Sociodemographic and clinical characteristics in child and youth mental health; comparison of routine outcome measurements of an Australian and Dutch outpatient cohort.

AIMS: Although of great value to understand the treatment results for mental health problems obtained in clinical practice, studies using naturalistic data from children and adolescents seeking clinical care because of complex mental health problems are limited. Cross-national comparison of naturalistic outcomes in this population is seldomly done. Although careful consideration is needed, such comparisons are likely to contribute to an open dialogue about cross-national differences and may stimulate service improvement. The aim of this observational study is to investigate clinical characteri

Does CBT facilitate emotional processing?

BACKGROUND: Cognitive Behavioural Therapy (CBT) is not primarily conceptualized as operating via affective processes. However, there is growing recognition that emotional processing plays an important role during the course of therapy. AIMS: The Emotional Processing Scale was developed as a clinical and research tool to measure emotional processing deficits and the process of emotional change during therapy. METHOD: Fifty-five patients receiving CBT were given measures of emotional functioning (Toronto Alexithymia Scale [TAS-20]; Emotional Processing Scale [EPS-38]) and psychological symptom

A latent class approach to the external validation of respiratory and non-respiratory panic subtypes.

BACKGROUND: The phenotypic variance observed in panic disorder (PD) appears to be best captured by a respiratory and non-respiratory panic subtype. We compared respiratory and non-respiratory panic subtypes across a series of external validators (temporal stability, psychiatric co-morbidity, treatment response) to determine whether subtypes are best conceptualized as differing: (1) only on their symptom profiles with no other differences between them; (2) on a quantitative (i.e. severity) dimension only; or (3) qualitatively from one another. METHOD: Data from a large epidemiological survey (

Obsessive-compulsive spectrum disorders in rheumatic fever with and without Sydenham's chorea.

BACKGROUND: Recent findings suggest that acute-phase rheumatic fever (RF) patients present with higher frequencies of obsessive-compulsive disorder (OCD) and tic disorders. Until now, there have been no such studies in RF in non-acute phases. OBJECTIVE: To verify whether patients with a history of RF with or without Sydenham's chorea (SC) present with higher rates of OCD, tic disorders, and other obsessive-compulsive (OC) spectrum disorders (such as body dysmorphic disorder [BDD]) than controls. METHOD: Between February 1999 and December 2002, 59 consecutive outpatients with non-acute RF (28

Childhood maltreatment and transdiagnostic connectivity of the default-mode network: The importance of duration of exposure.

Childhood maltreatment (CM) has been demonstrated to be associated with changes in resting-state functional connectivity of the default-mode network (DMN) across various mental disorders. Growing evidence regarding severity of CM is available but transdiagnostic research considering the role of both severity and duration of CM for DMN connectivity at rest is still scarce. We recruited a sample of participants with varying levels of CM suffering from three disorders in which a history of CM is frequently found, namely, post-traumatic stress disorder, major depressive disorder, or somatic sympto

The development of a sexual abuse severity score: characteristics of childhood sexual abuse associated with trauma symptomatology, somatization, and alcohol abuse.

Childhood sexual abuse (CSA) is common and is associated with both mental and physical health problems in adulthood. Using data from an age- and sex-stratified population survey of 600 Olmsted County, Minnesota, residents, a Sexual Abuse Severity Score was developed. The abuse characteristics of 156 CSA respondents were associated with self-reported trauma, somatization, and alcohol use. Characteristics included age of first sexual abuse, more than one perpetrator, degree of coercion, severity of abuse (i.e., attempted intercourse is more severe than fondling), and the number of occurrences. Comorbidity of body dysmorphic disorder and eating disorders: severity of psychopathology and body image disturbance.

OBJECTIVE: The current study examined comorbidity and clinical correlates of eating disorders in a large sample of individuals with body dysmorphic disorder (BDD). METHOD: Two hundred individuals with DSM-IV (4th ed. of the Diagnostic and Statistical Manual of Mental Disorders. Washington, DC: American Psychiatric Association; 1994) BDD completed reliable interviewer-administered and self-report measures, including diagnostic assessments and measures of body image, symptom severity, delusionality, psychosocial functioning, quality of life (QOL), and history of psychiatric treatment. RESULTS:

Clinical factors associated with short-term changes in outcome of patients with somatized mental disorder in primary care.

BACKGROUND: There is little research that examines demographic, clinical and treatment factors associated with changes in physical symptoms, psychiatric symptoms and functional outcome in patients with somatized depression or anxiety in primary care. METHOD: Factors associated with the outcome of psychologized or somatized depression or anxiety were derived from the literature. These factors were tested individually for their effects on changes in physical symptoms, psychiatric symptoms and functional outcome between baseline consultation with the general practitioner and 1 or 3 months later

Psychiatric illness in patients referred to a dermatology-psychiatry clinic.

There is a recognized psychiatric morbidity among those who attend dermatology clinics. We aimed to determine the pattern of psychological and social problems among patients referred to a liaison psychiatrist within a dermatology clinic. Notes from 149 patients were reviewed and more detailed assessments performed in a subgroup of 32 consecutive referrals. All but 5% merited a psychiatric diagnosis. Of these, depressive illness accounted for 44% and anxiety disorders, 35%. Less common general psychiatric disorders included social phobia, somatization disorder, alcohol dependence syndrome, obse

Obesity may be more associated with disordered eating behaviors, somatization, insecure attachment styles, and sexual dysfunction: an exploratory study.

BACKGROUND: Obesity is a challenging disease due to its multifactorial pathogenesis. However, sexual health is a poorly explored aspect in these patients, and the interaction between eating behavior, psychological factors, and sexual function remains insufficiently characterized, although it may represent a key aspect in clinical management. AIMS: To provide a characterization of psychological, eating, and sexual function characteristics of patients with obesity through an exploratory analysis. METHODS: A cross-sectional, single-center study was carried out at Umberto I Hospital of Sapienza

[Severe depression : pharmacological treatments].

Depression is a common illness with a large clinical phenotype, and clinicians have numerous guidelines to treat this disorder : many antidepressant drugs are available with different pharmacological profiles and stepped strategies are proposed to obtain a remission. It exists a relationship between baseline depression symptom severity and treatment response and patient with higher levels of severity received significantly more intervention visits, more months of antidepressant treatment and more antidepressant trials, but there is not accepted and consensual definition for severe depression.

A somatic type delusional disorder secondary to peripheral neuropathy: a case report.

Delusional disorders are one of the psychiatric disorders with unknown etiology. Various factors have been considered as risk factors (predisposing factors) for delusional disorders. Positive family history of psychiatric disorders particularly delusional disorders, is the most important risk factor. In this report, we present a 40-year old man with somatic delusions following intoxication with Phenobarbital and left brachial plexus injury (C6, C7, C8) in stupor state. Two weeks after discharge from the hospital, he developed a significant somatic delusion about his left upper extremity. To ou

[Chronic fatigue syndrome--a functional somatic syndrome].

Chronic fatigue can be categorized as a functional somatic syndrome (fss), because there are findings of typical preconditions, trigger mechanisms and maintaining conditions. With relevance for therapy it makes sense to see it as an medical-psychiatric interface-disorder Subsyndromal short episodes of chronic fatigue are many more frequent as three or six month during clearly diagnosed episodes of "neurasthenia" or "chronic fatigue syndrome". Their descriptions are very similar and obvious those means the same matter. For original aetiological assumptions it wasn't any evidence. But there are

Psychometric properties of MERIT: A concise tool for community health workers to screen priority mental disorders.

BACKGROUND: Task shifting is an accepted way to address the treatment gap for mental health disorders. Community Health Workers (CHWs) can play vital roles through screening, referral, and providing basic counseling. To address the dearth of a concise screening instrument for CHWs, the "Mental HEalth ScReenIng Tool for Community Health Workers in India [MERIT]" was designed and developed. In this paper, we have examined the psychometric properties of MERIT. METHODS: MERIT consists of nine stems (consisting of 11 questions) covering the following five domains: (a) Substance Abuse (alcohol, tob

Prevalence of psychopathology in childhood epilepsy: categorical and dimensional measures.

Few studies have utilized both categorical and dimensional measures of psychopathology in children with epilepsy. We evaluated 173 children (88 males, 85 females; mean age 11.7y [SD 1.8]; range 9-14y) who had epilepsy (generalized 36%, partial 61%) for at least 6 months. The primary caregiver completed a dimensional measure, the Child Behavior Checklist (CBCL), and a categorical measure, either the Child Symptom Inventory (CSI) or the Adolescent Symptom Inventory (ASI). Correlation coefficients were computed between the CBCL scores and CSI/ASI symptom scores. For all children, diagnostic risk

Gender similarities in somatic depression and in DSM depression secondary symptom profiles within the context of severity and bereavement.

BACKGROUND: Most population studies report higher rates of depression among women than men, and some researchers have observed gender differences in depression symptoms overall, or in sub-groupings (e.g. somatic depression). However, gender symptom differences have been inconsistent, prompting this investigation of gender differences in secondary DSM symptom profiles in the context of bereavement status, age, and depression severity. METHODS: Individuals with symptoms of core depression (flat affect or anhedonia) were selected from a large survey of adults in the Alberta, Canada workforce. An
