Patient-reported swelling in arthralgia patients at risk for rheumatoid arthritis: is it of value?

OBJECTIVE: Patients with clinically suspect arthralgia (CSA) are at risk for developing rheumatoid arthritis (RA). These patients often repo … ling was associated with subclinical joint inflammation. Using MRI, it associated specifically with tenosynovitis (odds ratio [OR] 3.7 [95% CI: 2.0, 6.9]) and when using US with synovitis (OR 2.3 [95% CI: 1.04, 5.3]). CSA patients with self-reported swelling at baseline developed arthritis more often, with hazard ratios of 3.7 (95% CI: 2.0, 6.9) and 3.4 (95% CI …

Psoas impingement causing labrum tear: a series from three tertiary hip arthroscopy centers.

BACKGROUND: The term hip impingement is usually associated with psoas impingement after arthroplasty or femoroacetabular impingement (FAI). A recently, less commonly described mechanism of impingement and labrum pathology is the psoas tendon applying pressure to the acetabular labrum more medial to the typical FAI labrum lesion. Much is still unkown about the anatomy, pathology, and treatment of this entity. PURPOSE: This paper describes the successful arthroscopic treatment of a series of patients with a recently recognized cause of hip pain in the young athletic population without significa

Prevalence of and factors associated with posterior tibial tendon pathology on sonographic assessment.

OBJECTIVE: To assess the frequency of and factors associated with supramalleolar posterior tibial tendon (PTT) pathology that often may be m … tients with supramalleolar PTT pathology had more severe grades of tear and increased prevalence of tenosynovitis and were more often women. It is essential to recognize supramalleolar PTT pathology so that consequences of nontreatment such as medial arch collapse can be prevented.

Clinical and genetic aspects of psoriatic arthritis "sine psoriasis".

OBJECTIVE: To characterize the clinical pattern of psoriatic arthritis (PsA) sine psoriasis. METHODS: Fifty-seven patients (31 men, 26 women, mean age 46.32 +/- 14.12 yrs) with undifferentiated spondyloarthropathy (SpA) were studied. Two subsets were defined: (1) 21 patients with familial psoriasis (12 men, 9 women, mean age 49.29 +/- 14.17 yrs); (2) 36 patients without familial psoriasis (19 men, 17 women, mean age 44.58 +/- 14.00 yrs). The prevalence of the following clinical variables was evaluated: low back pain, enthesopathy, dactylitis, distal interphalangeal (DIP) arthritis, spinal inv

The use of magnetic resonance imaging in posterior tibial tendon dysfunction.

The role of magnetic resonance imaging in the evaluation of the patient with posterior tibial tendon dysfunction is discussed. Considerations for the proper positioning of the patient and optimal technique to obtain appropriate images of the posterior tibial tendon and associated joint abnormalities are highlighted. Cases are presented to show the effectiveness of magnetic resonance imaging in different clinical situations. The treatment algorithm for posterior tibial tendon dysfunction should include magnetic resonance imaging as a diagnostic tool when appropriate.

Multicenter study of piroxicam in the treatment of acute musculoskeletal diseases involving 3011 patients.

An open non-comparative study was conducted to assess the efficacy and tolerability of piroxicam in patients with acute musculoskeletal disease. A total of 3011 patients of both sexes, aged between 18 and 70, were treated with piroxicam: 4 capsules (40 mg) after dinner, on the first two days and two capsules (20 mg) on the following days. Patients were evaluated for a maximum period of 14 days. Assessment of efficacy was made including local pain and limitation of mobility. The patients' self-assessment of pain was also analyzed. A clear improvement in each of the parameters analyzed was found

Complications following internal fixation of unstable distal radius fracture with a palmar locking-plate.

OBJECTIVES: The increasing number of fixed-angle plate systems used to treat distal radius fractures carries with it the problem of determin … llicis longus tendon, 2 ruptures of the extensor pollicis longus tendon, 4 cases of extensor tendon tenosynovitis, and 9 cases of flexor tendon tenosynovitis. Carpal tunnel syndrome was observed in 3 patients, and complex regional pain syndrome occurred in 5 patients. In 2 cases, loosening of a single screw was seen. Delayed fracture union occurred in 3 patient …

Comparison of nonsurgical treatment measures for de Quervain's disease of pregnancy and lactation.

de Quervain's disease of pregnancy and lactation is usually self-limited and responds well to nonsurgical treatment. We conducted a randomized prospective study on 19 wrists of 18 patients with de Quervain's disease who were either pregnant or breast-feeding. One group had a cortisone injection into the tendon sheath and the other group used thumb spica splints. All 9 patients with injections had complete pain relief with only one late recurrence. None of the patients with splints had complete pain relief; however, at the end of the lactation period, 8 had spontaneous resolution of symptoms an

Modified Guide-Assisted Percutaneous Trigger Finger Release Using a Custom-Made Kirschner Wire (K-wire): Long-Term Outcomes of a Retrospective Case Series From Gaza City.

Background Trigger finger (stenosing flexor tenosynovitis) is a common cause of hand pain and disability. Percutaneous release is a minimally invasive alternative to open surgery, but few guide-assisted, low-cost techniques have been described for resource-limited settings. This study evaluated the long-term outcomes, recurrence rate, safety, and patient satisfaction of a modified percutaneous trigger finger release using a custom-made Kirschner wire (K-wire) guide. Methods In this retrospective observational case series, 13 patients (26 trigger digits) treated between May 2016 and May 2026 at…

Modified Dixon MRI to detect subclinical inflammation in clinically suspect arthralgia as a risk factor for rheumatoid arthritis development: should we image one or two hands?

OBJECTIVES: MRI-detected subclinical inflammation in clinically suspect arthralgia (CSA) predicts progression to inflammatory arthritis (IA) … ateral wrist and metacarpophalangeal 2-5 joints were scored for subclinical inflammation (synovitis/tenosynovitis/osteitis) using RA MRI scoring. The hand with the most self-reported painful joints was used for unilateral analysis. Patients were followed for ≥6 months, with IA and RA development assessed at 6 months. We compared prognostic performance of bilate …

Age-Stratified Sequelae After Nonoperative Distal Radius Fractures: A Propensity-Matched Cohort Study.

BACKGROUND: Distal radius fractures (DRFs) in older adults are frequently managed nonoperatively. However, treating "older adults" as a sing … t demonstrated higher hazards of corrective repair, complex regional pain syndrome, and De Quervain tenosynovitis. In landmark analysis, the younger cohort remained at increased hazard of late-onset carpal tunnel syndrome and reconstructive procedures beyond 90 days. CONCLUSION: Despite similarly low rates of fracture healing complications across age strata, p …

Concomitant Biceps Tenodesis Does Not Compromise Arthroscopic Rotator Cuff Repair Outcomes.

PURPOSE: To compare outcomes of patients who underwent rotator cuff repair (RCR) with concomitant biceps tenodesis with those who underwent an isolated RCR. METHODS: Exclusion criteria included previous ipsilateral shoulder surgery, irreparable rotator cuff tears, rotator cuff arthropathy, calcific tendinitis, adhesive capsulitis requiring a capsular release, or advanced osteoarthritis of the glenohumeral joint. Patients were indicated for biceps tenodesis if they had any degree of tendon tearing, moderate-to-severe tenosynovitis, instability, or a significant degenerative SLAP tear. Primary …
