Salmonella typhi osteitis can mimic tuberculosis. A report of three cases.

We report three cases of Salmonella typhi osteitis. One patient was an immunocompetent woman with a single focus of osteitis, another had heterozygous sickle cell disease with multifocal osteitis, and the remaining patient had acute discitis. Tuberculosis was considered in all three patients, based on origin from an endemic area (sub-Saharan Africa), a chronic course in the first two patients, and granulomas in a biopsy specimen from one patient.

Psoas abscess diagnosed at a Northern university hospital.

Abscess of the psoas muscle is an infrequent diagnosis at hospitals in Northern countries. We report on 16 patients who had this diagnosis during the period 1991-2001. Eight patients were immigrants who had previously been healthy and most of them had experienced symptoms for approximately 1 y. MRI or CT scans revealed spondylodiscitis in 6 of these patients and Mycobacterium tuberculosis was identified as the causative agent. With the exception of 1 patient who was exclusively treated with antituberculous agents, all 8 immigrant patients were successfully treated with antituberculous agents i

Recurrent breast abscess caused by Lawsonella clevelandensis: A case report and literature review.

RATIONALE: Lawsonella clevelandensis is a gram-positive bacterium, partially acid-fast, strictly anaerobic, nonspore-forming, and catalase-positive. This microorganism was once overlooked in clinical microbiology due to its stringent growth requirements in laboratory cultures, but it has recently attracted recognition as a potential pathogen. Available reports implicate Lawsonella clevelandensis infection with abscess formation, including breast, spinal, abdominal, and deep soft tissue abscesses. Here, we present a case of recurrent breast abscess caused by Lawsonella clevelandensis infection.

Extensively Drug-resistant Tuberculosis of the Lumbar Spine in a Six-year-old Child: A Case Report.

INTRODUCTION: The emergence of extensively drug-resistant tuberculosis (XDR-TB) is a challenging paradigm shift faced by the TB control programs worldwide today. The treatment is further compounded with unique management difficulties faced in pediatric patients. Treatment of XDR-TB requires prolonged chemotherapy with second-line drugs which offer lesser potency and increased risk of drug-related side effects. We present a case of spinal XDR-TB in a child, managed with extended second-line antitubercular chemotherapy (ATT). CASE REPORT: A 6-year-old, Caucasian male child complained of persist

Posterior approach vertebrectomy in the thoracolumbar spine with expandable cage reconstruction: indications and techniques based on eight cases.

We present a series of eight patients undergoing circumferential neural decompression and stabilization of the thoracolumbar spine via a single midline posterior approach, with complete vertebrectomy for a variety of indications. Four had primary tumours, two secondary tumours, one patient had tuberculosis and one had delayed deformity due to trauma. This single posterior approach is made possible via the use of an expandable cage that may be expanded in situ to offer anterior and middle column reconstruction. There were no major neurological complications and all patients with deficits improv

Concomitant spine infection with mycobacterium tuberculosis and pyogenic bacteria: case report.

STUDY DESIGN: A case report of an extremely rare condition describing lumbar spine tuberculosis associated with concurrent pyogenic infection is presented. OBJECTIVE: To establish that isolation of pyogenic bacteria from an infected spine does not exclude the possibility of spine tuberculosis. SUMMARY OF BACKGROUND DATA: During a MEDLINE data search from January 1960 through October 2001, no cases of combined spine tuberculosis and pyogenic infection were found. METHODS: A 52-year-old man reported left-side gluteal swelling, backache, and fever of 20 days duration. A similar swelling in the

Integrating metagenomic next-generation sequencing into a multimodal diagnostic framework for spinal infection: enhancing etiological identification and clinical prediction.

BACKGROUND: Spinal infection (SI) remains diagnostically challenging because of heterogeneous etiologies, nonspecific clinical manifestations, and the limited sensitivity of conventional microbiological approaches, particularly following empirical antimicrobial exposure. Although metagenomic next-generation sequencing (mNGS) enables unbiased pathogen detection, its incremental clinical value beyond pathogen identification and its role within integrated diagnostic strategies remain incompletely established. METHODS: We retrospectively analyzed 208 consecutive patients with suspected SI between

Chronic-relapsing experimental autoimmune encephalomyelitis. Myelin basic protein induces suppression of blastogenesis during remissions but not during exacerbations.

Previous work from this laboratory has shown that resistance to acute experimental autoimmune encephalomyelitis (EAE) correlates with disease-specific, antigen-induced suppression of blastogenesis in vitro. We now report that this suppression in vitro also occurs during remissions in animals with chronic-relapsing EAE. Hartley strain guinea pigs were injected with an homogenate of guinea pig spinal cord in complete Freund's adjuvant (CFA) to induce EAE or, for control purposes, with CFA alone. Animals injected with spinal cord homogenate developed EAE. Susceptible animals displayed up to 3 exa

Surgeon-directed transcranial motor evoked potential spinal cord monitoring in spinal deformity surgery.

AIMS: Spinal deformity surgery carries the risk of neurological injury. Neurophysiological monitoring allows early identification of intraoperative cord injury which enables early intervention resulting in a better prognosis. Although multimodal monitoring is the ideal, resource constraints make surgeon-directed intraoperative transcranial motor evoked potential (TcMEP) monitoring a useful compromise. Our experience using surgeon-directed TcMEP is presented in terms of viability, safety, and efficacy. METHODS: We carried out a retrospective review of a single surgeon's prospectively maintaine

[Development and application of artificial vertebral body].

Artificial vertebral body has achieved good results in treating spinal tumors, tuberculosis, fracture and other diseases. Currently, artificial vertebral body with variety of kinds and pros and cons, is generally divided into two types: fusion type and movable type. The former according to whether the height could be adjusted and strength of self-stability is divided into three types: support-fixed type, adjust-fixed type and self-fixed type. Whether the height of self-fixed type could be adjusted is dependent on structure of collar thread rotation. The latter is due to mobile device of ball-a

Pott's Paraplegia--67 cases.

Sixty-seven patients were treated for Pott's paraplegia: 58 were adults and 9 were children. Sixty-four patients had active disease, and 3 had healed disease. All patients had triple chemotherapy with or without decompression surgery. Thirteen patients, including 9 children, were treated conservatively, whereas 54 patients who met the selection criteria for surgery were treated surgically. Fifty-two patients had anterior radical decompression surgery, and for 14 of them, anterior surgery was preceded by posterior instrumental stabilization surgery. Two patients with healed disease had posterio

[Surgical interventions on the vertebral bodies of the thoracic spine (author's transl)].

27 operations on the vertebral bodies of the thoracic spine are reported. Indications for operation included markedly progressive infantile and juvenile scoliosis, correction of severe kyphoses in congenital malformation, fractures, Scheuermann's disease and post-laminectomy as well as tuberculous spondylitis and tumors. The two uppermost thoracic vertebrae can be reached by the method of Southwick and Robinson, the two lowest vertebrae from an extraperitoreal-subdiaphragm approach. Thoracotomy was chosen for the remaining thoracic vertebrae. If the staff and material are available, operations
