Intramedullary cement augmentation in locking plate fixation for Neer four-part proximal humeral fractures: a technique for preventing early mechanical failure.

BACKGROUND: Locking plate fixation for Neer four-part proximal humeral fractures is frequently complicated by varus collapse and screw cut-out, leading to mechanical failure, particularly in osteoporotic bone. This study evaluates the clinical and radiographic outcomes of a modified technique involving intramedullary polymethylmethacrylate (PMMA) bone cement augmentation applied during its thread-forming phase to create an internal support structure. METHODS: In this retrospective cohort study, 66 consecutive patients with Neer four-part proximal humeral fractures treated between January 2023

Self-standing banana-peel-derived hard carbon/CNF anodes for sodium-ion batteries with voltage-window-dependent electrochemical behaviour.

Sustainable and binder-free electrode architectures are attracting increasing interest for sodium-ion batteries, particularly when waste-derived active materials can be integrated into self-standing conductive networks. Herein, we report a freestanding hard-carbon/carbon-nanofibre anode obtained by incorporating banana-peel-derived hard carbon into an electrospun polyacrylonitrile (PAN)-derived carbon nanofibre (CNF) scaffold. The banana-peel precursor was converted into activated hard carbon through dual H3PO4/ZnCl2 treatment followed by carbonization, while electrospinning and thermal conver

A full-thickness spatial transcriptomics atlas of medically refractory ulcerative colitis identifies niche signatures associated with biologic non-response.

BACKGROUND: Ulcerative colitis (UC) is a chronic mucosal immune disorder in which 30-50% of patients fail biologic therapy, yet the full-thickness immune architecture of medically refractory disease and reliable tissue-level markers of treatment response remain undefined. METHODS: Using 10x Visium HD spatial transcriptomics (16-µm bins; 615,274 bins), we generated a full-thickness spatial atlas of surgically resected colon from patients with medically refractory UC (n=3) and controls (n=3), integrating niche-stratified differential expression, spatial cell-cell communication, SCENIC regulon i

The GRACE Cycle: A General Large-Language-Model Framework for Phenotype Discovery with Unknown Cluster Number.

Phenotype discovery-the data-driven identification of clinically or biologically meaningful subgroups-is fundamental to precision medicine, but conventional clustering methods require the number of clusters K to be specified a priori and struggle with heterogeneous, multimodal, or longitudinal data. We introduce the GRACE Cycle (Generate hypothesis, Retrieve evidence, Align, Converge, Evaluate), a general large-language-model (LLM)-assisted framework for phenotype discovery in which a hypothesis, an LLM, and an evidence base are iteratively refined until they agree. The framework discovers K a

Lateral Femoral Neck and Peritrochanteric Fractures: Anatomical Classifications and Pre-Operative Reduction Techniques-A Narrative Review.

Lateral femoral neck and peritrochanteric fractures are common and clinically challenging injuries, particularly in the elderly population, with significant implications for morbidity, mortality, and functional recovery. Traditional classification systems are widely used to guide treatment, yet their reproducibility and clinical applicability remain debated. Increasing attention has been directed toward trabecular architecture and its role in fracture behavior and reduction strategies. This review aims to summarize current evidence on classification systems, trabecular-based fracture patterns,

Genetic Determinants of T-Cell Homeostasis in Critical Illness: An Exploratory Analysis of Immune Gene Variants and TREC Dynamics.

Background: Chronic critical illness (CCI) following acute brain injury involves persistent immune dysfunction, yet its genetic determinants remain unclear. We investigated whether the rate of T-cell receptor excision circle (TREC) depletion-a proposed marker of adaptive homeostatic resilience-is associated with the burden of rare damaging genetic variants. Methods: Whole-exome sequencing (WES) was performed on a cohort of 84 patients (64 with traumatic brain injury, 20 with stroke). In a longitudinal sub-cohort (n = 27), patients were stratified into quartiles (Q1-Q4) based on the slope of th

Comparative pre- and post-treatment effects of aqueous Zingiber officinale Lsfunction in wistar rats: insights into NF-κB, Nrf2, and caspase-3 pathways.

Cadmium (Cd) is a highly toxic heavy metal that accumulates in the kidney, causing oxidative stress, inflammation, apoptosis, and renal dysfunction. Ginger (Zingiber officinale) possesses antioxidant and anti-inflammatory properties, but comparative studies evaluating pre-treatment versus post-treatment regimens against cadmium nephrotoxicity remain limited. This study investigated the nephroprotective effects of aqueous Z. officinale rhizome extract in a comparative pre- and post-treatment design using histopathological, immunohistochemical, and molecular approaches. Thirty adult male Wistar

Application of dual branch and bidirectional feedback feature extraction networks for real time accurate positioning of stents.

The installation of arterial stents refers to the use of stents (also known as vascular stents) to maintain the patency of arteries during the treatment of arterial stenosis or blockage. Arterial stents are typically made of metal or polymer materials and are structured as a mesh that provides support within the blood vessel, preventing it from collapsing again after interventional treatment. The installation of arterial stents is an effective interventional therapy that can significantly improve symptoms caused by arterial stenosis or blockage and enhance the quality of life for patients. End

Enhancing removal of contaminants by organic waste-derived chars via steam activation and salt treatment.

This study aims to investigate the effects of salt treatment and steam activation on chars derived from biomass wastes (manure, food, and kenaf) and spent tires. Batch sorption experiments were used to evaluate the absorption performance of these chars for benzene, phenol, trichloroethylene (TCE), and arsenate. Steam activation of biochars at 900°C increased their specific surface area, enhancing the sorption of all contaminants. Steam-activated chars exhibited sorption capacities of 25-125 mg/g and 15-70 mg/g for benzene and phenol, respectively, one order of magnitude higher than those of pr

Fibular head osteotomy: A new technique for better exposure of postero-lateral tibial plateau fracture.

OBJECTIVE: Various osteotomy techniques have been explored for exposing the posterolateral tibial plateau in previous studies. However, these methods are often complex, cause significant damage to normal anatomical structures, compromise knee joint stability, and pose risks to knee function, thus limiting their clinical application. This study proposes a new fibular head osteotomy technique for treating posterolateral tibial plateau fractures, aiming to achieve favorable surgical outcomes. METHODS: Thirteen patients who underwent surgical treatment for posterolateral tibial plateau fractures

Autograft vs Allograft With Bone Marrow Aspirate Concentrate as an Osteotomy Gap Filler in Lateral Column Lengthening for Progressive Collapsing Flatfoot Deformity.

BACKGROUND: Progressive collapsing flatfoot deformity (PCFD) is a complex condition characterized by hindfoot valgus, midfoot varus, and forefoot abduction, leading to functional impairment and pain. Surgical correction often includes lateral column lengthening (LCL), which addresses structural deformity and restores alignment. Autografts remain the gold standard as an osteotomy gap filler for LCL despite donor site morbidity. Allografts augmented with patient's own bone marrow aspirate concentrate (BMAC) offer a potential alternative because of their osteogenic properties. This study compares

Oral absorption of semaglutide: pharmacokinetic modeling and molecular dynamics simulations.

This paper is an attempt to assess India's performance in generating the required quantity and quality of employment for its growing population since independence in 1947. But the exercise is set in a longer period that covers India's population growth since the turn of the twentieth century (1901) in relation to its ability to generate employment. The half-a-century preceding independence, despite a slow population growth, was a disaster in generating employment and any signs of structural change. Detailed analysis of the issue since independence shows that there was indeed a demographic burd

An Atypical Descending Variant of Guillain-Barré Syndrome With Bulbar Palsy, Autonomic Instability, and Delayed Colonic Pseudo-Obstruction: A Case Report.

Guillain-Barré syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy that is the most common cause of acute flaccid paralysis worldwide. The classical form manifests as an ascending, symmetrical weakness, but atypical variants, including descending presentations, have been increasingly recognized. Such variants often pose diagnostic challenges and are associated with more severe disease and prolonged recovery. We describe a 70-year-old woman with well-controlled hypertension who presented with progressive dysphagia, pooling of saliva, and upper limb weakness. Clinical examination

Acute stent placement for dynamic airway collapse after cardiac surgery in congenital heart disease with bronchial stenosis: A case report.

RATIONALE: Tracheomalacia, typically seen in relapsing polychondritis,[1] is rarely reported in association with congenital heart disease (CHD). In patients with pulmonary hypoperfusion-type CHD, surgical repair results in a rapid increase in pulmonary blood flow, predisposing them to mucus retention, airway obstruction, and respiratory distress. We describe acute airway collapse in a patient with double outlet right ventricle and congenital bronchial stenosis following cardiac repair. Surgically augmented pulmonary flow critically narrowed the stenotic airway, rapidly reversed by bronchoscopi

Artificial intelligence in computed tomography for quantifying lung changes in the era of CFTR modulators.

BACKGROUND: Chest computed tomography (CT) remains the imaging standard for demonstrating cystic fibrosis (CF) airway structural disease in vivo. However, visual scoring systems as an outcome measure are time consuming, require training and lack high reproducibility. Our objective was to validate a fully automated artificial intelligence (AI)-driven scoring system of CF lung disease severity. METHODS: Data were retrospectively collected in three CF reference centres, between 2008 and 2020, in 184 patients aged 4-54 years. An algorithm using three 2D convolutional neural networks was trained w
