[Status of the microcirculatory bed of the bulbar conjunctiva in various forms of vestibular dysfunction].

Using biomicroscopy of the eye conjunctiva, blood microcirculation was investigated in 200 patients with vestibular dysfunction (cervical osteochondrosis, Ménière's disease, neurocirculatory dystonia). All the patients showed microcirculation changes such as arteriolar spasms, reduced number of functioning capillaries, congestion and convolution of venules of medium and small caliber, and red blood cell aggregation. Capillary blood flow was intermittent: dash-dotted or bead-like. The arteriovenular coefficient decreased. Blood microcirculation changes were detected on the normal and affected s

Does endolymphatic hydrops affect cochlear and vestibular nerve volumes? An MRI-based morphometric study.

OBJECTIVE: To investigate the relationship between the severity of endolymphatic hydrops (EH) and volumetric changes in cochlear and vestibular nerves using high-resolution MRI, and to assess associations with clinical audiovestibular symptoms. METHODS: A cohort of 108 patients with clinically and radiologically confirmed EH underwent 3 T MRI to obtain volumetric measurements of the cochlear and vestibular nerves. Nerve volumes were compared across EH grades using non-parametric statistics. Cochlear nerve volumes in affected and unaffected ears were compared in a subgroup with unilateral hear

Clinical characteristics associated with isolated unilateral utricular dysfunction.

PURPOSE: The ocular vestibular evoked myogenic potential (oVEMP) represents the sound-induced activation of extraocular muscles and is believed to originate from the utricle and superior vestibular nerve. Isolated unilateral oVEMP abnormalities with otherwise normal balance function test (BFT) results have not yet been characterized in a large patient series, and their clinical significance remains unclear. MATERIALS AND METHODS: Retrospective review of adult patients with vestibular complaints at a tertiary academic neurotologic referral center was performed. Patients with isolated unilatera

Smartphone-Supported Vestibular Rehabilitation in Individuals With Vestibular Dysfunction: Pilot Randomized Crossover Trial Assessing Functional Clinical Outcomes and Anxiety.

BACKGROUND: Vestibular disorders impair balance, increase fall risk, and reduce quality of life due to dizziness and vertigo. They are frequently accompanied by heightened anxiety, which may further limit daily functioning and contribute to avoidance behaviors. Although vestibular rehabilitation has been extensively studied and shown to be effective in managing vestibular disorders, adherence to home-based exercises remains low for many dizzy patients. This is often attributed to uncertainty about correct performance, lack of feedback, or difficulty maintaining a structured routine. To help ad

Pre-treatment audiological and vestibular assessment in adults starting platinum-based chemotherapy.

PURPOSE: To evaluate the feasibility of incorporating both standard and novel audiological and vestibular assessments into routine oncology care. A secondary objective was to characterize audiovestibular status prior to platinum-based chemotherapy and identify potential risk factors for cochleotoxicity and vestibulotoxicity. METHOD: This single-center, cross-sectional prospective study enrolled 110 adults scheduled for their first platinum-based chemotherapy cycle. All assessments were performed at one time point prior to treatment initiation. Pre-treatment assessments included tympanometry,

Vestibular evoked myogenic potentials and health-related quality of life in patients with vestibular neuritis.

OBJECTIVE: To evaluate the usefulness of vestibular evoked myogenic potentials (VEMPs) in subjects with vestibular neuritis (VN) and to determine the impact of the disease in health-related quality of life (HRQoL). STUDY DESIGN: Case series. SETTING: Tertiary referral center. PATIENTS: Fifty patients with VN (episode of sudden onset of prolonged vertigo [>24 h] associated with peripheral vestibular hypofunction, imbalance in absence of hearing loss, or other neurologic symptoms). INTERVENTION: VEMPs were measured in 41 patients by using an air-conducted 500 Hz tone burst. HRQoL was evaluat

Acute blockade of inner ear marginal and dark cell K+ secretion: Effects on gravity receptor function.

Specific pharmacological blockade of KCNQ (Kv7) channels with XE991 rapidly (within 20 min) and profoundly alters inner ear gravity receptor responses to head motion (Lee et al., 2017). We hypothesized that these effects were attributable to the suppression of K+ secretion following blockade of KCNQ1-KCNE1 channels in vestibular dark cells and marginal cells. To test this hypothesis, K+ secretion was independently inhibited by blocking the Na+-K+-2Cl- cotransporter (NKCC1, Slc12a2) rather than KCNQ1-KCNE1 channels. Acute blockade of NKCC1 with ethacrynic acid (40 mg/kg) eliminated auditory res

[Visual prevention from motion sickness in cars].

The differential effects of vision on motion sickness in cars were tested under real road conditions using linear accelerations, in order to confirm earlier laboratory results on visual modulation of vestibular nausea induced by angular accelerations of the body. The 18 voluntary subjects were exposed to repetitive braking maneuvers (linear accelerations: 0.1-1.2 g) on a highway. The simultaneous visual stimulus conditions for the 3 separate days were: I) eyes open, visual control of car motion; II) eyes closed; III) eyes open, artificial stationary visual field (reading). The severity of moti

Altered Insular and Occipital Responses to Simulated Vertical Self-Motion in Patients with Persistent Postural-Perceptual Dizziness.

BACKGROUND: Persistent postural-perceptual dizziness (PPPD) is a common functional vestibular disorder characterized by persistent symptoms of non-vertiginous dizziness and unsteadiness that are exacerbated by upright posture, self-motion, and exposure to complex or moving visual stimuli. Recent physiologic and neuroimaging data suggest that greater reliance on visual cues for postural control (as opposed to vestibular cues-a phenomenon termed visual dependence) and dysfunction in central visuo-vestibular networks may be important pathophysiologic mechanisms underlying PPPD. Dysfunctions are t

Torsional optokinetic nystagmus after unilateral vestibular loss: asymmetry and compensation.

The aim of this study was to analyse torsional optokinetic nystagmus (tOKN) in 17 patients with Menière's disease before and after (1 week, 1 month and 3 months) a curative unilateral vestibular neurotomy (UVN). The tOKN was investigated during optokinetic stimulations around the line of sight directed towards either the lesioned or the healthy side, at various constant angular velocities. Dynamic properties of tOKN and static ocular cyclotorsion were analysed using videonystagmography. Patients' performances were compared with those of 10 healthy subjects. The results indicate that, in the ac

Anti-saccade as a tool to evaluate cognitive impairment in vestibular migraine.

BACKGROUND: Vestibular migraine (VM), an intricate subtype of migraine, amalgamates the dual attributes of migraine and vestibular disorders. In clinical settings, individuals with VM frequently articulate concerns regarding the manifestation of subjective cognitive impairment. This cognitive dysfunction is intricately linked with diminished mobility, heightened susceptibility to falls, and increased absenteeism in afflicted patients. Consequently, comprehending the features of cognitive impairment in VM patients holds potential clinical significance. The pursuit of rapid and objective methods

Auditory brainstem function in women with vestibular migraine: a controlled study.

BACKGROUND: Vestibular migraine (VM) has been recognized as a diagnostic entity over the past three decades. It affects up to 1% of the general population and 7% of patients seen in dizziness clinics. It is still underdiagnosed; consequently, it is important to conduct clinical studies that address diagnostic indicators of VM. The aim of this study was to assess auditory brainstem function in women with vestibular migraine using electrophysiological testing, contralateral acoustic reflex and loudness discomfort level. METHODS: The study group consisted of 29 women with vestibular migraine in
