Characteristics of children with permanent mild hearing impairment.

The report was intended to provide up-to-date information about permanent mild hearing impairment (HI) in early childhood. Altogether 33/250 children with a permanent mild HI in the better ear (average at frequencies of 0.5, 1, 2, 4 kHz >or=25-39 dB HL) were found during the 7-year period from 1994 to 2001 in the Outpatient Clinic of the Department Phoniatrics/Pedaudiology, University of Göttingen. Twenty-two of 33 children had a mild HI in one or both ears. The loss was sensorineural in 73% of the children (88% bilateral, 12% unilateral), and conductive in 27% (67% bilaterally, 33% unilateral

Free-flap mandibular reconstruction: a 10-year follow-up study.

Free-flap reconstruction of oncologic mandibular defects has become the modern standard of care. However, no previous studies have established the long-term results of such reconstructions. The objective of this study was to review functional and aesthetic outcomes in patients a decade after free-flap mandibular reconstruction.A single surgeon's experience with free-flap reconstruction of the mandible was retrospectively reviewed. Eighty-two consecutive patients who underwent reconstruction from January of 1987 to December of 1990 were identified. Of the 34 patients still alive, 20 agreed to p

The tongue may not move, but the voice will: preoperative AI voice cloning for identity preservation in major glossectomy - a prospective feasibility study.

BACKGROUND: Major glossectomy frequently results in profound speech impairment. We investigated whether a patient's voice could be captured preoperatively and reconstructed using artificial intelligence voice cloning for potential future use as a personalized assistive communication voice during postoperative recovery. METHODS: In this prospective single-centre feasibility study, adult patients with treatment-naïve carcinoma of the tongue planned for major glossectomy underwent a single approximately 30-60 second preoperative voice recording in a sound-treated room. Voices were cloned offline

Reconstruction of lateral through and through oro-mandibular defects following oncological resections.

BACKGROUND: Resections of oromandibular squamous cell carcinoma involving lateral mandible, oral cavity, and the skin, lead to composite oromandibular defects that can be approached in several ways depending on the extension of the bone defect, of the soft tissue and cutaneous resection, the patient's general status and the prognosis. Purpose of the study is to evaluate retrospectively functional and esthetic outcome obtained with different reconstructive technique employed. METHODS: A retrospective evaluation of 42 patients has been performed. The study population consisted of 24 males (57.1

Unveiling the causal pathway of Parkinson's disease dysphonia: A voice causal generative model (VCGM) approach.

BACKGROUND: Although artificial intelligence (AI) models have demonstrated high accuracy in diagnosing Parkinson's disease (PD) from speech signals, their "black-box" nature prevents mechanistic understanding of vocal impairment, limiting clinical trust and utility. A paradigm shift from correlation-based explanation to causal reasoning is needed to unlock the potential of AI in computational medicine. METHODS: We propose the Voice Causal Generative Model (VCGM), a novel computational framework designed to infer physiologically plausible causal pathways from observational speech data. The cor

Anterior and middle cranial fossa skull base reconstruction using microvascular free tissue techniques: surgical complications and functional outcomes.

Surgical ablation for oncologic disease requiring skull base resection can result in both facial disfigurement and a complex wound defect with exposed orbital content, oral cavity, bone, and dural lining. Inadequate reconstruction can result in brain abscesses, meningitis, osteomyelitis, visual disturbances, speech impairment, and altered oral intake. This study assesses the functional outcomes of patients who undergo anterior and middle cranial fossa skull base reconstruction using microsurgical free tissue transfer techniques. Using a prospectively maintained database, a 10-year, single inst

[(Neurological CPC.55). A 60-year-old woman with progressive cerebellar ataxia, myoclonus, and dementia].

We report a 60-year-old woman with progressive ataxia, myoclonus, choreoathetosis, and dementia. She was well until 27 years of the age when she noted an onset of gait disturbance and speech disturbance. She noted abnormal involuntary movements in her four limbs at 42 years of the age. Her symptoms had progressively become worse and she fell down frequently by her 52 years of the age. In addition, her family members noted gradual decline in her intelligence. She was admitted to our hospital in February of 1993 when she was 57-year-old. On admission, she showed dementia, scanning speech, ataxic

Hearing Aid Noise Reduction Algorithms Support Neural Speech Tracking by Enhancing Spectrotemporal Consistency of Phase-Locked Brain Responses to Acoustic Edges.

Previous works that have analyzed electroencephalographic (EEG) speech envelope encoding in hearing-aid users using stimulus reconstruction (SR) approaches have shown that noise reduction (NR) algorithms can improve the neural tracking of speech in noise. Beyond enhancing speech envelope encoding, NR may also restore the perceptibility of acoustic edges that trigger auditory late responses (ALRs), likely promoting more consistent ALR elicitation and thus enhancing brain-to-speech synchronization. To investigate this hypothesis, 26 hearing-impaired listeners were fitted with bilateral hearing a

[Artificial intelligence in health education: blessing or curse?].

Introduction: ChatGPT is a large language model developed by OpenAI that has brought significant advances in natural language processing. With GPT-4.0, the model is capable of human-level conversations, which is particularly useful in education and research. In healthcare education, students can practice patient admission in virtual situations, receiving real-time feedback. It also allows the personalisation of educational materials, tailored to the individual needs of students. Objective: The aim of this study is to demonstrate the potentials of ChatGPT, i.e., the application of artificial in

Outcomes of Cleft Palate Repair in Patients with Pierre Robin Sequence: A Matched Case-Control Study.

BACKGROUND: Pierre Robin sequence results from a cascade of events that occur during embryologic development and frequently presents with cleft palate. Some studies have shown speech outcomes to be worse in patients with Pierre Robin sequence after cleft palate repair. METHODS: A cohort of Pierre Robin sequence patients who all required an airway intervention and nasogastric feeding in the neonatal period were identified and speech outcomes assessed at 5 years of age. A cleft- and sex-matched non-Pierre Robin sequence, cleft palate-only comparison group was also identified from the same insti

Cognitive functions changes among Egyptian sewage network workers.

The current work aimed at investigating the cognitive functions impairment among workers of sewer networks due to exposure to hydrogen sulfide (H(2)S) and the relation of this impairment, if any, to the level of H(2)S exposure biomarker 'urinary thiosulfate.' Besides, the validity of using Mini Mental State Examination (MMSE) as screening test for cognitive impairment among the exposed workers was tested. The work was conducted among 33 sewage network maintenance male workers and a matched unexposed control group (n = 30). The participants were subjected to clinical neurological history, estim

Unilateral expansion of a narrow mandibular dental arch combined with bimaxillary osteotomies in a patient with hypoglossia.

A 23-year-old female with hypoglossia, who had a narrow mandibular dental arch, was treated using the gradual expansion technique. Three lower incisors were missing and the right molar occlusion showed a scissor bite. Her speech was acceptable. Gradual unilateral expansion of the mandibular alveolar bone was performed. Orthodontic tooth alignment was performed prior to surgical treatment. A tooth-borne expander was devised using a hyrax-type screw to move the inclined right alveolar bone into an upright position. Alveolar bone osteotomies were performed under general anesthesia and the expande

Listeners' attitudes: speech supplementation strategies for improving effectiveness of speakers with mixed dysarthria as a result of motor neuron disease.

This study examined unfamiliar and familiar listener attitudes towards the use of combined alphabet-topic cues and a control condition (habitual speech with no cues) associated with the speech of three individuals with severe mixed dysarthria. Two listener groups (N = 36) were shown experimentally imposed visual images of the combined alphabet-topic cue strategy in conjunction with recorded auditory presentations with the habitual speech of three individuals with mixed dysarthria. Using a 7-point Likert scale, listeners were asked to rate how effective they thought the speakers communicated; h

Answering contextually demanding questions: pragmatic errors produced by children with Asperger syndrome or high-functioning autism.

UNLABELLED: This study examined irrelevant/incorrect answers produced by children with Asperger syndrome or high-functioning autism (7-9-year-olds and 10-12-year-olds) and normally developing children (7-9-year-olds). The errors produced were divided into three types: in Type 1, the child answered the original question incorrectly, in Type 2, the child gave a correct answer, but when asked a follow-up question, he/she explained the answer incorrectly, and in Type 3, the child first gave a correct answer or explanation, but continued answering, which ultimately led to an irrelevant answer. Effect of Tinnitus and Duration of Deafness on Sound Localization and Speech Recognition in Noise in Patients With Single-Sided Deafness.

Patients with single-sided deafness (SSD) often experience poor sound localization, reduced speech understanding in noise, reduced quality of life, and tinnitus. The present study aims to evaluate effects of tinnitus and duration of deafness on sound localization and speech recognition in noise by SSD subjects. Sound localization and speech recognition in noise were measured in 26 SSD and 10 normal-hearing (NH) subjects. Speech was always presented directly in front of the listener. Noise was presented to the deaf ear, in front of the listener, or to the better hearing ear. Tinnitus severity w

Universal Applicability of the Furlow Palatoplasty: Resident as Primary Surgeon in a Consecutive, Nonselective Series.

BACKGROUND: Questions persist regarding the general applicability of the Furlow palatoplasty technique, and thus, widespread adoption of its use has been uneven. This study describes a consecutive, nonselective series of primary Furlow palatoplasties. Highlighted is the unique fact that plastic surgical residents functioned as the primary surgeon for all steps of all procedures throughout the series. METHODS: A retrospective review was undertaken of all primary palatoplasties performed under the direction of the senior surgeon between December 2005 and April 2012. A stringent requirement for

What influences literacy outcome in children with speech sound disorder?

PURPOSE: In this study, the authors evaluated literacy outcome in children with histories of speech sound disorder (SSD) who were characterized along 2 dimensions: broader language function and persistence of SSD. In previous studies, authors have demonstrated that each dimension relates to literacy but have not disentangled their effects. Methods Two groups of children (86 SSD and 37 controls) were recruited at ages 5-6 and were followed longitudinally. The authors report the literacy of children with SSD at ages 7-9, compared with controls and national norms, and relative to language skill a

The 'Reading the Mind in Films' Task [child version]: complex emotion and mental state recognition in children with and without autism spectrum conditions.

Children with autism spectrum conditions (ASC) have difficulties recognizing others' emotions. Research has mostly focused on basic emotion recognition, devoid of context. This study reports the results of a new task, assessing recognition of complex emotions and mental states in social contexts. An ASC group (n = 23) was compared to a general population control group (n = 24). Children with ASC performed lower than controls on the task. Using task scores, more than 87% of the participants were allocated to their group. This new test quantifies complex emotion and mental state recognition in l
