Cost-effectiveness and budget impact analysis of switching from apixaban to rivaroxaban treatment among patients with nonvalvular atrial fibrillation in a German healthcare setting.

Aim: Direct oral anticoagulant (DOAC) switching often occurs in patients with nonvalvular atrial fibrillation (NVAF) for medical and nonmedical reasons. Limited data describe the economic consequences of DOAC switching in patients with NVAF. This study evaluates the cost-effectiveness and budget impact of initiating apixaban and switching to rivaroxaban versus initiating and continuing apixaban for patients with NVAF, from a German payer perspective. Materials & methods: Built on an existing model, a cohort-level lifetime Markov model was developed, including dynamic pricing assumptions to acc

Long-lasting Rivaroxaban Use Is Associated With Lower Aortic Valve Leaflet Calcification in Severe Aortic Stenosis.

BACKGROUND: In vitro studies have demonstrated that direct oral anticoagulants (DOACs) downregulate expression of proteins involved in calcification and inflammation. In this hypothesis-testing study we evaluated whether DOAC therapy was associated with decreased valvular calcification in patients with aortic stenosis (AS) who were anticoagulated due to concomitant atrial fibrillation (AF). METHODS: In this case-control study, 72 Caucasian patients with isolated severe AS were compared with 53 individuals with AF concomitant to severe AS who were treated with DOACs for 28.7 ± 13.8 months. Six

Comparison of Apixaban With Warfarin in Patients on Dialysis Within an Integrated Health Care System.

INTRODUCTION: The risk and benefit for patients on dialysis receiving anticoagulation (AC) therapy have not been clearly established. This study aimed to compare the rates of clinically significant thrombotic and bleeding events in patients on dialysis. METHODS: This is a retrospective cohort study conducted using Kaiser Permanente Northern California's electronic medical record database from 2013 to 2021. Patients over 18 years old on dialysis who were prescribed warfarin or apixaban were included. Patients with mechanical valves were excluded. The outcomes were analyzed using Mann-Whitney t

Unfractionated Heparin Dosing in Pediatric Venoarterial Extracorporeal Membrane Oxygenation: Single-Center Retrospective Study, 2015-2021.

OBJECTIVE: Unfractionated heparin (UFH) is the most widely used anticoagulant during extracorporeal membrane oxygenation (ECMO). Optimal dosing of UFH for children undergoing ECMO is unknown, leading to suboptimal exposure. We aimed to develop a population pharmacokinetic (PK) model for UFH in children undergoing venoarterial (VA) ECMO, using anti-Xa activity, to improve the initial dosing regimen. DESIGN: A retrospective observational study (March 2015 to May 2021) using nonlinear, mixed-effect modeling software (Monolix) and Monte Carlo simulations to optimize the dosing. SETTING: Single-c

Factors Influencing the Selection of Non-Vitamin K Antagonist Oral Anticoagulants for Stroke Prevention in Patients With Non-Valvular Atrial Fibrillation.

BACKGROUND: Major atrial fibrillation (AF) guidelines recommend non-vitamin K antagonist oral anticoagulants (NOACs) over warfarin, except in rare clinical circumstances based on 4 randomized controlled trials comparing each NOAC with warfarin. We aimed to investigate the current NOAC prescription behaviors in alignment with the recent clinical evidence available. METHOD: We conducted a cross-sectional analysis of NOAC-using patients with non-valvular atrial fibrillation (NVAF) who were aged ≥65 years on the index date (July 1, 2018) based on nationwide claims data. The types of NOACs being t

Association of Oral Anticoagulants and Verapamil or Diltiazem With Adverse Bleeding Events in Patients With Nonvalvular Atrial Fibrillation and Normal Kidney Function.

IMPORTANCE: Direct oral anticoagulants (DOACs) are purported to have fewer drug-drug interactions than warfarin. However, potential interactions with coprescribed medications are still a safety concern. Verapamil hydrochloride and diltiazem hydrochloride are combined P-glycoprotein (P-gp) and CYP3A4 inhibitors and may be associated with increases in the risk of bleeding with DOACs. OBJECTIVE: To evaluate the risk of bleeding with DOACs and verapamil or diltiazem using an active comparator design. DESIGN, SETTING, AND PARTICIPANTS: A comparative effectiveness active comparator cohort study wa

The incidence of left atrial appendage thrombi on transesophageal echocardiography after pretreatment with apixaban for cardioversion in the real-world practice.

The risk of thromboembolisms during the post-cardioversion period is high. For patients with persistent atrial fibrillation (AF), anticoagulation with warfarin (INR 2.0~3.0) is recommended for at least three weeks prior and four weeks after cardioversion. We aimed to evaluate the efficacy of apixaban in preventing thromboembolic events during post-cardioversion. We enrolled 127 consecutive persistent AF patients (83 persistent, 44 longstanding persistent AF), scheduled to undergo cardioversion and were pretreated with apixaban. All patients underwent transesophageal echocardiography (TEE) to r

Interrupted versus uninterrupted novel oral anticoagulant peri-implantation of cardiac device: A single-center randomized prospective pilot trial.

BACKGROUND: Many patients requiring cardiac implantable electronic device (CIED) implantation are on long-term oral anticoagulant therapy. While continuation of warfarin has been shown to be safe and reduce bleeding complications compared to interruption of warfarin therapy and heparin bridging, it is not known which novel oral anticoagulants (NOAC) regimen (interrupted vs uninterrupted) is better in this setting. METHODS: One-hundred and one patients were randomized to receive CIED implantation with either interrupted or uninterrupted/continuous NOAC therapy before surgery. No heparin was us

Intracranial hemorrhage in patients with atrial fibrillation receiving anticoagulation therapy.

We investigated the frequency and characteristics of intracranial hemorrhage (ICH), the factors associated with the risk of ICH, and outcomes post-ICH overall and by randomized treatment. We identified patients with ICH from the overall trial population enrolled in the Apixaban for Reduction in Stroke and Other Thromboembolic Events in Atrial Fibrillation trial who received ≥1 dose of the study drug (n = 18 140). ICH was adjudicated by a central committee. Cox regression models were used to identify factors associated with ICH. ICH occurred in 174 patients; most ICH events were spontaneous (71

Successful resolution of a left ventricular thrombus with apixaban treatment following acute myocardial infarction.

A 62-year-old man was admitted to our emergency department owing to prolonged chest pain that had lasted for 3 h. An electrocardiogram showed ST elevation in leads I, aVL, and V1-6, and the patient's laboratory revealed elevated myocardial necrosis marker levels. Emergency coronary angiography showed total occlusion of the proximal left anterior descending coronary artery. Subsequent percutaneous coronary intervention was performed by balloon angioplasty followed by stent implantation, and the patient showed improvement. However, echocardiographic examination 2 weeks after the percutaneous cor

Cost effectiveness of apixaban versus aspirin for stroke prevention in patients with non-valvular atrial fibrillation in Belgium.

BACKGROUND AND OBJECTIVE: Evidence indicates that vitamin K antagonists (VKAs) and oral anticoagulant therapy are under-utilised for stroke prevention in patients with non-valvular atrial fibrillation (AF), and patients who decline or cannot tolerate such treatment are often prescribed aspirin instead. Apixaban has been shown in the AVERROES trial to be superior to aspirin in preventing stroke and systemic embolism without significantly increasing the risk of major bleeding among patients with AF who are unsuitable for VKA therapy. This study estimates the economic implications and potential c

Antithrombotic strategies and DOAC dosing following left atrial appendage occlusion: a network meta-analysis.

AIMS: The optimal short-term antithrombotic strategy following left atrial appendage occlusion (LAAO) remains uncertain, with the need to balance thromboembolic prevention and bleeding risk presenting a critical challenge. Recent evidence suggests that direct oral anticoagulants (DOACs) may provide a favourable safety-efficacy profile, with low-dose regimens showing potential benefits during the device endothelialization period. This network meta-analysis (NMA) aimed to compare the efficacy and safety of various antithrombotic strategies, including DOAC dosing, following LAAO. METHODS AND RES

Effectiveness and safety of rivaroxaban following peripheral arterial endovascular revascularization in real-world practice.

OBJECTIVE: The VOYAGER PAD clinical trial demonstrated that low-dose rivaroxaban (LDR; 2.5 mg twice daily) reduces major adverse limb events (MALE) at the expense of increased bleeding rates in patients undergoing lower extremity revascularization for peripheral artery disease (PAD). However, it is unclear whether LDR therapy is associated with similar outcomes in real-world practice. This retrospective cohort study was designed to evaluate the effectiveness and safety of LDR therapy following peripheral vascular intervention (PVI) for patients with PAD. METHODS: The Merative MarketScan datab

Comparative Efficacy and Safety of Low-Dose Direct Oral Anticoagulants Versus Dual Antiplatelet Therapy Following Left Atrial Appendage Occlusion in Patients With Nonvalvular Atrial Fibrillation: A Systematic Review and Meta-Analysis.

BACKGROUND: Left atrial appendage occlusion (LAAO) is an alternative to chronic oral anticoagulation (OAT) for stroke prevention in nonvalvular atrial fibrillation (NVAF) patients with contraindications to OAT. Postprocedure antithrombotic therapy (ATT) is essential to reduce the risk of device-related thrombosis (DRT), but the optimal regimen remains uncertain. AIMS: This study aims to compare the safety and efficacy of low-dose direct oral anticoagulants (DOACs) versus dual antiplatelet therapy (DAPT) following LAAO.
