Zellweger syndrome, a paradigm of human peroxisomal disorders is characterized by dysmorphic features, hypotonia, severe neuro-developmental delay, hepatomegaly, renal cysts, sensorineural deafness and retinal dysfunction. This is a case report of a baby boy born with facial dysmorphism, profound hypotonia, seizures, and hepatomegaly. The diagnosis was not evident initially but only later when he presented with obstructive jaundiced and renal cysts. He died at the age of seven months. Biochemical studies revealed elevation of very long chain fatty acids and phytanic acid consistent with a pero

Gastric volvulus can occur along the organoaxial axis or the mesenteroaxial axis. We present a patient with a gastric bascule: a gastric volvulus due to two lead points. A 17-year-old boy with dextrogastria, asplenia, and left diaphragmatic eventration presented with acute onset of nonbilious emesis, jaundice, and diffuse abdominal tenderness. Surgical exploration demonstrated a gastric volvulus, with lead points of torsion at the gastroesophageal junction and the second part of the duodenum, causing biliary obstruction. After decompression, reduction, and gastropexy, the patient recovered wel

Cecal volvulus is an uncommon cause of acute bowel obstruction in adults. The mechanism is torsion of the enlarged, poorly-fixed or hypermobile cecum. Patients with this condition may display highly variable clinical presentations, ranging from intermittent, self-limiting abdominal discomfort to acute abdominal pain associated with intestinal strangulation and sepsis. The treatment needs to be individualized for each case, but surgical management is required in almost every case. In the presence of gangrene or perforation of the cecum, resection and primary ileocolic anastomosis is recommended

Tumor-associated gastroparesis, though reported in association with various malignancies, is rare in patients with cholangiocarcinoma. We report a 55-year-old woman who presented with dysphagia and recurrent vomiting. Esophagogastroduodenoscopy revealed dilated stomach and excess residue without organic obstruction. 99mTc sulfur colloid solid gastric emptying study, radio-opaque marker gut transit study, and esophageal manometry showed features suggestive of gastroparesis and achalasia cardia; electrogastrography revealed bradygastria. Cholangiocarcinoma was detected on CT scan performed after

Although local expertise needs to be considered, the following general guidelines for the palliation of malignant biliary obstruction have been proposed: (1) patients in good general condition with small tumors should undergo laparotomy to assess resectability and undergo surgical bypass; (2) patients with advanced disease and poor general condition are suitable for endoscopic stenting to reduce jaundice, pruritus, and risk of cholangitis; (3) patients with poor cognition and limited life expectancy should be managed pharmacologically as appropriate; and (4) research is required to assess opti

Biliary diseases in the elderly results in two major problems: gallstone disease and malignant jaundice. Ultrasonography and direct cholegraphy via the transpapillary or transhepatic route are the appropriate diagnostic methods. "Open" or "minimal invasive" laparoscopic cholecystectomy is indicated in patients with symptomatic cholecystolithiasis requiring definitive treatment. Elderly patients will rarely be good candidates for conservative management with gallbladder stone fragmentation and/or dissolution. In patients with acute cholecystitis and gallstone ileus surgical intervention is reco

Acute hepatic failure developed in four patients (aged 7 to 13 months) who had extrahepatic biliary atresia treated initially by portoenterostomy. Two were stable outpatients with minimal jaundice, while the other two were hospitalized for metabolic or nutritional complications. Postmortem examination in each patient revealed massive acute hepatic infarction with few surviving hepatocytes. In all cases, the hepatic failure had been preceded by an episode of hypotension and/or hypovolemia. The exact pathogenesis of the infarction remains unclear but it may be related to decreased hepatic blood

A previously healthy, 59-year-old man suddenly developed epigastric pain followed by jaundice. Extensive clinical investigations suggested that the patient had a sclerosing cholangitis. At laparotomy, the extrahepatic bile ducts near their confluence were found markedly thickened. A biopsy specimen was obtained from the common bile duct. It showed a diffuse lymphohistiocytic infiltration, suggesting a lymphomatous infiltration. No evidence of a lymphoma was found in the abdominal cavity or in the retroperitoneal spaces. The clinical consensus at that time was that the patient had a primary scl

Background/Objectives: Icteric hepatocellular carcinoma (HCC) is characterized by obstructive jaundice, impaired liver function, and limited treatment options. Biliary drainage may relieve jaundice and permit subsequent anticancer therapy, but the clinical benefit of systemic treatment remains uncertain in this setting. Methods: This retrospective, single-center cohort study included 78 patients with icteric HCC who underwent biliary drainage via endoscopic retrograde cholangiopancreatography (ERCP) or percutaneous transhepatic cholangial drainage (PTCD). Thirty patients received systemic trea

Kawasaki disease (KD) is a systemic inflammatory illness characterized by medium blood vessel vasculitis and is considered to be the leading cause of acquired heart disease in children less than 5 years old in developed countries. KD can present as either a classical or atypical/incomplete presentation based on distinct laboratory parameters seen in Table 1. The exact cause of KD remains unknown; however, specific purported infectious agents have been theorized to be linked to it. It classically presents with fever and four of the following: conjunctival injection, oral mucosal changes, cervic

Choledochal cysts are very uncommon congenital anomalies of the biliary tract, most commonly diagnosed in childhood. Adult presentation is uncommon and often atypical. Giant choledochal cysts, defined as cysts measuring more than 10 cm, are exceptionally rare and present significant diagnostic and surgical challenges. A 19-year-old female presented with a two-month history of dull-aching abdominal pain, progressive right upper quadrant abdominal swelling, and features of obstructive jaundice. Physical examination revealed deep jaundice and a large, non-tender abdominal mass. Imaging studies, i

INTRODUCTION AND IMPORTANCE: Neuromas are a nonneoplastic proliferation of Schwann cells around regenerating axons in the peripheral nerves. Traumatic neuromas are rare and can occur in any location of the body.
CASE PRESENTATION: We present a very rare case of traumatic bile duct neuroma in a male patient in his sixth decade who presented with moderate acute cholangitis and jaundice thirteen years after laparoscopic cholecystectomy with iatrogenic common bile duct injury, primary end-to-end biliary anastomosis was made. Radiological studies showed tumour in the common hepatic duct - most lik

Up-to-date endoscopic correction methods for obstructive diseases of the pancreatic-biliary system occupy a leading position among the arsenal of methods to treat mechanical jaundice. The purpose of the paper is to verify the mechanisms and factors promoting pancreatitis development after endoscopic retrograde cholangiopancreatography (ERCP) and to summarize the efforts to prevent and treat this complication. Materials and methods. A detailed analysis of the structure of nosological units resulting in frequent development of mechanical jaundice was made by the results of treatment of 1214 pati

INTRODUCTION: Mirizzi syndrome is an obstructive jaundice caused by extrinsic compression of the common bile duct by a stone embedded in the cystic duct [1].Cholangitis is a diagnostic and therapeutic emergency whose main risk is that of septic shock [1]. It can also progress to "Ictero-Uremigenic Angiocholitis" associated with sometimes extremely serious renal failure [2].
CASE PRESENTATION: We reported the case of a 73-year-old patient admitted to the emergency room with septic shock on severe cholangitis. Given the presence of acute febrile cholangitis with criteria of septic shock on the

Gall bladder polyps occur in 0.4% of patients undergoing cholecystectomy, the majority of gall bladder polyps are benign, they are classified into 3 types: epithelial or adenomatous polyps, mesenchymal polyps, and pseudopolyps. Gall bladder polys mostly affect females and those more than 50 years of age. Ultrasound is a very sensitive tool in the diagnosis. An 88-year-old woman presented with epigastric pain and right hypochondrial pain, fever, and vomiting for 1 week. Clinical examination showed jaundice and tenderness at the right hypochondrial region. Investigations showed elevated WBC, bil

BACKGROUND Rupture of a pyogenic liver abscess is rare but serious complication. In patients after pancreatoduodenectomy, there are some conditions causing the development of liver abscesses (e.g., chronic reflux-cholangitis, efferent jejunal loop stasis, stenosis of the biliary anastomosis, and pancreatogenic diabetes). However, the number of published cases of liver abscess after pancreatoduodenectomy is small. CASE REPORT A 42-year-old male was admitted with severe abdominal pain, fever, and jaundice. Nineteen years previously, he had undergone pancreatoduodenectomy and cholecystectomy for

This is a case report of a patient of lymphoma with liver metastasis who had persistent pain and jaundice despite multiple lines of chemotherapy. She underwent palliative liver irradiation. This is an uncommon modality due to fear of radiation induced hepatitis. We reviewed literature available on the use of liver irradiation for palliation and concluded that it can be safely used with good results. A 33-year-old female with Non-Hodgkin's Lymphoma (NHL) presented with obstructive jaundice. She underwent stenting followed by chemotherapy. She was lost to follow up to later present with a progre

BACKGROUND: With the widespread use of abdominal imaging, an incidentally found dilated common bile duct (CBD) is a common radiographic finding. The significance of a dilated CBD as a predictor of underlying disease and long-term outcome have not been well elucidated.
GOALS: A systematic review of studies on patients with dilated CBD was performed to identify etiologies and clinical factors that may predict which patients require further diagnostic testing and long-term outcomes. A PubMed search for relevant articles published between 2001 and 2014 was performed.
RESULTS: The search yielded

INTRODUCTION: Autoimmune pancreatitis is now a defined entity and it could mimic a pancreatic malignancy. True oncological emergencies in pregnant patients are rare.
CASE REPORT: A 39 years-old pregnant woman was admitted to our emergency unit due to right upper quadrant abdominal pain and evidence of obstructive jaundice. Since computed tomography-scan and endoscopic retrograde cholangiopancreatography are contraindicated in pregnant woman, a cholangio-Nuclear Magnetic Resonance was performed, confirming the biliary tract dilatation with stenosis of the intrapancreatic portion of the common

AIM: Jaundice, potentially fatal encephalopathy, is common in approximately two-thirds of all well term infants. It is largely due to low expression of constitutive androstane receptor (CAR) in newborns; however, the mechanisms for this low expression were poorly understood.
MATERIALS AND METHODS: Expression of miR-137 and CAR was compared between neonatal and adult mice and between healthy and a mouse model of obstructive jaundice (OJ) using real-time RT-PCR and Western blot methods. Rate of bilirubin clearance was measured. DNA methylation of miR-137 was analyzed.
KEY FINDINGS: Inverse exp
