Relative timing of neuronal activity in distinct temporal lobe areas during a recognition memory task for words.

Nine neurologically normal volunteers engaged in a recognition memory task for visually presented words while the magnetic flux normal to the scalp surface was measured with a whole-head neuromagnetometer. The relative timing of activity occurring during the task in different brain areas was determined. Occipital lobe areas became active first, followed by basal temporal areas. Temporoparietal and medial temporal areas became active next, during later portions of the recorded epoch, and appeared to activate in concert. Except for the occipital lobe, activity was predominantly in the left hemis

Early amyloid deposition in the medial temporal lobe of young Down syndrome patients: a regional quantitative analysis.

The presence of the neuropathological alterations of Alzheimer's disease (AD) in essentially all older Down syndrome (DS) patients suggests that the examination of younger DS patients may clarify the early pathological progression of AD. We examined the hippocampus and parahippocampal-inferior temporal gyri of 42 DS patients (ages 4 days to 38 years) for the deposition of amyloid beta protein (Abeta) using both a modified Bielschowsky stain and immunohistochemistry for Abeta protein. The parahippocampal and inferior temporal gyri demonstrated Abeta staining in cases as young as 8 years of age.

Dichotic listening performance in subtypes of developmental dyslexia and a left temporal lobe brain tumor contrast group.

This study examined dichotic listening performance in three subtypes of developmental dyslexia and in children with left temporal lobe brain tumors (clinical contrast group). Each child was administered a free-recall CV syllable dichotic paradigm (30 pairs). Analysis of variance and Tukey-HSD pairwise follow-up comparison indicated that the dichotic listening performance of the left temporal lobe brain tumor contrast group (strong LEA/right ear suppression) was significantly different from those of the visual-spatial/dyseidetic (strong REA/left ear depression) and mixed (moderate REA/bilateral…

Defining cognitive impairment in temporal lobe epilepsy with IC-CoDE phenotypes: A comparison of normative and ipsative approaches.

OBJECTIVE: The International Classification of Cognitive Disorders in Epilepsy (IC-CoDE) is used to classify cognitive phenotypes in individuals with epilepsy. This study compared the relative utility of ipsative versus normative approaches of IC-CoDE classification. METHODS: 1,010 patients with temporal lobe epilepsy (TLE) were classified into cognitive phenotypes (intact, single-domain, bi-domain, or generalized impairment) using both the established normative method and a novel ipsative method (cognitive impairment defined relative to premorbid IQ). Agreement between methods was quantified…

Differences in mRNA expression of neuroinflammation-related genes in the temporal lobe of patients with drug-resistant focal epilepsy.

OBJECTIVES: Approximately one-third of epilepsy patients will not achieve seizure freedom with current antiseizure medications, and the identification of novel treatment targets is needed. We characterized alterations in neuroinflammation-related mRNA levels to aid in the identification of possible molecular mediators of DRTLE and discuss the potential for immunomodulatory therapies in this condition. METHODS: Temporal lobe samples from DRTLE and postmortem controls without neurological conditions were obtained. RT-qPCR of 60 genes of interest was performed on BioMark Fluidigm custom-made gen…

Identification of proteins with altered PTM levels in patients with temporal lobe epilepsy in different regions of the human brain.

In this study we have reanalyzed the original proteomic data from studies of hippocampal [DOI: 10.1172/jci.insight.188612] and temporal cortex [DOI: 10.1016/j.expneurol.2025.115361] tissues of patients with temporal lobe epilepsy. The goal was to identify proteins whose levels of post-translational modifications (PTMs) could be altered in this disease. Three datasets (PXD064519, ZMP8KU, and PXD010154) deposited at PRIDE and Texas Data Repository were used. Phosphorylation and N-terminal acetylation were considered as target PTMs. During re-identification of peptides we considered only peptides…

Neuroprotective Effects of Early TLR4 Blockade with Compound C34 in Temporal Lobe Epilepsy: Alleviation of Neuroinflammation and Apoptosis.

BACKGROUND: Temporal lobe epilepsy (TLE) is a chronic neurological disorder characterized by hippocampal necrosis and apoptosis. Neuroinflammation plays a critical role in the pathophysiology of TLE, with toll-like receptor 4 (TLR4) serving as a key mediator. Activation of TLR4 leads to the release of pro-inflammatory cytokines, such as IL-6 and TNF-α, which contribute to neuronal injury and apoptosis. The TLR4 signaling pathway promotes neuroinflammation through nuclear factor kappa-B (NF-κB) activation, further exacerbating neuronal damage over time. Therefore, timely inhibition of TLR4 may …

The functional and structural language-associated brain network in patients with temporal lobe epilepsy and atypical language organization.

BACKGROUND: Evidence suggests that epilepsy may contribute to both functional and structural brain alterations, with atypical language organization (ALO) being more frequently observed in epilepsy patients compared to healthy controls. This study aimed to investigate structural connectivity differences between left-hemispheric language dominance (LLD) and ALO in patients with temporal lobe epilepsy (TLE) using probabilistic tractography. METHODS: Twenty-two TLE patients with ALO and 27 age- and sex-matched patients with LLD who underwent pre-surgical language assessment using 3 Tesla function…

Autism Spectrum Disorder and Temporal Lobe Epilepsy in a Physician: A Self-Reported Case of Neuropsychiatric and Functional Adaptations.

While the comorbidity of autism spectrum disorder (ASD) and epilepsy is well-documented, first-person accounts from physician-patients remain scarce. This report bridges that gap, offering both clinical and functional insights into the lived experience of this dual diagnosis. We detail the case of a 28-year-old physician, a recent medical school graduate, with confirmed ASD level 1 and structural temporal lobe epilepsy (TLE), alongside generalized anxiety disorder and major depressive disorder. The clinical picture is defined by a pronounced sensory hyper-reactivity and a lowered seizure thres…

NEDD4-2 and the CLC-2 channel regulate neuronal excitability in the pathogenesis of mesial temporal lobe epilepsy.

An increasing number of studies have focused on the role of NEDD4-2 in regulating neuronal excitability and the mechanism of epilepsy. However, the exact mechanism has not yet been elucidated. Here, we explored the roles of NEDD4-2 and the CLC-2 channel in regulating neuronal excitability and mesial temporal lobe epilepsy (MTLE) pathogenesis. First, chronic MTLE models were induced by lithium-pilocarpine in developmental rats. Coimmunoprecipitation analysis revealed that the interaction between CLC-2 and NEDD4-2. Western blot analyses indicated that NEDD4-2 expression was downregulated, while …

Impact of JQ1 treatment on seizures, hippocampal gene expression, and gliosis in a mouse model of temporal lobe epilepsy.

Epilepsy is a neurological disease characterised by recurrent seizures with complex aetiology. Temporal lobe epilepsy, the most common form in adults, can be acquired following brain insults including trauma, stroke, infection or sustained status epilepticus. The mechanisms that give rise to the formation and maintenance of hyperexcitable networks following acquired insults remain unknown, yet an extensive body of literature points towards persistent gene and epigenomic dysregulation as a potential mediator of this dysfunction. While much is known about the function of specific classes of epig…

A novel AAV9-dual microRNA-vector targeting GRIK2 in the hippocampus as a treatment for mesial temporal lobe epilepsy.

Mesial temporal lobe epilepsy (mTLE) is the most prevalent type of epilepsy in adults. First and subsequent generations of anti-epileptic therapy regimens fail to decrease seizures in a large number of patients suffering from mTLE, leaving surgical ablation of part of the hippocampus as the only therapeutic option to potentially reach seizure freedom. GluK2 has recently been identified as a promising target for the treatment of mTLE using gene therapy. Here, we engineered an adeno-associated virus serotype 9 vector expressing a cluster of two synthetic microRNAs (miRNAs), expressed from the hu…

Identification of TIMP1 as an inflammatory biomarker associated with temporal lobe epilepsy based on integrated bioinformatics and experimental analyses.

BACKGROUND: Epilepsy is the second most prevalent neurological disease. Although there are many antiseizure drugs, approximately 30% of cases are refractory to treatment. Temporal lobe epilepsy (TLE) is the most common epilepsy subtype, and previous studies have reported that hippocampal inflammation is an important mechanism associated with the occurrence and development of TLE. However, the inflammatory biomarkers associated with TLE are not well defined. METHODS: In our study, we merged human hippocampus datasets (GSE48350 and GSE63808) through batch correction and generally verified the d…

Cuproptosis-related genes signature and validation of differential expression and the potential targeting drugs in temporal lobe epilepsy.

Introduction: Temporal lobe epilepsy (TLE) is the most common subtype of epilepsy in adults and is characterized by neuronal loss, gliosis, and sprouting mossy fibers in the hippocampus. But the mechanism underlying neuronal loss has not been fully elucidated. A new programmed cell death, cuproptosis, has recently been discovered; however, its role in TLE is not clear. Methods: We first investigated the copper ion concentration in the hippocampus tissue. Then, using the Sample dataset and E-MTAB-3123 dataset, we analyzed the features of 12 cuproptosis-related genes in TLEs and controls using t…

Mass Spectrometry as a Quantitative Proteomic Analysis Tool for the Search for Temporal Lobe Epilepsy Biomarkers: A Systematic Review.

Temporal lobe epilepsy (TLE) is the most common form of epilepsy in adults. Tissue reorganization at the site of the epileptogenic focus is accompanied by changes in the expression patterns of protein molecules. The study of mRNA and its corresponding proteins is crucial for understanding the pathogenesis of the disease. Protein expression profiles do not always directly correlate with the levels of their transcripts; therefore, it is protein profiling that is no less important for understanding the molecular mechanisms and biological processes of TLE. The study and annotation of proteins that…

Alterations in static and dynamic regional homogeneity in mesial temporal lobe epilepsy with and without initial precipitating injury.

OBJECTIVES: Initial precipitating injury (IPI) such as febrile convulsion and intracranial infection will increase the susceptibility to epilepsy. It is still unknown if the functional deficits differ between mesial temporal lobe epilepsy with IPI (mTLE-IPI) and without IPI (mTLE-NO). METHODS: We recruited 25 mTLE-IPI patients, 35 mTLE-NO patients and 33 healthy controls (HC). Static regional homogeneity (sReHo) and dynamic regional homogeneity (dReHo) were then adopted to estimate the alterations of local neuronal activity. One-way analysis of variance was used to analyze the differences bet…

Influence of psychological factors on the relationship between subjective and objective memory in adults with pharmacoresistant temporal lobe epilepsy.

PURPOSE: Many adults with temporal lobe epilepsy (TLE) report subjective cognitive impairment; however, prior studies have shown a discrepancy between these subjective complaints and objective cognitive deficits on neuropsychological measures. Mood disorders/symptoms are also common in TLE and have been linked to greater subjective cognitive difficulties. To further understand these relationships, this retrospective study sought to determine if symptoms of depression and anxiety moderate or mediate the relationship between subjective cognitive impairment and objective cognitive performance in …

The multifactorial etiology of cognitive deficits in epilepsy and the neuropathology of mesial temporal lobe epilepsy beyond hyperphosphorylated tau.

In response to the recent review by Zawar and Kapur on the overlap between mesial temporal lobe epilepsy (MTLE) and Alzheimer's disease (AD), we (1) underscore that the bidirectionality between epilepsy and dementia is of high interest, also from the epileptological perspective; (2) outline the multifactorial etiology of cognitive deficits in epilepsy; (3) emphasize that the most prevalent neuropathological findings in MTLE comprise hippocampal sclerosis, dysplastic lesions, and neurodevelopmental neoplasm; and (4) state that anti-seizure medication can also have adverse effects on cognition. …
