Missile injuries of the anterior skull base.

Missile injuries of the anterior skull base usually occur during war or war-like situations. These injuries may be isolated or associated with multiple traumatic injuries. We report 23 such cases managed during military conflicts and peacekeeping operations. All were adult males. Four of these patients sustained bullet injuries; the rest were injured from shrapnel. Eighteen patients had injury to the visual apparatus with permanent blindness. Proptosis was seen in 16, cerebrospinal fluid (CSF) leak from the wound in seven, and CSF orbitorrhea in three patients. Sixteen had irreparable injury t

Chronic rhinosinusitis with nasal polyps: predictors of recurrence 5 years after surgery.

BACKGROUNDS: Chronic rhinosinusitis with nasal polyps (CRSwNP) is a medical condition characterized by persistent inflammation of the nasal mucosa and sinuses, accompanied by the presence of nasal polyps. When medical treatments prove ineffective, endoscopic sinus surgery (ESS) is recommended, yielding variable outcomes and recurrence rates ranging from 5% to 60%. AIMS/OBJECTIVES: The primary objectives of this study were to evaluate the outcomes of ESS and to identify independent predictors of recurrence and revision surgery. MATERIAL AND METHODS: Retrospective cross-sectional study, focusi

Headaches provoked by cortical stimulation: Their localizing value in focal epileptic seizures.

OBJECTIVE: Electrical stimulations performed in awake patients identified dura mater, venous sinuses, and arteries as pain-sensitive intracranial structures. However, cephalic pain has been only occasionally reported in patients with epilepsy undergoing stereo-electroencephalography (SEEG) stimulations. METHODS: The aim of our study was to investigate whether headache can be triggered by SEEG stimulations and might be related to specific cortical areas. Data were gathered from 16 050 stimulations collected in 266 patients who underwent a SEEG as part of a presurgical assessment of their drug-

Anatomic Variations of Ethmoid Roof and Risk of Skull Base Injury in Endoscopic Sinus Surgery: Statistical Correlations.

BACKGROUND: Recent developments in endoscopic sinus surgery (ESS) have increased the need to investigate the complex anatomic variations in the ethmoid roof and skull base, to inform the surgeon about the risk of damaging these crucial areas during ESS. OBJECTIVE: To offer a detailed description of sinus anatomy focusing on the key surgical landmarks in ESS and frontal recess surgery to standardize a systematic approach during the preoperative sinuses imaging evaluation.Methodology: A total of 220 computed tomography (CT) scans were reviewed to obtain six sets of measurements: the depth of th

A Case Report of Pycnodysostosis Associated with Multiple Pituitary Hormone Deficiencies and Response to Treatment.

Pycnodysostosis is a rare autosomal recessive osteosclerotic bone disorder associated with short stature and multiple bony abnormalities. Growth hormone (GH) deficiency may contribute to short stature in about 50% of patients. Available literature has rarely reported other pituitary hormone deficiencies in pyknodysostosis. Though the management remains conservative, recombinant human GH (rhGH) has been tried in selected patients. Here we present a case of pycnodysostosis which was evaluated for associated co-morbidities and found to have multiple pituitary hormone deficiencies. A 7-year-old gi

Incorporation of particulated bone implants in the facial skeleton.

The purpose of this study was to compare the regenerative response of autogenous cortical and cancellous bone chips and a natural particulate resorbable bone mineral (RBM) (Bio-Oss, Geistlich-Pharma, Wolhusen, Switzerland) in standardized bony defects relating paranasal sinuses to one another and to bone blocks. On 13 skeletally mature female goats four standardized critical-sized full thickness bone defects were made in the frontal bone overlying the frontal sinus. These defects were filled at random with cortical bone chips, cancellous bone chips, spongiosa granules of a RBM or left empty. F

Malignant Transformation of a Brain Neurenteric Cyst: A Case Report.

BACKGROUND & OBJECTIVE: Neurenteric cysts (NCs) are rare, benign congenital anomalies originating from the endoderm, and they rarely undergo malignant transformation. Only a small number of such cases have been reported in the literature. CASE REPORT: A 55-year-old man presented with a one-month history of headache and diplopia. Magnetic resonance imaging (MRI) showed a 49 × 41 × 54 mm solid mass in the left frontal lobe, extending into the frontal, sphenoid, and ethmoid sinuses as well as the left orbit. The patient underwent craniotomy for tumor resection. Histopathology showed small remnan

The sinus septi nasi and other minor pneumatizations of the nasal septum.

The osseous nasal septum (NS) consists of the perpendicular plate of the ethmoid bone (PPE) and the vomer bone. Few studies evaluated the possibilities of septal pneumatization of the PPE, or adjacent to it. We aimed to observe the anatomical possibilities of NS pneumatizations. A retrospective lot of cone-beam computed tomography (CBCT) files was used. One hundred seventy-one CBCT files from 51 males and 120 females were documented. There were found 46 files that were null for septal pneumatization. The other cases (73.1%) had different septal pneumatizations extended from neighboring air spa

Use of rotational periosteal flaps in primary repair of sinofacial fractures in seven horses.

Case history: Medical records were reviewed of horses (n = 7) undergoing surgery for fracture of one or more facial bones extending into the paranasal sinuses that was repaired primarily within 24 hours of the time of injury using a rotational periosteal flap, between April 2009 and May 2017. A kick from another horse was the cause of the injury of three horses, and one horse was injured when it collided with a tree. The cause of the injury of three horses was unknown.Clinical findings and treatment: Fractures were of the right maxillary bone in two horses, the left maxillary bone in two horse

A possible venous connection between the cranial and nasal cavity.

PURPOSE: The foramen cecum (FC) is a fine bony canal with the aperture located immediately anterior to the crista galli (CG). The venous structures in the regions of the FC and CG have been inconsistently described and are not well understood. Here we explore these veins using magnetic resonance imaging. MATERIALS AND METHODS: We enrolled 101 patients who underwent contrast examinations and exhibited intact skin, skull, dura mater, and intracranial dural sinuses. Imaging data were obtained as thin-sliced, seamless sagittal sections and were transferred to a workstation for analysis. RESULTS:

Chapter 1: Sinonasal anatomy and function.

An understanding of paranasal sinus anatomy based on important fixed landmarks rather than variable anatomy is critical to ensure safe and complete surgery. The concept of the paranasal surgical box defines the anatomic limits of dissection. The boundaries of the surgical box include the middle turbinate medially, orbital wall laterally, and skull base superiorly. The "vertical component" of the surgical box defines the boundaries of the frontal recess and includes the middle turbinate and intersinus septum medially, medial orbital wall and orbital roof laterally, nasofrontal beak anteriorly,

Highly effective unconventional management of aspergillosis of the left maxillary sinus in an 11-year-old girl with rhabdomyosarcoma embryonale of the frontal sinus.

Invasive fungal infections are common causes of death in children treated for malignancies, and therefore present an important and growing clinical problem. Fungal invasion usually affects immunocompromised patients, but increased incidences are also associated with intensification of antineoplastic therapy and increased numbers of organ and bone marrow transplantations. Fungal infections in parameningeal and cerebral locations carry high risks of treatment failure. We describe the case of an 11-year-old female patient with rhabdomyosarcoma embryonale of the frontal sinuses with metastases to

[Multifactorial analysis of preoperative functional symptoms in nasal polyposis (report of 403 patients)].

PURPOSE: Nasal polyposis is an invading disease whose evolution is dominated by functional symptoms. The aim of this study was to evaluate in cortico-resistant patients with nasal polyps the pre-operative functional symptoms in order to define factors that might increase them. METHODS: 403 patients were included in this study. All of them met the inclusion criteria of follow-up and initial examination. Anosmia, pain, nasal obstruction, rhinorrhea and sneezing were evaluated by patients themselves. Radiological data, respiratory function test results and histological data were matched. RESULT

[Midfacial degloving: an alternative approach to the frontobasal area, the nasal cavity and the paranasal sinuses].

Midfacial degloving can be characterized as an alternative surgical approach for exposing the bony structures of the midface. In combination with transient partial osteotomies the nasal cavities, the paranasal sinuses, the pterygopalatine fossa and the posterior parts of the anterior skull base are easily accessible. Using an intercartilaginous, a transseptal and a circumvestibular incision in the nose and a vestibular incision in the oral cavity the soft tissues of the upper face are mobilized and transposed cranially up to the infraorbital rim, the nasion and the lacrimal sac. Thus one can a

[Dural arteriovenous malformation of the anterior fossa with subarachnoid hemorrhage].

Idiopathic dural arteriovenous malformation which occurs in the posterior fossa uses predominantly transverse and sigmoid sinuses. Cavernous sinus comes next and others are rather rare. However, we have recently experienced such a rare case which was operated on and cured completely. The malformation was fed through the anterior ethmoid artery and drained to the cortical vein. The case was a 36-year-old male and admitted in our clinic for having headache and nausea as the chief complaints. He was diagnosed subarachnoid hemorrhage due to the following findings: CT scans showed a high density zo

The anatomy and histology of the nasal cavity of the koala (Phascolarctos cinereus).

The anatomy of the nose of the koala was studied from fixed 0.5 cm thick sections of a whole head. Right and left nasal cavities are separated by a slender septum which does not exhibit 'swell bodies'. Dorsal and ventral conchae are simple curved plates without elaborate scrolls; the ventral concha is recurved to form a bulla. The nasal cavity communicates with confluent rostral maxillary and frontal air sinuses. A ventrocaudal maxillary sinus opens from the ventral meatus close to the choanae. There is a frontal sinus dorsocaudal to the ethmoid region which does not communicate with the nasal

Meningotheliomatous meningioma accompanied by aspergillosis at the skull base.

A 73-year-old man was admitted because of right frontal headache and gradual loss of right visual acuity, which had been occurring for 1 year. He had been treated with corticosteroids under the diagnosis of retrobulbar optic neuritis at a nearby clinic. Magnetic resonance imaging (MRI) revealed a nodular lesion at the tuberculum sellae, which showed isointensity on T1-weighted images, iso- to low-intensity on T2-weighted images, and heterogeneous enhancement with Gd-DTPA. Meningioma was diagnosed, and surgery was performed but was limited to biopsy because of intraoperative detection of purule

[Congenital intranasal cephalocele: diagnosis and treatment].

The term cephalocele indicates a rare congenital malformation in which intracranial contents are extended through a defect in the cranium and dura mater. Intranasal cephaloceles belong to the group of basal cephaloceles. They can easily be misdiagnosed as nasal polyps and this can be potentially fatal after erroneous polypectomy. Three cases of transethmoidal cephalocele are presented, each with intermittent cerebrospinal fluid (CSF) rhinorrhea. The presence of a positive 2-transferrin-band in the immunological tests performed on the nasal fluid proved particularly helpful in diagnosing CSF. O

Sphenoid sinusitis. A review of 30 cases.

We studied 30 patients with infectious sphenoid sinusitis (15 acute cases and 15 chronic cases) in an effort to characterize the clinical presentation, bacteriology, and associated complications of this frequently misdiagnosed infection. Severe frontal, temporal, or retro-orbital headache that radiated to the occipital regions or pain in the trigeminal (V1 to V3) distribution or both were the most prominent presenting symptoms. In acute cases, purulent exudate was frequently seen in the middle and superior nasal turbinates. Computerized axial tomography or sinus tomography and cannulation of t
