We report the case of a 42-year-old female patient who developed peculiar skin lesions due to subcutaneous polyethylene glycol (PEG) interferon beta-1a. The dermatological examination showed hypochromic macules that had coalesced into a 10-cm-diameter patch. On the abdomen injection sites, there was a greyish diffuse hyperpigmentation arranged irregularly in annular macules. Fungal infection, vitiligo and pityriasis alba were excluded. After 6 months, the lesions had worsened. This is the first case of localized pigmentation disorder reported with interferon beta, and while the clinical findin…

While oral isotretinoin is renowned for its ability to treat acne vulgaris, many of its off-label uses continue to go underappreciated. Since the last review on the unapproved indications of isotretinoin, relevant publications have surfaced with new recommendations. This article attempts to provide physicians with the latest information regarding successful and unsuccessful use of isotretinoin as an effective treatment for dermatological conditions, such as rosacea, psoriasis, pityriasis rubra pilaris, condyloma acuminatum, granuloma annulare, Darier's disease, systemic cutaneous lupus erythem…

The Febrile Ulceronecrotic Mucha-Habermann (FUMHD) disease is a rare variant of pityriasis lichenoides et varioliformis acuta (PLEVA). Its etiology still remains unknown and it is characterized by a sudden onset of ulceronecrotic skin lesions associated with systemic symptoms. It is reported here the case of a male patient with a sudden and acute evolution of macules and papules, ulceronecrotic and vesicle-bullous lesions associated with systemic symptoms. The patient was treated with prednisone 0.5 mg/kg/day with a dramatic response. The FUMHD is a severe variant of PLEVA and its diagnosis is…

A 29-year-old woman presented for evaluation of an ulcerated nodule on her right temple in the presence of numerous additional scaly papules, plaques, and seborrheic keratosis-like lesions that were distributed on her forehead, frontal scalp, and temples. Further examination showed pityriasis rosea-like lesions on her neck, back, and arms and verruca plana-like lesions on the dorsa of the hands and forearms. A biopsy specimen of the ulcerated nodule showed a nodular basal-cell carcinoma. Two additional biopsy specimens from her forehead showed one invasive and one in-situ squamous-cell carcino…

In 1847 the student of medicine Theodor Sluyter (1817-1895) from Greifswald published his thesis in Berlin including the first well-documented case of human pulmonary aspergillosis. In 1856 Rudolf Virchow (1821-1902) classified the depicted fungi as an Aspergillus species. Possibly Carl Ferdinand Eichstedt (1816-1892) carried out the autopsy. He is known by the first description of a fungus as the cause of pityriasis versicolor in 1846. Further involved scientists from Greifswald were Wilhelm Baum (1799-1883), Theodor Litzmann (1815-1890) and the botanist Johann Konrad Schauer (1813-1848). The…

The composition of the total skin surface lipid, free fatty acids and fatty acid of triglyceride was used in discriminant analyses to classify patients with severe acne vulgaris (AV), pityriasis versicolor (PV), seborrhoeic dermatitis (SD) and atopic dermatitis (AD). It was not possible to discriminate between the PV and SD patients but all the lipid groups gave a satisfactory separation between AV, PV + SD and AD patients. A similar separation was obtained when the sex of the patient was studied but not when both sex and disease were taken into account. The results suggest that there are fund…

Hundreds of asymptomatic pale erythematous lesions, 2 to 15 mm in diameter, with complex angulated shapes in a mantle distribution over the upper portion of the chest and back and the shoulders, developed in two unrelated young men. Actinic porokeratosis, discoid lupus erythematosus, pityriasis versicolor, and acne scars were considered as possible diagnoses, but none of these appeared appropriate. Multiple skin biopsy specimens from both patients showed tumors of follicular infundibulum (infundibulomas), benign platelike proliferations of the external root sheath that were outlined by a promi…

Keratinization defects can be very severe and disfiguring diseases. The development of retinoids such as etretinate has provided us with an effective symptomatic form of oral therapy for these skin conditions. Based on our own experience, we briefly outline the therapeutic potential of etretinate in various keratinization defects (lamellar ichthyosis, Netherton syndrome, Sjögren-Larsson syndrome, mal de Meleda and juvenile pityriasis rubra pilaris). The toxicology of etretinate is reviewed with special regard to the treatment of children. Bone changes such as premature closure of the growth li…

The appearance of widely diffuse hypopigmented keratotic lesions has been linked to a number of dermatological conditions like pityriasis versicolor, stucco keratoses, confetti leukoderma, and guttate leukoderma of Darier's disease. Specific clinical and histologic characteristics distinguish them from one another. One of the differentials for diffuse hypopigmented keratotic lesions has been a novel entity called disseminated hypopigmented keratoses (DHK). We are hereby highlighting the importance of bedside investigations like potassium hydroxide wet mount and dermatoscopy in ruling out such …

Fungal infections are being increasingly reported from immuno-compromised as well as immuno-competent patients. Transplant patients are on long term immunosuppressive therapy which makes them highly vulnerable to opportunistic fungal infections .These infections can be cutaneous or systemic. Several fungi have been reported to be the culprits such as Candida spp., Aspergillus spp., C. neoformans, P. carinii, and zygomycetes group of fungi. Cutaneous infections are most commonly caused by Pityriasis (tinea) versicolor, dermatophytes, and candida sp but these days the demtiaceous fungi are becom…

Cutaneous fungal infections occur frequently in the adolescent population, and are second only to acne in generating skin-related adolescent anguish. Though superficial fungal infections of the skin are rarely life-threatening, they can cause significant worrisome cutaneous findings (e.g., white spots from pityriasis versicolor, distorted nails from onychomycosis). Such changes are particularly threatening to an adolescent who wants to be just like everybody else. This article focuses on several of the more common cutaneous fungal infections in adolescence, including pityrosporum (tinea) versi…

Demodex mites are common parasites in the hair follicles and in the pilosebaceous glands. Demodex folliculorum and D. brevis have been identified in humans and play a role in three facial conditions: Pityriasis folliculorum, rosacea-like demodicidosis and Demodicidosis gravis. The highly infected sites by this mites are: forehead, area around the orbit, area around the nose, chin and area around the mouth. Infection of the skin by D. folliculorum is more frequent among females and in the elderly people. The presence of mites can be determined by microscopic inspection of secretion from sebum g…

Skin scrapings from a patient with Pityriasis versicolor were plated on a bile salt medium containing 10% glycerol (for cryoprotection) and Pityrosporum orbiculare was harvested for freeze-fracture replication. Resting and budding cells were senn. The overall pattern of indentation of the plasma membrane towards the cytoplasm was well revealed as a spiral arrangement of major indentations with minor ones disposed more or less at right angles. Hexagonally-arranged particles were not observed. Mitochondria were elongated with constrictions at intervals, and the fracture face of the limiting memb…

Netherton syndrome (NTS) is a genetic disorder that predominantly affects the hair and the skin, and it can have a wide variety of presentations. The genetic syndrome is more common with consanguineous parents. Given the rarity and varying presentation of the condition, a few cases have been reported in the literature. We present an unusual case of two incidental diagnoses of NTS in siblings of consanguineous parents, manifesting as erythroderma and other symptoms that were initially diagnosed as pityriasis rubra pilaris and psoriasis in separate visits. Physicians must maintain a high index o…
