AIMS: Permanent junctional re-entry tachycardia is a relatively uncommon form of re-entry tachycardia with antegrade conduction occurring through the atrioventricular node and retrograde conduction over an accessory pathway usually located in the postero-septal region. It was the aim of the study to investigate the course of permanent junctional re-entry tachycardia with particular regard to the effectiveness of pharmacological treatment and ablation procedures; evaluation was performed with respect to the patient's symptoms, tachycardia rate, frequency of the tachycardia and left ventricular …

Acupuncture physicians have studied the application of reflexotherapy to cardiology. However, no one has investigated the connection of ancient Chinese diagnostic methods with modern tools. A total of 102 patients (54 men and 48 women) with heart pathology, namely, sick-sinus syndrome, Wolff-Parkinson-White syndrome, and atrioventricular blockade, were studied using the usual instrumental methods (transesophageal electrophysiological study of the heart, echocardiography), after which they underwent Akabane thermopuncture testing as in traditional Chinese medicine. The results of cardio examina

Sick sinus syndrome with symptomatic bradycardia is an indication for a permanent pacemaker. Either a single (AAIR) or dual-chamber (DDDR) pacemaker can be implanted in these patients with normal atrioventricular nodal function. This report presents a 92-year-old male with right ventricular apical pacing related recurrent acute pulmonary edema and mechanical asynchrony demonstrated by three-dimensional echocardiogram. Although three-dimensional echocardiography has been available for many years, it has seldom been applied to evaluate pacing-related intraventricular asynchrony. The systolic asy

OBJECTIVES: This study was designed to evaluate the effect of septal reduction therapies on the conduction system for patients with hypertrophic cardiomyopathy (HCM).
BACKGROUND: Heart block is a potential complication of both catheter-based and surgical procedures to relieve left ventricular outflow tract obstruction in HCM, but it is important to understand the different effects of these treatments on the conduction system.
METHODS: The electrocardiograms and postoperative course of patients who underwent percutaneous alcohol septal ablation or surgical myectomy at Mayo Clinic between 1999

Preexcitation and aberrant conduction both cause a broad QRS complex. An unusual case of an atriofascicular accessory pathway effectively functioning as an ectopic right bundle branch and responsible for both physiologically normal ventricular activation and pathologic preexcited tachycardias as part of a split AV node-bundle branch system is presented.

A 17-year-old girl with a corrected complex congenital heart disease and recurrent episodes of supraventricular tachycardia was referred for catheter ablation. Electrophysiologic studies revealed the presence of an accessory pathway (AP) with bidirectional conduction and decremental properties. We demonstrated a course parallel to the node-His AV conduction system. Transient abolition of the bidirectional conduction through the AP was obtained by radiofrequency application to the ventricular insertion located in the distal right bundle branch and to the atrial insertion, located in the mid-ant

Although effective, there is a disturbing incidence of sudden death after AV node ablation. The mechanism may be related to proarrhythmia associated with prolongation in ventricular repolarization from the sudden decrease in heart rate. To examine this issue, we studied 15 patients undergoing complete radiofrequency ablation of the AV node for rapid atrial arrhythmias. Twelve-lead ECGs of paced rhythms at rates of 60, 80, 100, and 120 beats/min were recorded at time points of 30 minutes, 24 hours, 1 week, and 1 month after ablation. The QT interval was measured in the limb and precordial leads

The effects of adenosine on action potentials, rate-dependent activation failure (the cellular basis for second-degree atrioventricular [AV] block), and the recovery of excitability in rabbit isolated single AV nodal cells were studied using the whole-cell patch-clamp technique. Adenosine (1 micromol/L) shortened the duration, depressed the amplitude, and reduced the rate of rise of the AV nodal cell action potential. Adenosine (10 micromol/L) caused a significant hyperpolarization (7 +/- 1 mV) of AV nodal cells. Adenosine increased the occurrence and the rate dependence of activation failure

We report the case of a 61-year-old man with type B Wolff-Parkinson-White (WPW) syndrome associated with a right atrioventricular (AV) accessory pathway and concomitant mitral stenosis, who underwent successful operative treatment by simultaneous surgery. His preoperative course had been characterized by cardiac failure and repeated episodes of atrial tachyarrhythmia, in the form of fibrillation and flutter, which were difficult to control by conventional medication. Preoperative electrocardiograms (ECGs) had suggested that the accessory pathway was located in the right posterior to posterosep

OBJECTIVES: The purpose of this study was to utilize vector mapping to investigate atrial and accessory pathway activation direction during orthodromic supraventricular tachycardia.
BACKGROUND: Although advances have been made in the electrophysiologic evaluation and management of accessory pathways, our understanding of accessory pathway anatomy and physiology remains incomplete. Vector mapping has been validated as a method of studying local myocardial activation.
METHODS: In 28 patients with a left-sided or posteroseptal accessory atrioventricular (AV) pathway referred for ablation, atria

Catheter ablation of accessory atrioventricular pathways using radiofrequency current was attempted in 61 children and young adolescents less than 18 years of age who were referred for treatment of symptomatic supraventricular tachycardia. Thirty-three children had the Wolff-Parkinson-White syndrome and 30 tachyarrhythmias related to an accessory pathway conducting only in retrograde fashion. Ablation of left sided accessory pathways was usually attempted utilizing an arterial approach to the annulus of the mitral valve, only in one case we used the transseptal approach, while the venous route

Temporary dual chamber atrioventricular (AV) pacing is often used to increase cardiac output (CO) after cardiac surgery. The AV interval was varied to investigate the effect on CO in 13 patients. CO was computed from the Fick principle using mixed venous oxygen saturation (SvO2), arterial oxygen saturation (SaO2), hemoglobin, and oxygen consumption (VO2). AV intervals were varied randomly from 0.025 to 0.300 seconds in 0.025-second increments. The effect on CO from increasing the AV interval was dependent on the control CO. In all patients, as the AV interval was increased from 0.025 to 0.100

Catheter ablation of an accessory atrioventricular connection using 500-kHz radiofrequency current was attempted in 10 children and adolescents aged between 6 and 15 years (mean 10.5 years). Six children had the Wolff-Parkinson-White syndrome and four had tachyarrhythmias related to a retrograde-only conducting ("concealed") accessory connection. No child had associated structural heart disease. Symptoms ranged from disabling palpitations to episodes of syncope (three patients) and cardiac arrest (one patient). Ablation was attempted from the left ventricle in all children; in one child, ablat

Results of catheter ablation of the atrioventricular (AV) junction in 41 patients were compared with results of cryosurgical ablation in 42 patients. Mean follow-up was 29 months among patients who underwent catheter ablation and 53 months among those who underwent cryosurgical ablation. In both groups complete heart block was produced in most patients (88% in the catheter ablation group, 86% in the cryosurgery group), and similar proportions of patients continued to receive antiarrhythmic drugs (27% in the catheter ablation group, 36% in the cryosurgery group). However, the short-term morbidi

Transvenous endocardial destruction of the AV junction, using an original bipolar technique, was carried out in 32 patients with supraventricular tachyarrhythmias, resistant to medication. The procedure was effective in 29 (91%) of 32 patients. Stable and complete AV block was achieved in 23 of 30 patients, with subsequent implantation of a pacemaker, while in 4 patients, clinical improvement was achieved with intact AV conductivity. Two patients with re-entry tachycardia of the AV junction were subjected to low-energy contusion of the AV junction, and the results were good. Major hemodynamic

Junctional ectopic tachycardia has been described in infants but not in adults. Five adults with rapid symptomatic paroxysmal junctional tachycardia, distinct from the more common slower nonparoxysmal junctional tachycardia, were recently evaluated. The tachycardia was irregular (rate 120 to 250) and accompanied by periods of atrioventricular dissociation and narrow QRS complexes. A junctional origin was documented during electrophysiologic study in four of the five patients. Analysis of Holter recordings; the response to exercise, isoproterenol, and propranolol; and the effects of atrial and

Fifteen patients with an isolated ventricular chamber were studied using angiography. These patients constitute 7% of the 218 patients with single ventricle who were examined at our institution over a 10-year period. The atrial situs was solitus in 6 and isomeric in 9. The ventricular chamber showed 3 varieties based on trabecular pattern: right type, mixed and indeterminate. The atrioventricular (AV) connection was through 2 valves in 4 hearts, through a common AV valve in 8 and through a single valve in 3. The morphologic features of the ventricular chamber and the AV valves are considered t

Pilots and new applicants with asymptomatic 'safe' WPW syndrome should be allowed to fly but not as pilot in sole command. Professional pilots with tachyarrhythmia related to a pre-excitation syndrome should usually be disqualified . Personnel who have undergone successful surgical treatment by ablation of an accessory pathway could be considered for recertification to fly but drug therapy is at present unacceptable.

A technique that makes use of real-time-directed M-mode echocardiography is described in which the fetal heart is evaluated for suspected cardiac arrhythmias. Simultaneous M-mode recording of aortic, mitral, or tricuspid valvular leaflet excursion and/or atrioventricular wall motion defines timing relationships between atrial and ventricular systole. With the use of the techniques described in this report, normal as well as abnormal atrioventricular contractions can be quantitated and cardiac arrhythmias accurately defined.
