Familial partial lipodystrophy (FPLD) is characterized by abnormal fat distribution and a metabolic syndrome with hypertriglyceridemia. We identified a family with a severe form of FPLD3 with never-reported clinical features and a novel mutation affecting the DNA binding domain of PPARγ (E157D). Apart from the lipodystrophy and severe metabolic syndrome, individuals presented musculoskeletal and hematological issues. E157D heterozygotes had a muscular habitus yet displayed muscle weakness and myopathy. Also, E157D heterozygotes presented multiple cytopenias and a susceptibility to autoimmune d

CASE HISTORY: In 2008, six lambs within a flock of Dorper-cross sheep were born with musculoskeletal and neurological disease. Clinical signs included hindlimb weakness, and urinary incontinence.
CLINICAL FINDINGS: All lambs had focal, inverted areas of alopecic skin over the caudal sacrum, and short, often kinked tails. Four affected lambs were subject to euthanasia, and necropsied. On gross examination, the arches of sacral vertebrae were absent, and spinal nerves and meninges were adherent to the overlying subcutis. Other gross lesions included narrow, elongated skulls, herniation of the o

Musculoskeletal evaluation skills are needed to examine postural compensation techniques, but little is known about ground reaction forces (GRF) in standing posture. Even though a number of studies have evaluated GRF in patients with low back pain (LBP) during vertical perturbations, it would be important to consider compensation characteristics which might be associated with abnormal patterns of postural responses. The vertical excursions of the body center of mass (BCOM) were measured with delay time and normalized amplitude of GRF. Overall, there was no difference based on the vertical excu

OBJECTIVE: To determine whether magnetic resonance imaging [MRI] in planes of imaging parallel to the known course of the extrinsic radiocarpal ligaments of the wrist aids in the visualization of these structures.
SUBJECTS AND METHODS: MRI was performed on 32 wrists: a single normal cadaveric wrist, 23 normal wrists in volunteers and 8 wrists in patients with known scapholunate dissociation and rotatory scaphoid subluxation. Proton-density and T2-weighted images were obtained in the sagittal plane and gradient recalled echo images in an oblique sagittal plane, selected to parallel the extrins

BACKGROUND: Costello syndrome (CS) is a rare genetic disorder within the spectrum of RASopathies, caused by activating mutations in the HRAS gene, leading to constitutive dysregulation of the RAS/MAPK signalling pathway. Among its multisystemic manifestations, a distinctive musculoskeletal involvement is frequently observed with reduction in muscle force, pain of musculoskeletal origin and muscular hypotrophy. Moreover, abnormal histological findings with variability in size, atrophy, and prevalence of type 2-fibers have been detected on anecdotal muscular biopsies of affected individuals.

Osteoporosis (OP) is a prevalent degenerative musculoskeletal disorder in middle-aged and elderly populations, characterized by reduced bone mineral density and an increased risk of fragility fractures. Its pathogenesis is closely linked to oxidative stress imbalance; however, effective long-term therapeutic options remain limited. Here, we report that diacerein, a clinically used drug for osteoarthritis, significantly ameliorates OVX (ovariectomy)-induced bone loss and abnormal bone metabolism. Mechanistically, we demonstrate that diacerein activates the transcription and activity of NRF2 (nu

Tendinopathy is a disease of the musculoskeletal system resulting from excessive or abnormal mechanical loading of tendons. Reconstruction of disorganized fibers remains a preceding issue and challenge in sports medicine. Therefore, developing novel delivery systems to promote regeneration after tendinopathy is imminently desired. A dual-dynamic crosslinked framework was constructed here through ligand bonds created between the amino or carboxyl groups in gelatin and Fe3+, as well as hydrogen-binding between gelatin and curcumin-grafted carboxymethyl chitosan (CMCS). Fibromyalgia represents a chronic pain disorder characterized by musculoskeletal pain, fatigue, and cognitive impairments. The exact mechanisms underlying fibromyalgia remain undefined; as a result, diagnosis and treatment present considerable challenges. On the other hand, the endogenous opioid system is believed to regulate pain intensity and emotional responses; hence, it might be expected to play a key role in the enhanced sensitivity experienced by fibromyalgia patients. One explanation for the emergence of disrupted pain modulation in individuals with fibromyalgia is a significant reduct

Nociplastic pain is defined as pain due to sensitization of the nervous system, without a sufficient underlying anatomical abnormality to explain the severity of pain. Nociplastic pain may be manifest in various organ systems, is often perceived as being more widespread rather than localized and is commonly associated with central nervous system symptoms of fatigue, difficulties with cognition and sleep, and other somatic symptoms; all features that contribute to considerable suffering. Exemplified by fibromyalgia, nociplastic conditions also include chronic visceral pain, chronic headaches an

Tendons are complex connective tissues that transmit tensile forces between muscles and tendons. Tendon injuries are among the most common orthopedic problems with long-term disability as a frequent consequence due to prolonged healing time. Furthermore, the repair tissue is of inferior quality, predisposing patients to high rates of recurrence following initial injury. Coordinated cellular processes and biological factors under the influence of mechanical loading are involved in tendon healing and our understanding of these events lags behind other musculoskeletal tissues. Tendons are relativ

BACKGROUND: A virtual navigation system (VNS) can combine two imaging modalities for synchronous observation and can be beneficial for diagnosis and treatment. Ultrasound elastography (UE) can distinguish between soft and hard tissues. However, the application of UE to musculoskeletal structures is rare.
OBJECTIVE: To evaluate UE of the supraspinatus tendon using ultrasound-magnetic resonance imaging (US-MRI) virtual navigation.
METHODS: Sixty patients with an ache in the shoulder were diagnosed with supraspinatus tendon injuries using MRI. US-MRI virtual navigation was used to identify lesi

INTRODUCTION: Three major features result from this abnormality: a short neck, a limited range of motion in the neck, and a low hairline at the back of the head. Most affected people have one or two of these characteristic features. Less than half of all individuals with Klippel-Feil syndrome have all three classic features of this condition. The etiology of Klippel-Feil syndrome and its associated conditions is unknown. The syndrome can present with a variety of other clinical syndromes, including fetal alcohol syndrome, Goldenhar syndrome, anomalies of the extremities etc. Associated anomali

UNLABELLED: : Tethered cord syndrome occurs when the distal spinal cord or filum adheres to adjacent structures resulting in progressive sensorimotor deficits in the lower extremities, fecal and/or urinary incontinence, and musculoskeletal deformities. Tethering of the distal cord may be idiopathic, may be associated with an intraspinal abnormality such as a lipoma, but most commonly the distal spinal cord remnant is adherent to the area of the original dysraphism repair in patients with myelodysplasia. Surgery to untether the cord is indicated in patients with worsening pain symptoms, progres

A family is described in which 5 of 9 living children were found to have restrictive cardiomyopathy associated with skeletal muscle and orthopedic abnormalities. In the absence of another identifiable etiology, a genetic cause for restrictive cardiomyopathy in this family is probable. Consistent with the poor prognosis encountered for children with restrictive cardiomyopathy, 2 children in this family died, whereas a third was symptomatic by age 3 years.

Occupational back pain in nurses (OBPN) constitutes a major source of morbidity in the health care environment. According to the National Institute for Occupational Safety and Health (NIOSH), occupational back injury is the second leading occupational injury in the United States. Among health care personnel, nurses have the highest rate of back pain, with an annual prevalence of 40-50% and a lifetime prevalence of 35-80%. The American Nursing Association believes that manual patient handling is unsafe and is directly responsible for musculoskeletal disorders encountered in nurses. It has been

BACKGROUND: Chromosome 19 is the most gene-dense chromosome in the human genome, with a high frequency of segmental duplications that predispose it to genomic rearrangements. While deletions of chromosome 19 have been associated with various clinical conditions, duplications remain poorly characterised. Here, we report three cases involving 19p13.11 duplication and describe the associated clinical phenotype.
METHODS: We describe three unrelated individuals with microduplications at 19p13.11 identified either via clinical whole-exome sequencing or chromosomal microarray. The probands underwent
