Renal Medullary Carcinoma: The Zebra Amongst the Horses.

A 13-year old Latino male presented with recurrent gross hematuria, 5cm right-sided poorly defined heterogeneous mass, enlarged retrocaval lymph nodes, and 1.2 cm paratracheal lymph node. Given the need for multiple blood transfusions, robot-assisted radical nephrectomy with lymph node dissection was performed. Pathology revealed pT3a high-grade tumor, clear margins, and positive lymph node. Additionally, with multiple sickled RBCs and loss of staining of SMARCB1 in tumor specimen, and hemoglobin electrophoresis suggesting sickle cell trait, diagnosis of metastatic renal medullary carcinoma wa…

Carriership of Factor V Leiden and evolutionary selection advantage.

Historically, lethal exsanguinations and severe infections have been two major causes of maternal death. Gene mutations that lower the risk of profuse hemorrhage or severe infections would give a survival advantage. A single mutation of coagulation factor V, known as FV Leiden (FVL), can be such a beneficial mutation. FVL is common among Caucasians and today confers an increased risk of thromboembolism. However, the high prevalence of FVL (up to 15%) in the general population suggests that it has given an evolutionary advantage. In this review, we discuss possible mechanisms of the evolutionar

Hyponatraemia: biochemical and clinical perspectives.

Hyponatraemia is a common bio-chemical abnormality, occurring in about 15% of hospital inpatients. It is often associated with severe illness and relatively poor outcome. Pathophysiologically, hyponatraemia may be spurious, dilutional, depletional or redistributional. Particularly difficult causes and concepts of hyponatraemia are the syndrome of inappropriate antidiuresis and the sick cell syndrome, which are discussed here in detail. Therapy should always be targeted at the underlying disease process. 'Hyponatraemic symptoms' are of doubtful importance, and may be more related to water overl

Foetal haemoglobin in Nigerian children with sickle cell anaemia. Effect on haematological parameters and clinical severity.

The relationship of foetal haemoglobin (HbF) to several clinical and haematological parameters in 86 children aged 5 to 16 years who had sickle cell anaemia (SCA) has been examined. The mean HbF level was 6.8 +/- 3.6%. Higher HbF values were significantly associated with fewer blood transfusions, fewer hospital admissions and higher rate of hepatomegaly. A trend for children with higher HbF levels to have better physical development and a more normal haemogram was also noted though this was not statistically significant.

Common problems in sickle cell disease.

Sickle cell disease is transmitted as an autosomal recessive trait. Symptoms of pallor, fever, abdominal and joint pain, and swelling of the liver, spleen, hands and feet first appear near the latter part of the first year of life. Intravascular sickling affects all organs. For clinical and therapeutic purposes, exacerbations may be classified as vasoocclusive or pain, aplastic, hemolytic or sequestration crisis. In addition to infection, complications include severe pain, cerebrovascular accidents, cholelithiasis, bone infarction, heart failure, hypotension and priapism. The most common cause

Erythrocyte sickling during exercise and thermal stress.

OBJECTIVE: To examine effects of exercise in the heat and fluid intake on erythrocyte sickling and neutrophil activation in carriers of sickle cell trait (HbAS). DESIGN, SETTING, AND PARTICIPANTS: Six African American men (2 HbAS; 42% HbS, 4 HbAA; 20.7 +/- 0.8 years; 87.4 +/- 9.6 kg) participated in 2 randomized sessions (separate days) each consisting of 45 minutes of brisk walking (treadmill) in a hot (33 degrees C) environment. INTERVENTION: Subjects consumed no fluids or fluid for 3 hours prior to (ad libitum) and during (1.02 L) testing. MAIN OUTCOME MEASUREMENTS: Core temperature, hea…

Uganda's Mandatory Newborn Sickle Cell Disease Screening Programme: Translating a Decade of Evidence into Differentiated Implementation.

Uganda's sickle cell disease (SCD) prevalence of 1.3% and sickle cell trait (SCT) prevalence of 13.3% nationally represent a substantial public health burden, with an estimated 20,000 affected infants born annually. On 9 February 2026, Uganda's Ministry of Health launched a nationwide mandatory newborn screening programme for SCD, placing the country among the first in sub-Saharan Africa to implement universal birth screening. This policy builds on over a decade of evidence, including the Uganda Sickle Surveillance Study, and cost-effectiveness analyses showing dried blood spot (DBS) screening…

Combination Therapy With Bortezomib in Renal Medullary Carcinoma: A Case Series.

BACKGROUND: Renal medullary carcinoma (RMC) is a very rare, aggressive neoplasm occurring almost exclusively in adolescents and young adults with sickle cell trait. Given the rare nature of this tumor, accounting for less than 0.5% of all renal carcinomas, most of the published data on therapies is from case reports and small case series, and current treatments are insufficient, with most patients succumbing to their disease in months. We report our experience with a cytotoxic chemotherapy regimen consisting of platinum-based therapy, doxorubicin, and bortezomib. METHODS: Three patients with …

Reduction in band 3 protein of red cells in sickle cell anaemia.

Twelve patients suffering from sickle cell anaemia (Hb less than 75 g/l) with SS-haemoglobin were studied with special regard to Band 3-protein content in their erythrocyte membranes on SDS-PAGE. They were compared in this respect with 7 asymptomatic individuals with AS-haemoglobin and 10 healthy controls with the ordinary adult haemoglobin. A heterogeneity in Band 3-protein content was evident among the 12 patients. Six of them belonged to a low content group while the other 6 belonged to a high content group comparable with the asymptomatic subjects and the controls. No simple relationship s

Sickle cell disease: implications for nursing care.

Abnormal haemoglobins produce a variety of anaemias which range in effect from being very mild to lethal. The most common of these are the sickle cell disorders and thalassaemias. In London, thalassaemia affects 3.5% of the Maltese population, 2.5% of the Italian population and 1.5% of the West Indian population respectively. Seventeen per cent of the Cypriot population carry a thalassaemia gene. Sickle cell disease occurs in 500:100,000 people in Britain's black population but is not exclusive to people of Afro-Caribbean origin. The gene is carried by 1 in 10 people. Sickle cell disease is st

Sonographic diameter of the common hepatic duct in sickle cell anemia.

The maximum diameter of the common hepatic duct was measured from the cholecystosonograms of 95 children and young adults with proven sickle cell anemia. Four mm was considered the upper limit of normal. Half the patients were male, half were female; 40% had gallstones and 25% had signs and symptoms of acute biliary diseases. The diameter of the common hepatic duct was normal in all but six patients. In five of these, the degree of enlargement was minor (1-3 mm). Three of the six patients with enlarged ducts had acute cholangitis without obstruction, two had clinically "silent" enlargements, a

Sickled erythrocytes, hyphema, and secondary glaucoma: VII. The passage of sickled erythrocytes out of the anterior chamber of the human and monkey eye: light and electron microscopic studies.

Sickled erythrocytes leave the anterior chamber of human and monkey eyes by several mechanisms: intracameral and intratrabecular hemolysis and phagocytosis; entry into and phagocytosis within the iris and ciliary body; and passage into Schlemm's canal. Sickled erythrocytes used as biologic tracers allowed definite confirmation, for the first time in the human, that intracameral erythrocytes can enter Schlemm's canal in the orthograde direction. Erythrocytes appeared to traverse the inner wall of Schlemm's canal through the cytoplasm of individual endothelial cells, rather than between adjacent
