A dry, droopy and dilated eye: a neuro-ophthalmic case with correlation of signs and anatomical tumour spread.

Adenoid cystic carcinoma (ACC) has a propensity for perineural tumour spread. This report concerns a 51-year-old woman who presented with a unilateral red eye with reduced vision, ptosis and a non-reactive pupil. This patient had a maxillary sinus ACC with perineural spread, leading to the neuro-ophthalmic signs. She is currently being treated with radiotherapy.

Extended transsphenoidal approach for pituitary adenomas invading the anterior cranial base, cavernous sinus, and clivus: a single-center experience with 126 consecutive cases.

OBJECT: The standard transsphenoidal approach has been successfully used to resect most pituitary adenomas. However, as a result of the limited exposure provided by this procedure, complete surgical removal of pituitary adenomas with parasellar or retrosellar extension remains problematic. By additional bone removal of the cranial base, the extended transsphenoidal approach provides better exposure to the parasellar and clival region compared with the standard approach. The authors describe their surgical experience with the extended transsphenoidal approach to remove pituitary adenomas invadi

Microvascular decompression of the accessory nerve for treatment of spasmodic torticollis: early results in 12 cases.

PURPOSE: To describe the early effectiveness of microvascular decompression (MVD) for the treatment of spasmodic torticollis (ST). METHODS: Twelve patients with spasmodic torticollis were treated by microvascular decompression of the accessory nerves using a microscopic neurosurgical technique via the retrosigmoid approach. The most common compressing blood vessels were the ipsilateral posterior inferior cerebral artery (PICA) and/or the vertebral artery. The intraoperative monitor was introduced to detect the accessory nerve and to avoid unnecessary damage to the nerve. RESULTS: Ten patient

[The use of a fiber-optic bronchoscope for the dynamic follow-up of the bulbar disorders in patients with tumors of the posterior cranial fossa].

Two case reports of patients operated for posterior fossa tumors complicated by postoperative hematoma are presented. In both cases patients underwent tracheostomy and were on ventilator for more than three weeks. Postoperative course was complicated by pneumonia and bronchitis. Fiberoptic bronchoscopy was performed to rule out lung complications. Fiberoptic laryngotracheoscopy was done for dynamic control of vocal cords paralysis and helped to make a decision regarding the time of the safe extubation.

[Diagnosis and treatment of the patient with trigeminal neuralgia].

Neuralgia of the trigeminus (NT) is the most common of cranial nerve neuralgias. Its diagnosis is entirely clinical and its most common form of presentation is well understood. Questions of differential diagnosis can emerge with certain entities such as atypical trigeminal neuralgia, short-duration unilateral neuralgiform cephalea of the trigeminus (SUNCT) arising from injection to the conjunctival, lacrimal or other glands, cluster headache, chronic paroxymal hemicrania, pain arising in the teeth and myofacial pain syndrome. The three main causative factors of NT are compression of the nerve

The expanded endonasal approach for an endoscopic transnasal clipping and aneurysmorrhaphy of a large vertebral artery aneurysm: technical case report.

OBJECTIVE: Aneurysms of the vertebral artery are rare, comprising less than 5% of all aneurysms. They can present with subarachnoid hemorrhage, medullary compression, and cranial neuropathies. In consideration of their surrounding regional anatomy, they present a formidable surgical challenge to the neurosurgeon using traditional techniques. Recent advances in endoscopic transnasal surgery have provided an additional approach for the treatment of these difficult lesions. CLINICAL PRESENTATION: We present a case of a large vertebral artery aneurysm causing mass effect on the medulla. Initial t

Radiosurgery vs. microsurgery for newly diagnosed, small petroclival meningiomas with trigeminal neuralgia.

Trigeminal neuralgia (TN) is an excruciating pain that can occur with petroclival meningiomas (PCMs). Gamma knife radiosurgery (GKRS) is an appealing option for small PCMs, but the role of microsurgery (MS) compared to GKRS is not well defined for small PCMs with regard to TN relief. From January 2009 to September 2019, 70 consecutive patients were treated by GKRS or MS for newly diagnosed, small (< 3.5 cm) PCMs with TN. GKRS or MS were performed for 35 patients each. The surgical outcome and TN control according to Barrow Neurological Institute (BNI) score were retrospectively analyzed and co

Laser Interstitial Thermal Therapy for Posterior Fossa Lesions: An Initial Experience.

BACKGROUND: The application of laser interstitial thermal therapy (LITT) for intracranial lesions in the posterior fossa tumors remains challenging due to the smaller size of this compartment as well as the thickness and angle of the occipital bone. In this study, we reviewed our experience with this treatment modality for posterior fossa lesions. METHODS: We retrospectively reviewed our series of 8 patients with posterior fossa tumors treated with LITT from an Institutional Review Board-approved brain tumor database (2012-2017) of more than 200 cases at our institution. RESULTS: The 8 patie

Clivus height value: a new diagnostic method for basilar invagination at CT.

AIM: To investigate whether clivus height (CH; the distance of the basion above Chamberlain's line) could be used to diagnose basilar invagination (BI) at computed tomography (CT). MATERIALS AND METHODS: This retrospective study received institutional review board approval, and was performed from 1 January 2013, to 1 July 2015 with a waiver of informed consent. CH was measured on mid-sagittal reconstructed CT images from 65 BI patients and 85 control subjects to compare the two groups by using the independent-samples t-test. Inter- and intra-observer reliability were evaluated with the intrac

Supracerebellar infratentorial approach to cavernous malformations of the brainstem: surgical variants and clinical experience with 45 patients.

OBJECTIVE: The supracerebellar infratentorial (SCIT) approach can be performed at the midline (median variant), lateral to the midline (paramedian variant), or at the level of the angle formed by the transverse and sigmoid sinuses (extreme lateral variant). We analyzed our experience with SCIT approaches for the surgical treatment of cavernous malformations of the brainstem (CMBs). METHODS: Demographic, clinical, radiologic, and surgical data from 45 patients (20 males and 25 females; mean age, 36.2 years) with CMBs surgically removed through SCIT approaches were reviewed retrospectively. Arteriovenous fistula arising from the persistent primitive olfactory artery with dual supply from the bilateral anterior ethmoidal arteries.

A 59-year-old male presented with generalized seizure. The patient had not been aware of any traumatic head injuries or preceding infection, and had no contributory medical history. On admission, he was alert and well oriented, without neurological impairment or headache. He was afebrile and blood examination showed no abnormal findings. Computed tomography revealed an irregular intracerebral hematoma, 3 x 1.5 cm in diameter, in the left rectal gyrus. Cerebral angiography showed an arteriovenous fistula (AVF) in the anterior cranial fossa supplied only by the persistent primitive olfactory art

Skull indices in a population collected from computed tomographic scans of patients with head trauma.

BACKGROUND: It is important to find out the skull's anatomic indices with computed tomography (CT), especially the skull's dimensions according to the patients age and sex, because it can give important results in neurosurgery. We will also describe the skull's parameters among the Kerman population. METHOD: Of the 2000 CT scans taken after head trauma injury in Kerman province from June 2002 to September 2006, we analyzed 1034 good quality films. The most frequent skull dimensions were measured on each film, and the data were computed and analyzed to assess the cranial indices in various age

Greater superficial petrosal nerve dissection: back to front or front to back?

OBJECTIVE: To introduce a novel surgical technique for the dissection of the greater superficial petrosal nerve (GSPN) in the middle fossa approach. METHODS: Interdural temporal elevation was performed with a front-to-back technique to preserve the GSPN in 12 sides of 6 injected cadaveric heads dissected through a middle fossa approach. RESULTS: The GSPN emerged from the facial hiatus in a shallow bony groove proximally, ran into a deeper sphenopetrosal groove, and eventually reached the mandibular nerve. With front-to-back dissection, this nerve was easily identified at the posterior border

Polyethylene glycol hydrogel dural sealant may reduce incisional cerebrospinal fluid leak after posterior fossa surgery.

OBJECTIVE: Incisional cerebrospinal fluid (CSF) leak remains a significant cause of morbidity, particularly after posterior fossa surgery, with ranges between 4 and 17% in most series. We aimed to determine whether the use of a new polyethylene glycol (PEG) dural sealant product (DuraSeal; Confluent Surgical, Waltham, MA) is effective at preventing incisional CSF leak after posterior fossa surgery. METHODS: One hundred cases of posterior fossa surgery with the PEG dural sealant applied at the time of dural closure were prospectively observed from May 2005 to April 2006. All patients underwent
