Isolated pancreatic tuberculosis presenting as mucinous cystic lesion.

A young man in his early 30s presented to our hospital with upper abdominal pain radiating to the back, progressive jaundice with pruritus, low-grade fever, vomiting and significant weight loss over 4 months. The syndrome of extrahepatic biliary obstruction was considered. There was no significant past, personal or family history. Imaging was suggestive of mucinous cystic lesions of the pancreas with significant lymphadenopathy. He was initially planned for surgery. However, endoscopic ultrasound-guided fine-needle aspiration from the lesion tested positive for tuberculosis and he improved dra

[Hemobilia into a metallic biliary stent due to pseudoaneurysm: a case report].

A 48-year-old man with locally advanced pancreatic cancer underwent combined treatment with gemcitabine and proton radiation therapy. Because of subsequent obstruction of the common bile duct, a metallic biliary stent was placed and he received further gemcitabine chemotherapy. During chemotherapy, he developed an acute abdomen with a sudden-onset of tarry stool and jaundice. Gastroduodenoscopy revealed hemobilia from the biliary metallic stent. Contrast-enhanced abdominal computed tomography revealed the presence of a pseudoaneurysm arising from the right hepatic artery adjacent to the top of

Right hepatic artery false aneurysm secondary to acalculous cholecystitis.

The present report describes a case of an extrahepatic right hepatic artery pseudoaneurysm caused by acalculous cholecystitis. An 85-year-old man was admitted with abdominal pain, cachexia and jaundice. A CT scan showed a saccular false right hepatic artery aneurysm within a soft tissue and fluid mass that was causing biliary obstruction. The soft tissue mass raised the possibility of malignant disease, but this resolved slowly after a period of time and on reviewing the patient's history the only upper gastrointestinal problem noted was an episode of acalculous cholecystitis 3 months previous

Unconjugated hyperbilirubinaemia in achalasia.

Moderate unconjugated hyperbilirubinaemia decreasing after pneumatic dilatation of the gastrooesophageal sphincter, so permitting a normal amount of food to be taken was observed in two patients with achalasia. Liver biopsy was performed, and hepatic digitonin-activated bilirubin UDP-glucuronyltransferase activity was decreased in both, as is usually found in patients with Gilbert's syndrome. In the patients examined the slight and variable hyperbilirubinaemia associated with Gilbert's syndrome seemed thus to have been aggravated because of the decreased food intake due to achalasia. This situ

Transjugular intrahepatic portosystemic shunt for severe jaundice in patients with acute Budd-Chiari syndrome.

AIM: To evaluate the feasibility of transjugular intrahepatic portosystemic shunt (TIPS) for severe jaundice secondary to acute Budd-Chiari syndrome (BCS). METHODS: From February 2009 to March 2013, 37 patients with severe jaundice secondary to acute BCS were treated. Sixteen patients without hepatic venule, hepatic veins (HV) obstruction underwent percutaneous angioplasty of the inferior vena cava (IVC) and/or HVs. Twenty-one patients with HV occlusion underwent TIPS. Serum bilirubin, liver function, demographic data and operative data of the two groups of patients were analyzed. RESULTS: T

Gastro-pancreaticostomy using a tube stent as a less invasive palliative treatment for malignant obstructive pancreatitis.

Surgical palliation of malignant obstructive pancreatitis poses a considerable burden to patients, especially those with extensive disease. We herein present a novel technique for performing gastro-pancreaticostomy using a tube stent as a less invasive palliative treatment for malignant obstructive pancreatitis. The main pancreatic duct was punctured via the wall of the pyloric antrum. After the insertion of a guidewire, a double pigtail-type tube stent with side holes was inserted into the main pancreatic duct through the wall of the pyloric antrum. This technique was useful as a means of acc

EXTRA-PANCREATIC MANIFESTATION IN AUTOIMMUNE PANCREATITIS VERSUS CONVENTIONAL PANCREATITIS: AN EGYPTIAN COHORT.

BACKGROUND: Autoimmune pancreatitis (AIP) is a specific form of pancreatitis that is characterized by obstructive jaundice and sometimes associated with pancreatic masses, lymphoplasmacytic infiltrate and fibrosis, with a marked response to steroids. According to International Consensus Diagnostic Criteria AIP is categorized into type 1, type 2, and not otherwise specified (NOS). AIP-1 is one of the presentations of the IgG4-related disease (IgG4-RD) characterized by lymphoplasmacytic infiltration and more than ten IgG4-positive plasma cells per high-power field (HPF), storiform fibrosis, and

Obstructive urinary tract infection-related septic shock in late pregnancy complicated by β-thalassemia intermedia and whipworm infection: a case report.

BACKGROUND: Sepsis is a leading cause of maternal mortality. Diagnosis in pregnancy is often delayed by non-specific symptoms and legitimate caution regarding diagnostic imaging. This case illustrates a complex diagnostic challenge: septic shock in a migrant woman, where unfamiliar, endemic comorbidities obscured the typical clinical picture. CASE PRESENTATION: An 18-year-old primigravida at 26 + 5 weeks’ gestation presented with fever and mild respiratory symptoms, which rapidly progressing to septic shock. Initial labs revealed severe microcytic anemia (hemoglobin 59 g/L), marked hyperbiliru

Adult bile duct strictures: role of MR imaging and MR cholangiopancreatography in characterization.

Bile duct strictures in adults are secondary to a wide spectrum of benign and malignant pathologic conditions. Benign causes of bile duct strictures include iatrogenic causes, acute or chronic pancreatitis, choledocholithiasis, primary sclerosing cholangitis, IgG4-related sclerosing cholangitis, liver transplantation, recurrent pyogenic cholangitis, Mirizzi syndrome, acquired immunodeficiency syndrome cholangiopathy, and sphincter of Oddi dysfunction. Malignant causes include cholangiocarcinoma, pancreatic adenocarcinoma, and periampullary carcinomas. Rare causes include biliary inflammatory p

Role of intervention for biliary and gastric/intestinal obstruction in gastric cancer with peritoneal metastasis.

BACKGROUND AND AIM: Gastrointestinal (GI) luminal obstruction or malignant biliary obstruction (MBO) is not a rare condition in gastric cancer patients with peritoneal metastasis. The role of endoscopic or percutaneous interventions is not fully elucidated in this setting. METHODS: A total of 123 patients with unresectable or recurrent gastric adenocarcinoma with peritoneal metastasis receiving intravenous and intraperitoneal paclitaxel combined with S-1 were retrospectively studied. Safety and efficacy of interventions for GI luminal obstruction and MBO were evaluated. RESULTS: A total of 2

Factors associated with acute pancreatitis in patients with impacted duodenal papillary stones: a retrospective cohort study.

OBJECTIVE: Impaction of a stone at the duodenal papilla can obstruct biliopancreatic outflow and thereby lead to acute pancreatitis. But not all patients with an impacted papillary stone (IPS) had the clinical features of pancreatitis. This study aimed to identify factors associated with acute pancreatitis in patients with IPS. METHODS: We retrospectively analyzed patients with IPS who underwent endoscopic retrograde cholangiopancreatography (ERCP) at Tianjin Nankai Hospital. Clinical factors were compared between patients with/without acute pancreatitis before ERCP. Factors associated with a

Cholemic Nephropathy: Role in Acute Kidney Injury in Cholestasis and Cirrhosis.

The concept of structural kidney damage and renal dysfunction as a result of jaundice attracted attention in the medical community in the early and mid-20th century. The postulated doctrine of the time was that the excretion of elevated concentrations of bile results in bile-stained casts occupying collecting and distal convoluted tubules, degeneration of tubular epithelium, and decreased renal function. Compared to the hepatorenal syndrome, the poster child of hepatology and nephrology collaboration, the notion of structural kidney damage and renal dysfunction as a result of cholemia lost its

Laparoscopic cholecystectomy after transcatheter arterial embolisation for haemobilia due to a pseudoaneurysm in the gallbladder: A case report.

Pseudoaneurysms of the cystic artery are a rare but significant complication of acute cholecystitis. Laparoscopic cholecystectomy may be a safe alternative to open cholecystectomy in such cases. We report the case of a female patient in her seventies, who presented with anaemia, jaundice, and hepatic dysfunction during hospitalisation for right knee pyogenic arthritis. The patient had no pain in the right upper quadrant. Her serum haemoglobin level was 6.5 g/dL, and upper gastrointestinal endoscopy revealed no abnormalities. Six days later, jaundice worsened, and endoscopic retrograde cholangi

Extrahepatic biliary obstrution secondary to neuroendocrine tumor of the common hepatic duct.

INTRODUCTION: Primary neuroendocrine tumors (NET) of the extrahepatic biliary tree are a rare entity with less than 100 reported cases in the literature. PRESENTATION OF CASE: Here, we report a case of NET of the extrahepatic bile duct in a 64-year-old male patient presenting with painless jaundice, direct hyperbilirubinemia, and mildly elevated transaminases. Diagnostic workup with an ultrasound revealed dilation of the intrahepatic biliary ducts, without cholelithiasis or choledocholithiasis. Additional cross sectional imaging identified a stricture at the confluence of the common hepatic a

Anomalous development of the hepatobiliary system in the Inv mouse.

Extrahepatic biliary atresia (BA) is a devastating disease of the neonate in which the hepatic and/or common bile duct is obliterated or interrupted. Infants and children with this diagnosis constitute 50% to 60% of the pediatric population that undergoes orthotopic liver transplantation. However, there is still very little known about the etiology and pathogenesis of BA. Several recent studies have demonstrated that anomalies of situs determination are more commonly associated with BA than previously recognized. In this study, we examined the pathogenesis of jaundice in the inv mouse, a trans

[Differential diagnosis of jaundice by combined noninvasive diagnostic means using a logistic discriminant analysis].

Using a logistic discriminant function we retrospectively evaluated the diagnostic value of laboratory features and abdominal sonography in 70 patients with jaundice. 18 patients had an extrahepatic obstruction of the common bile duct (EHO), 22 patients had metastatic liver disease (MLD) and 30 patients had an infectious or toxic hepatocellular disease (HCD). The sensitivity resp. specificity of the 5 laboratory values AP, GGT, GPT, GOT and bilirubin was 22% resp. 90% for diagnosing EHO, 82% resp. 79% for diagnosing MLD and 67% resp. 68% for diagnosing HCD. The diagnostic value determined by t

Use of SpyGlass™ DS II Direct Visualization System With SpyBite™ Max Forceps for the Removal of Echinococcus Granulosus Causing Biliary Obstruction.

Hydatid disease, also known as cystic echinococcosis, affecting the liver can vary in clinical presentation from uncomplicated to most complicated one that can rupture into the bile duct. In the past, surgery was the mainstay of treatment for biliary complications. However, with the increasing expertise of endoscopic retrograde cholangiopancreatography, biliary complications are less common. Endoscopic retrograde cholangiopancreatography can be used preoperatively and postoperatively in patients presenting with jaundice to treat biliary obstruction and biliary fistula. In our case report, we i

Pulmonary Embolism in Long Standing Diabetes: A Hint Towards Pancreatic Carcinoma.

Pancreatic carcinoma has remained one of the leading causes of cancer-related mortality worldwide. Cancer originating in the head of pancreas is often detected early in the disease due to biliary obstruction resulting in jaundice. In contrast, cancer of the pancreatic body and tail remains indolent, presenting late with significantly increased tumor burden and distant metastasis. Unfortunately, a single laboratory screening study is neither sensitive nor specific for early detection of pancreatic cancer. In this report, we present a patient with longstanding diabetes incidentally detected to h

[Treatment of Ampullary Adenocarcinoma].

The ampulla of Vater is a small projection formed by the confluence of the main pancreatic duct and common bile duct in the second part of the duodenum. Primary ampullary adenocarcinoma is a rare malignancy, accounting for only 0.2% of gastrointestinal cancers and approximately 7% of all periampullary cancers. Jaundice from a biliary obstruction is the most common symptom of ampullary adenocarcinoma. In the early stages, radical pancreatoduodenectomy is the standard surgical approach. On the other hand, no randomized controlled trial has provided evidence to guide physicians on the choice of a
