Coronary artery disease in adult congenital heart disease: outcome after coronary artery bypass grafting.

BACKGROUND: Atherosclerotic coronary artery disease may be seen during repair of adult congenital heart disease (ACHD). There are few data outlining outcomes of concomitant coronary artery bypass grafting (CABG) in these patients. METHODS: Between February 1972 and August 2009, 122 patients (77 men) underwent concomitant CABG at the time of ACHD repair; median age was 64 years (range 40 to 85 years). Thirty patients (25%) had preoperative angina, 7 patients (6%) had previous myocardial infarction (MI), and 6 patients (5%) had previous percutaneous intervention. Most common primary cardiac dia

Tricuspid regurgitation: contemporary management of a neglected valvular lesion.

Right-sided cardiac valvular disease has traditionally been considered less clinically important than mitral or aortic valve pathology. However, detectable tricuspid regurgitation (TR) is common and recent data suggest that significant TR can lead to functional impairment and reduced survival, particularly in patients with concomitant left-sided valvular disease. The tricuspid valve is a complex anatomical structure and advances in three dimensional echocardiography and cardiac MRI have contributed to a greater understanding of tricuspid valve pathology. These imaging techniques are invaluable

Teratogenesis associated with antibipolar agents.

OBJECTIVE: To review the teratogenic effects associated with the use of Food and Drug Administration-approved agents for bipolar disorder. METHODS: A PubMed search of all English language articles published from January 1966 to December 2008 was conducted. The key search terms included all major bipolar agents, cross-referenced with: teratogenicity, teratogen, safety, pregnancy, fetus, bipolar disorder, and malformation. The search was augmented with manual reviews of relevant article reference lists as well as http://clinicaltrials.gov and http://www.fda.gov (both last accessed in April 2008

Perinodal cryosurgery for atrioventricular node reentry tachycardia in 23 patients.

Atrioventricular node reentry tachycardia is the most common cause of paroxysmal supraventricular tachycardia. Available nonpharmacologic therapies include (1) catheter ablation or cryosurgical ablation of the His bundle and insertion of a permanent pacemaker and (2) surgical dissection around the atrioventricular node or discrete cryosurgery of the perinodal tissues, in an attempt to divide or ablate only one of the dual atrioventricular node conduction pathways responsible for the tachycardia while leaving the other intact. This report describes 23 consecutive patients who underwent the disc

[Value of 2-dimensional echocardiography in symptomatic heart diseases in the newborn infant and in early infancy].

From January 1980 to March 1983, 73 distressed newborns and young infants, in whom congenital cardiac defects were suspected, were investigated by two-dimensional echocardiography. The diagnosis in these infants could be re-established later by cardiac catheterization and cineangiocardiography, operation or autopsy. Correct diagnoses were set in 97% of the patients (71 out of 73). Specific echocardiographic criteria for the diagnosis of the individual defects are discussed.

Imaging congenital heart disease in adults.

Transthoracic echocardiography is the first-line modality for cardiovascular imaging in adults with congenital heart disease (ACHD). The windows of access that are possible with transthoracic echocardiography are, however, rarely adequate for all regions of interest. The choice of further imaging depends on the clinical questions that remain to be addressed. The strengths of MRI include comprehensive access and coverage, providing imaging of all parts of the right ventricle, the pulmonary arteries, pulmonary veins and aorta. Cine images and velocity maps are acquired in specifically aligned pl

[Diagnostic and frequency of fetal heart disease by echocardiography in pregnancies with high-risk factors].

BACKGROUND: Fetal heart diseases are increasingly frequent. These are part of birth defects and, therefore, appear in early phases of the gestation. Diagnosis or early detection of functional and structural abnormalities of the heart allows to the obstetrician and the pediatric cardiologist to plan, in a timely manner, the treatment and prenatal and postnatal monitoring with the purpose of preventing complications. OBJECTIVE: To determine the types and frequencies of heart diseases in pregnancies with high-risk factors. PATIENTS AND METHOD: It was analized an observational, descriptive and r

Differential diagnosis of arrhythmogenic cardiomyopathy: phenocopies versus disease variants.

Arrhythmogenic cardiomyopathy (ACM) is a genetic heart muscle disease caused by mutations of desmosomal genes in about 50% of patients. Affected patients may have defective non-desmosomal genes. The ACM phenotype may occur in other genetic cardiomyopathies, cardio-cutaneous syndromes or neuromuscular disorders. A sizeable proportion of patients have non-genetic diseases with clinical features resembling ACM (phenocopies). The identification of biventricular and left-dominant phenotypic variants has made differential diagnosis more difficult because of the broader spectrum of phenocopies which

Ebstein's malformation of the tricuspid valve: short-term outcomes of the "cone procedure" versus conventional surgery.

OBJECTIVES: We report our analysis of conventional surgery and the cone procedure for Ebstein's malformation (EM) of the tricuspid valve at a single institution. Previous conventional surgery for EM, including use of bioprosthetic valves, has inherent problems especially in pediatrics. The newer cone procedure aims to construct a funnel-like valve out of native leaflets, obviating problems with artificial valves. METHODS: This is a retrospective cohort study to examine short-term outcomes of both surgeries for EM. RESULTS: Nineteen patients (our initial cohort) had the cone procedure, and 13

[Ablation of postoperative "incisional" reentrant atrial tachycardia and flutter in children using the CARTO system].

OBJECTIVE: In children with congenital heart diseases who have undergone surgical interventions, postoperative arrhythmias frequently complicate the clinical course. "Incisional" atrial tachycardia or flutter is one of the most common forms of postoperative arrhythmias in these patients and can lead to significant morbidity and even mortality. The aim of this study was to investigate how to use antiarrhythmic drugs and the CARTO system to treat these cases. METHODS: There were 12 patients with "incisional" atrial tachycardia or flutter complicating surgery for congenital heart diseases in thi

Intracardiac echocardiography (9 MHz) in humans: methods, imaging views and clinical utility.

A new low-frequency (9 MHz, 9 Fr) catheter-based ultrasound (US) transducer has been designed that allows greater depth of cardiac imaging. To demonstrate the imaging capability and clinical utility, intracardiac echocardiography (ICE) using this lower frequency catheter was performed in 56 patients undergoing invasive electrophysiological procedures. Cardiac imaging and monitoring were performed with the catheter transducer placed in the superior vena cava (SVC), right atrium (RA) and/or right ventricle (RV). In all patients, ICE identified distinct endocardial structures with excellent resol

Infective endocarditis in the grown-up congenital heart (GUCH) population.

AIMS: Infective endocarditis accounts for 4% of admissions to a specialized unit for grown-up congenital heart patients. This study defines lesions susceptible to infection, antecedent events, organisms, outcome and surgical treatment in a group of such patients. METHODS AND RESULTS: The grown-up congenital heart disease database was searched for all patients aged 13 years and above with adequate documentation of infective endocarditis retrospectively between 1983-1993 and thereafter between 1993-1996. There were 185 patients (214 episodes) divided into Group I: 128 patients unoperated or pal
