Ataxia with oculomotor apraxia type 2: a clinical and genetic study of 19 patients.

Ataxia with oculo-motor apraxia type 2 (AOA2) is a recently described autosomal recessive cerebellar ataxia (ARCA) caused by mutations in the senataxin gene (SETX). We analysed the phenotypic spectrum of 19 AOA2 patients with mutations in SETX, which seems to be the third most frequent form of ARCA in Algeria after Freidreich ataxia and Ataxia with vitamin E deficiency. In AOA2 patients, the mean age at onset for all families was in the second decade. Cerebellar ataxia was progressive, slowly leading to disability which was aggravated by axonal polyneuropathy present in almost all the patients

Principles of facial trauma: orbital fracture management.

Often, complex, optimal management of the orbital fracture is directly dependent on thorough initial evaluation, correct injury assessment, and timely initiation of chosen therapy. Most often, secondary to assault or motor vehicle collision, these fractures are frequently associated with additional traumatic injury. With recent advances in imaging, alloplastic materials, and bone fixation technology, the evolution of orbital fracture management now enables reconstruction of even the most severe injuries.

Correlating the cochleovestibular deficits with tumor size of acoustic neuroma.

CONCLUSION: The tumor size of acoustic neuroma correlates with cochleovestibular deficits. Those tumors with global frequency hearing loss, bilateral gaze nystagmus, or absent caloric and VEMP responses may indicate a tumor size >2.5 cm. OBJECTIVE: This study aimed to investigate the correlation between cochleovestibular deficits and the size of acoustic neuroma. PATIENTS AND METHODS: A total of 44 patients with acoustic neuroma were enrolled in this study. Pure tone audiometry, electronystagmography, caloric test, vestibular evoked myogenic potential (VEMP) test, and MRI were conducted. RE

Auditory agnosia caused by a tectal germinoma.

The authors describe a patient with auditory agnosia caused by a tectal germinoma. Despite having normal audiometric tests, the patient failed to recognize words and musical characters. On head MRI, the inferior colliculi were infiltrated by tumor. Neuropsychological tests revealed severe impairment in recognition of environmental sounds and words, defective musical perception, and stop consonant-vowel discrimination. Inferior colliculus may play a role in the analysis of sound properties.

[Detailed results of orbital and optic nerve decompression].

Orbital complications of various pathogenesis include impairment or loss of vision inhibition of globe motility and exophthalmos, and are treated best by an interdisciplinary team. Preoperative strategy should include information about underlying diseases, since success of treatment is related to diagnosis. To analyse our postoperative results we examined four groups of cases: injuries, tumors, infections and mucoceles. Diseases were located in the orbit or paranasal sinuses and produced similar symptoms. But prognosis and results of surgical treatment produced significant differences. The bes

Clinical features and disease course of neurological involvement in Behcet's disease: HUVAC experience.

BACKGROUND/AIM: Neurological involvement (Neuro-Behcet's Disease: NBD) is a rare manifestation of Behcet's Disease (BD) and it is related with significant mortality and morbidity. We aimed to evaluate disease course and outcome of NBD patients registered in Hacettepe University Vasculitis Center (HUVAC) prospective database starting from October 2014. METHODS: Totally, 419 patients (329 of the patients had fulfilled the International Study Group (ISG) criteria and 90 patients were considered as incomplete BD) were recorded as BD to March 2018. We retrospectively reviewed the charts of 123 pat

Supracerebellar infratentorial approach with paramedian expansion for posterior third ventricular and pineal region lesions.

BACKGROUND: Surgical treatment for lesions in the posterior third ventricle is technically challenging. Surgical approaches to this area carries a risk of both venous and neural injury, with subsequent morbidity. Several approaches are used to reach the pineal region. The supracerebellar infratentorial approach is the commonly approach used for such lesions. OBJECTIVES: This work describes adding a paramedian expansion to the "classical median supracerebellar infratentorial approach" for posterior third ventricular and pineal region lesions with lateral expansion. This study discusses the res

Lyme neuroborreliosis: a treatable cause of acute ocular motor disturbances in children.

BACKGROUND: Lyme neuroborreliosis (LNB) designates central nervous system involvement caused by the tick-borne spirochaete Borrelia burgdorferi (Bb). The present study describes a spectrum of acquired ocular motor disorders in children with LNB. METHODS: Six paediatric patients (age 3-15 years) with ocular motor symptoms as first manifestations of LNB evaluated by a paediatrician and ophthalmologist are presented. Diagnosis was based on new onset ocular motor disturbances and detection of cerebrospinal fluid (CSF) pleocytosis and intrathecal synthesis of Bb IgM and/or IgG antibodies by lumbar

Orbital floor restoration with traction of the infraorbital nerve using a vessel loop in posterior orbital floor fractures.

BACKGROUND: The authors retracted the infraorbital nerve (ION) using a vessel loop to explore the orbital floor fracture site and analysed when the traction of the ION was needed. METHODS: In ninety-one patients, the location of the fracture according to its position relative to the infraorbital groove, the location of the infraorbital groove from the midpoint of the orbital floor, and involvement of the ION in the fracture site were recorded retrospectively from computed tomography scans. An analysis of any associations between the traction of ION and the location of the fracture and ION pas

Orbital fractures in children: a review of outcomes.

The third most common facial fractures in children are fractures of the orbit, and the medial wall and floor are the commonest sites affected. The aetiology, clinical presentation, and timing of operation all differ from those of adults. If there are few or no clinical signs, but oculocardiac reflex is present, it is highly suggestive of trapdoor injury. This retrospective study includes all consecutive children (younger than 18 years) referred with confirmed fractures of the orbital floor over a 5-year period (2005-2010). A total of 24 patients were identified with a mean age of 13.5 years, a

Ocular flutter associated with a lesion of the right upper pontine tegmentum.

Ocular flutter is a relatively rare eye movement disorder characterized by intermittent bursts of conjugate horizontal saccades without an intersaccadic interval. Its anatomical and neurophysiological basis remains controversial. Here we present a case of ocular flutter in which the flutter declined as the clinical stage of the associated ailment declined, until it eventually disappeared. Radiological examination revealed a tiny lesion in the right upper pontine tegmentum that included areas of the reticular formation, superior cerebellar peduncle and locus ceruleus. Because the lesion occupie

Vitreous hemorrhage and Rhegmatogenous retinal detachment that developed after botulinum toxin injection to the extraocular muscle: case report.

BACKGROUND: The authors report a case of a rare complication that occurred after botulinum toxin injection to the extraocular muscle, which was easily overlooked and successfully corrected by surgery. CASE PRESENTATION: A 34-year-old female patient visited our clinic for diplopia and ocular motility disorder after removal of an epidermoid tumor of the brain. At her initial visit, her best-corrected visual acuity (BCVA) was 20/20 for both eyes. An alternate cover test showed 45 prism-diopter esotropia and 3 prism-diopter hypertropia in the right eye. Following 6 months of observation, the devi

[Hypertrophic pachymeningitis and ophthalmological disturbances: description of two case reports].

CASE REPORTS: We report two cases of hypertrophic pachymeningitis with ophthalmological disturbances. One patient suffered from hypertrophic pachymeningitis supposedly secondary to a focal neurosarcoidosis and had a sixth nerve paresis. The second patient suffered from a diffuse idiopathic hypertrophic chronic pachymeningitis (a rare form) and displayed disturbances in vision and ocular motility. DISCUSSION: Hypertrophic pachymeningitis is a condition with a wide spectrum of etiologies and clinical manifestations, and needs to be considered as the cause in patients with alterations in ocular

Bilateral frontoparietal polymicrogyria: clinical and radiological features in 10 families with linkage to chromosome 16.

Polymicrogyria is a common malformation of cortical development characterized by an excessive number of small gyri and abnormal cortical lamination. Multiple syndromes of region-specific bilateral symmetric polymicrogyria have been reported. We previously have described two families with bilateral frontoparietal polymicrogyria (BFPP), an autosomal recessive syndrome that we mapped to a locus on chromosome 16q12-21. Here, we extend our observations to include 19 patients from 10 kindreds, all linked to the chromosome 16q locus, allowing us to define the clinical and radiological features of BFP

A subgroup of spinocerebellar ataxias defective in DNA damage responses.

A subgroup of human autosomal recessive ataxias is also characterized by disturbances of eye movement or oculomotor apraxia. These include ataxia telangiectasia (A-T); ataxia telangiectasia like disorder (ATLD); ataxia oculomotor apraxia type 1 (AOA1) and ataxia oculomotor apraxia type 2 (AOA2). What appears to be emerging is that all of these have in common some form of defect in DNA damage response which could account for the neurodegenerative changes seen in these disorders. We describe here sensitivity to DNA damaging agents in AOA1 and evidence that these cells have a defect in single str

The characteristics of midfacial fractures and the association with ocular injury: a prospective study.

Ocular injuries commonly occur in patients with facial fractures. This prospective study was set up to determine the incidence of ocular injuries, as assessed by an ophthalmologist, in patients who had sustained midfacial fractures. Over a 2-year period, a study of 363 patients who had sustained midfacial trauma sufficient to lead to a facial bone fracture (438 fractures) was undertaken and patients received a comprehensive examination by an ophthalmologist and an orthoptist within 1 week of injury. The characteristics of the eye injuries sustained were related to the aetiology of the fracture
