Tissue engineering via local gene delivery.

The first goal of this review is to describe a local plasmid gene transfer technology known as the gene activated matrix (GAM). GAM was the first gene therapy designed specifically for tissue engineering applications, and the mechanism of action of plasmid gene transfer is closely tied to the normal sequence of events associated with wound healing. The normal sequence of wound healing events is stereotyped for most tissues, and one assumption has been that GAM could serve as a platform technology for local gene delivery in various tissues and organs. This hypothesis essentially has been proved

Feasibility and Safety of Brachial Plexus Blocks in Patients With Cervical Spinal Cord Injury Undergoing Upper Extremity Surgery: Case Series.

Upper extremity reconstructive surgeries in patients with cervical spinal cord injury (SCI) can significantly improve function, but anesthesia planning and postoperative pain management are complex. Continuous peripheral nerve blocks (CPNBs) may offer intraoperative anesthesia and postoperative analgesia benefits, yet literature on their safety and patient tolerance in those with cervical SCI is limited. This case series reviewed the records of 18 patients with cervical SCI who underwent upper extremity tendon transfer, nerve transfer, or reconstructive surgery and received an ultrasound-guide…

[Treatment of foot deformities in patients with Duchenne muscular dystrophy].

AIM: There are different approaches to treat foot deformities in Duchenne muscular dystrophy. Some authors recommend conservative, others on … y (p < 0.05). CONCLUSION: In patients with DMD, lower extremity surgery including posterior tibial tendon transfer can successfully correct and maintain foot position, as well as prolong ambulation. Early surgical treatment is required since conservative means can not prevent progression to severe equinovarus deformities.

[Surgical treatment of recurrent dislocation of the patella in children].

OBJECTIVE: To study an operative method for repair of recurrent dislocation of the patella in children. METHODS: 36 children with recurrent patellar dislocation were treated surgically at an average age of 9.1 years. They were followed on average for 4 years and 4 months. Nine of the 36 children underwent bilateral repair, with 45 knees were treated. The combined soft tissue operations included lateral retinacular releasing, medial retinacular tightening of the knee, vastus medialis muscle transfer to the patella, medial and distal transfer of the half patellar tendon. RESULTS: 28 patients h

Limb salvage despite extensive tissue loss. Free tissue transfer combined with distal revascularization.

Extensive lower-extremity tissue loss may preclude limb salvage despite successful arterial reconstruction. We attempted to avoid limb loss in such patients by combining arterial bypass with microvascular free tissue transfer. Fourteen patient (12 diabetic), 33 to 74 years of age, presented with extensive tissue loss in 15 lower extremities, exposing bone or tendon on the heel, ankle, lower part of the leg, or hindfoot. Mean ulcer size was 5 X 8 cm. Four patients had had previous contralateral below-knee amputations. Femorodistal (seven), popliteal-distal (three), or femoropopliteal (four) byp

The biomechanics of tennis elbow. An integrated approach.

Tennis elbow afflicts 40% to 50% of the average, recreational tennis players; most of these players more than 30 years of age. Tennis elbow is thought to be the result of microtrauma, the overuse and inflammation at the origin of the ECRB as a result of repeated large impact forces created when the ball hits the racket in the backhand stroke. Several authors have found that EMG activity in the ECRB, the muscle and tendon complex afflicted in tennis elbow, is high during the acceleration and early follow-through phases of the groundstrokes and during the cocking phase of the serve. Unfortunatel

The "Eve" procedure: the transfer of vascularized seventh rib, fascia, cartilage, and serratus muscle to reconstruct difficult defects.

Very few microvascular units entertain the possibility of simultaneous vascularized transfer of bone, cartilage, muscle, and gliding fascia. In exceptionally complex conditions with loss of an essential joint, adjacent bone and functional muscle, reconstruction of all these structures at once may be necessary. At the same time, gliding tissue is often required to cover tendons. Reconstruction in one sitting prevents formation of dense scar tissue due to multiple interventions. Additionally, less bone resorption is seen if vascularized bone is used. Therefore, a more undisturbed tissue composit

Neuromuscular Choristoma Variant in the Forearm Presenting as a Posterior Interosseous Nerve Palsy: A Case Report.

CASE: A 14-year-old boy presented with an 18-month history of progressive left wrist drop. Magnetic resonance imaging studies were concerning for mass infiltration of the posterior interosseous nerve (PIN). Surgical resection and pathology confirmed a variant of neuromuscular choristoma (NMC), infiltrated with and surrounded by proliferation of smooth muscle, rather than skeletal muscle. Given the wide-spanning nerve involvement, the patient underwent tendon transfers at the time of surgical resection. CONCLUSION: We report here the first case report of a NMC in the PIN and the first patholog…

Replantation of ring avulsion of index, long, and ring fingers.

The salvage rate of degloved amputation of the fingers is as low as 50%. This case report consists of a multiple-finger degloving amputation characterized by intact tendons, pulleys, and bone, but avulsed neurovascular bundles. After replantation, the range of motion of the fingers was nearly full. Protective-to-light touch sensation was obtained with transpositional digital nerve transfers. Grasp strength was 80% that of the uninjured side. In addition to good functional recovery, the aesthetic appearance obtained by replantation of the degloved skin was far better than that obtained by other

Evaluation of the Kidner procedure in treatment of symptomatic accessory tarsal scaphoid.

The Kidner operation is a successful method of treatment for symptomatic feet with Type II and Type III accessory tarsal scaphoids that do not respond to conservative methods of treatment. The success of this operation is related more to the elimination of the bony prominence on the medial aspect of the foot and its mechanical irritation than to the correction of the pronated foot by means of posterior tibial tendon transfer. The age of occurrence of symptoms can be correlated with the level of physical activity and ossification or growth of this accessory bone. Symptoms in older patients deve…

Transdeltoid Approach to Axillary Nerve Repair: Anatomical Study and Case Series.

PURPOSE: In cases of isolated paralysis of the axillary nerve, dissection of the distal stump at the posterior deltoid border can be difficult because of scarring from an injury or previous surgery. To overcome this, we propose dissecting the anterior division of the axillary nerve (ADAN) using a deltoid-splitting approach. We investigated the anatomy of the ADAN as it pertains to the transdeltoid approach and report the clinical application of this approach in 9 patients with isolated axillary nerve injury. METHODS: The axillary nerve and its branches were dissected in 9 fresh cadaver specim

Immobilisation versus early active motion following primary flexor pollicis longus tendon zones 1-2 repair: protocol for a registry-based multicentre randomised controlled trial in Sweden.

INTRODUCTION: Flexor pollicis longus (FPL) tendon injuries in the thumb are surgically challenging and require careful postoperative rehabilitation to avoid complications such as tendon rupture, stiffness and impaired hand function. Rupture rates after FPL repairs are substantially higher than those reported for finger flexor tendons. Although controlled early active motion (EAM) is widely used following FPL repairs, rehabilitation protocols developed for finger flexor tendons may not be directly transferable to the thumb because of differences in anatomy, biomechanics and functional demands.

Reconstruction of chronic Achilles tendon ruptures using a doubled peroneus longus tendon free graft and interference screw fixation: a technical note.

BACKGROUND: The surgical management of chronic Achilles tendon ruptures (CATR) with significant tissue defects (Myerson Type II/III) presents a clinical challenge. Traditional reconstructive methods relying on free hamstring autografts or flexor hallucis longus transfers are often limited by donor-site morbidity, infection risks, or are unavailable in patients with a history of anterior cruciate ligament (ACL) reconstruction. This technical note describes a refined, tissue-preserving surgical technique utilising a doubled free autologous peroneus longus (PL) tendon graft. METHODS: Under regio
