular disease and evaluation of the vasodilatory capacity by the acetazolamide test.

Cerebral blood flow (CBF) was measured in a series of patients with ischemic cerebrovascular disease using xenon-133 inhalation and single photon emission computer tomography. The spontaneous course of CBF changes in a consecutive series of stroke patients was evaluated. A quite heterogeneous pattern of flow changes was observed: In patients with large cortical/subcortical infarcts, extensive hypoperfused areas were observed, often significantly larger than the corresponding hypodense lesion on the CT scan. Smaller CT lesions caused relatively smaller flow changes. Patients with lacunar infarc
