Hybrid approach: vascular surgical and image-guided intervention for BroCa-induced superior vena cava syndrome (SVCS).

BACKGROUND: The aim of this study was to compare the outcome of a novel hybrid technique with the results of conventional approaches when treating tumor-induced superior vena cava syndrome (SVCS). The failure of a thrombectomy via transjugular aspiration led to an interdisciplinary approach to treat malignant SVCS. METHODS: The technique is relatively unknown but is performable if vascular surgeons and interventional radiologists work side by side in the operating room. We give an in-depth description of the hybrid technique, including surgical thrombectomy of the SVC, left brachiocephalic ve

"Resting" CBF in the epileptic baboon: correlation with ketamine dose and interictal epileptic discharges.

BACKGROUND: Photosensitive epileptic (SZ) baboons demonstrate different cerebral blood flow (CBF) activation patterns from asymptomatic controls (CTL) during intermittent light stimulation (ILS). This study compares "resting" CBF between PS and CTL animals, and CBF correlations with ketamine dose and interictal epileptic discharges (IEDs) between PS and CTL animals. METHODS: Continuous intravenous ketamine was administered to eight PS and eight CTL baboons (matched for gender and weight), and maintained at subanesthetic doses (4.8-14.6 mg/kg/hr). Three resting H(2)(15)O-PET studies were attem

Global registration of multiple bone fragments using statistical atlas models: feasibility experiments.

We propose a new technique to automatically register multiple bone fragments of a fracture using a global registration method guided by a statistical anatomical atlas model. The atlas is created by using the principal component analysis of three-dimensional meshes generated from the Computed Tomography (CT) data of five left femur bone cadavers. The fracture fragments are initially aligned to the generated atlas by using a local point descriptor which can robustly identify a set of potential corresponding points among fragments and the mean atlas model. A global registration algorithm is then

Differentiation of sCJD and vCJD forms by automated analysis of basal ganglia intensity distribution in multisequence MRI of the brain--definition and evaluation of new MRI-based ratios.

We present a method for the analysis of basal ganglia (including the thalamus) for accurate detection of human spongiform encephalopathy in multisequence magnetic resonance imaging (MRI) of the brain. One common feature of most forms of prion protein diseases is the appearance of hyperintensities in the deep grey matter area of the brain in T2-weighted magnetic resonance (MR) images. We employ T1, T2, and Flair-T2 MR sequences for the detection of intensity deviations in the internal nuclei. First, the MR data are registered to a probabilistic atlas and normalized in intensity. Then smoothing

The usefulness of repeated ictal SPET for the localization of epileptogenic zones in intractable epilepsy.

This study investigated whether repeated ictal single-photon emission tomography (SPET) is helpful in the localization of epileptogenic zones and whether it can provide information confirming that an area of increased perfusion is really the culprit epileptogenic lesion. Fifty-four repeated ictal SPET studies were performed in 24 patients with ambiguous or unexpected findings on the first ictal SPET study. These patients were enrolled from among 502 patients with intractable epilepsy in whom pre-operative localization of epileptogenic zones was attempted with a view to possible surgical resect

Minimally invasive multilevel lateral lumbar interbody fusion with posterior column osteotomy compared with pedicle subtraction osteotomy for adult spinal deformity.

BACKGROUND CONTEXT: Pedicle subtraction osteotomy (PSO) is highly effective as a sagittal correction approach in patients with adult spinal deformity, but relevant issues such as surgical complexity and long-term complications limit its applicability. Recently, minimally invasive techniques have been reported to be useful for surgical treatment of adult spinal deformity; however, few reports have directly compared these techniques with PSO. PURPOSE: The purpose of this study was to evaluate the radiological and clinical efficacies of oblique lateral interbody fusion (OLIF) with posterior colu

Image registration improves inter-reader agreement of objective response in CT assessment of pancreas adenocarcinoma.

OBJECTIVES: Pancreatic ductal adenocarcinoma (PDAC) exhibits ill-defined appearance on imaging during neoadjuvant therapy (NAT), which may limit consistency of dimensional measurements for objective response. This study evaluates inter-reader variability of current clinical measures of tumor size and interval response between baseline and restaging CT, and compares reader reliability with and without image registration assistance. METHODS: Two senior and two junior radiologists retrospectively reviewed imaging of resectable PDAC patients enrolled in an IRB-approved Phase II clinical trial. Re

Patient-specific biomechanical modeling of ventricular enlargement in hydrocephalus from longitudinal magnetic resonance imaging.

Ogden type of hyperelastic constitutive law has recently emerged in modeling ventricular enlargement in hydrocephalic brain with finite element method, but this material property for brain tissue has not been investigated in a patient-specific setting in hydrocephalus. Consequently, the accuracy of the simulated ventricular enlargement using this hyperelastic tissue property remains unknown. In this study, we evaluated this brain material model in four patients with communicating hydrocephalus under a small trans-mantle pressure difference (TPMD) between brain ventricle and subarachnoid space

Inadvertent subclavian artery catheter placement complicated by stroke: endovascular management and review.

BACKGROUND: Removal of large-bore venous catheters misplaced within the subclavian artery risks significant hemorrhage due to the noncompressible entry site. Comorbidities in these patients often make traditional surgical approaches prohibitive. CASE DESCRIPTION: We present a case of inadvertent subclavian artery placement of a central venous catheter complicated by thromboembolic stroke, illustrate treatment with a combination of percutaneous closure and temporary balloon tamponade, and review the endovascular management of this unusual problem. CONCLUSION: Endovascular treatment of inadver

Identification and characterization of a novel gene, Mcpr1, and its possible function in the proliferation of embryonic palatal mesenchymal cells.

We cloned a novel mouse cDNA, Mcpr1 (mouse cleft palate-related gene 1), between retinoic acid (RA)-treated murine embryonic palatal and control shelves by improved subtractive hybridization. Its transcript was identified by Northern blotting. The open reading frame encodes 132 amino acids and shows almost no identity to other genetic products. Mcpr1 expression could be detected extensively in adult mouse tissues and during murine embryonic development. It was identified to be significantly stimulated by RA in murine palatal shelves at embryonic day 12 and in palatal mesenchymal cells in vitro

Clinically significant incidental findings on the unenhanced CT portion of PET/CT studies: frequency in 250 patients.

UNLABELLED: PET/CT technology is in rapid evolution. It remains unclear if the unenhanced CT portion, performed for attenuation correction and lesion localization, provides additional independent diagnostic information not apparent on PET alone. The objective of the current study was to evaluate the incremental added value and frequency of potentially clinically significant incidental findings from the independent reading of the unenhanced CT portion of PET/CT studies by an expert CT radiologist. METHODS: PET/CT was performed on 250 patients (123 men and 127 women; mean age, 56.5 y) referred

Parathyroid scintigraphy with 99mTc-MIBI and 123I subtraction: a comparison with magnetic resonance imaging and ultrasonography.

The aim of this study was to evaluate the efficacy of 99mTc-MIBI and 123I subtraction scintigraphy for the detection of abnormal parathyroid glands to be referred for surgical treatment. Thirty-nine consecutive patients, including 35 primary and four secondary cases of hyperparathyroidism, were evaluated. 99mTc-MIBI/123I subtraction scintigraphy (MIBI/I) was performed on all patients, and the results were compared with delayed images of 99mTc-MIBI (D-MIBI), magnetic resonance imaging (MRI) and ultrasonography (US). The overall sensitivity of MIBI/I, MRI, US and D-MIBI was 55.9%, 43.4%, 50.8% a

Identification of gene products suppressed by human immunodeficiency virus type 1 infection or gp120 exposure of primary human astrocytes by rapid subtraction hybridization.

Neurodegeneration and human immunodeficiency virus type 1 (HIV-1)-associated dementia (HAD) are the major disease manifestations of HIV-1 colonization of the central nervous system (CNS). In the brain, HIV-1 replicates in microglial cells and infiltrating macrophages and it persists in a low-productive, noncytolytic state in astrocytes. Astrocytes play critical roles in the maintenance of the brain microenvironment, responses to injury, and in neuronal signal transmission, and disruption of these functions by HIV-1 could contribute to HAD. To better understand the potential effects of HIV-1 on

Hydrolytic stability and surface roughness of additively and subtractively manufactured CAD/CAM and indirect restorative resin materials.

BACKGROUND: Hydrolytic degradation and changes in surface roughness may compromise the long-term clinical performance of contemporary restorative resin materials, particularly when different manufacturing techniques are used. This study aimed to evaluate the hydrolytic stability and surface roughness of restorative resin materials manufactured using additive and subtractive techniques by assessing their behavior after water immersion. METHODS: Disc-shaped specimens (10 mm × 1.5 mm) were prepared for the evaluation of water sorption (Wsp) and water solubility (Wsl) using the same specimens (n =

Management of sagittal balance in adult spinal deformity with minimally invasive anterolateral lumbar interbody fusion: a preliminary radiographic study.

OBJECT: Minimally invasive (MI) fusion and instrumentation techniques are playing a new role in the treatment of adult spinal deformity. The open pedicle subtraction osteotomy (PSO) and Smith-Petersen osteotomy (SPO) are proven segmental methods for improving regional lordosis and global sagittal parameters. Recently the MI anterior column release (ACR) was introduced as a segmental method for treating sagittal imbalance. There is a paucity of data in the literature evaluating the alternatives to PSO and SPO for sagittal balance correction. Thus, the authors conducted a preliminary retrospecti
