A review of 100 aphakic extended-wear soft contact lenses is presented for the period July 1980 to August 1981. Four previously successfully fitted patients with either American Optical Company's Sofcon or Cooper Laboratories' Permalens for extended wear developed corneal ulcers either directly under the lenses or shortly after removal. Three of the female patients were well controlled diabetics without retinopathy, one of whom sustained severe visual loss and neovascular glaucoma after a pseudomonas ulcer. Dark adaptation before and after exposure to bright light was investigated in a total of 129 aphakic and phakic eyes. Aphakic patients whose vision had been corrected with spectacles, contact lenses and intraocular implants were compared with phakic controls. Eyes with implants compared favorably with other optical correction systems; the rapidity of dark adaptation, however, was delayed in pseudophakia after exposure to bright light.

The effect of topical administration of Buphenine was studied in a group of aphakic patients (53 eyes). The action of the drug was paradoxical: it induced an increase, often remarkable, in intraocular pressure in more than 70% of the eyes thus treated. The cause of the elevated IOP is discussed.

Among 173 patients who had rhegmatogenous retinal detachment with spared maculas, high percentage achieved anatomic and functional success. We reattached 171 (99%) of the retinas; 150 (76%) of the eyes had final visual acuities of 6/15 (20/50) or better. Eighteen eyes (10%) had final postoperative visual acuities reduced more than two lines from the preoperative level. The intraocular lens restores general vision and some degree of independence and mobility to patients with dense cataracts and macular degeneration. The patient, however, must be repeatedly warned that fine central vision, particularly reading, will not be possible after the surgery. An aphakic spectacle leaves such patients a narrow band of vision when superimposed over the macular lesion, and contact lenses are too small for the patient to manage insertion without help.

Of 857 cultures of material collected from the eyelids, conjunctivae, and contact lenses of 51 extended-wear soft-contact-lens wearers, 44 (5%) grew potential pathogens. The aphakic patients wearing contact lenses had fewer recoverable pathogens (24 of 295 or 8%) than a group of preoperative cataract patients (63 of 295 or 21%). On five occasions, positive cultures accompanied clinical symptoms, and on three occasions, repeated cultures grew the same potential pathogen.

PURPOSE: To report the clinical outcomes of a modified suture technique used for transscleral intraocular lens (IOL) fixation.
METHODS: This study enrolled patients with aphakia, dislocated IOL, or crystalline lens luxation. All eyes underwent transscleral IOL fixation using the L-suture technique. The authors report a case of tube obstruction of a nonvalved glaucoma drainage device (Aurolab aqueous drainage implant; AADI) with a dislocated Soemmering's ring (SR) leading to a postoperative intraocular pressure (IOP) spike after an initial IOP reduction.
