An introduction to the implant crown with an esthetic adhesive margin (ICEAM).

UNLABELLED: This article describes a novel technique with the addition of a pressed porcelain abutment margin capable of bonding to the porcelain margin of an implant crown restoration. This allows for supragingival margin placement, reduces the potential effect of excess cement-induced peri-implant disease, and provides a controlled environment for the bonding process. Another advantage is the matching esthetics of the crown and supporting abutment, which in the event gingival recession occurs, the restoration appears as a longer tooth without the risk of exposing an underlying abutment margi

Extraction as a treatment alternative follows repeated trauma in a severely handicapped patient.

Handicapped patients with protruding maxillary incisors are prone to repeated dental trauma. A 13-year-old girl with cerebral palsy, severe mental retardation and seizure disorder was referred to our department for restoring the traumatized anterior teeth. Despite drug combination, the frequency of seizure attack was around 10 times a month. The oral examination showed multiple caries, gingival hyperplasia, class II malocclusion with 14 mm overjet and deep overbite. During the first 3 years of a 7-year follow-up period, six episodes of anterior tooth trauma due to seizure attack occurred. The

Fracture resistance of roots obturated with bioceramic or resin-based sealers after nanohydroxyapatite and calcium hydroxide dressing.

This study evaluated the effect of calcium-rich nanohydroxyapatite (nano-HA) as an intracanal medicament on root fracture resistance compared with calcium hydroxide (CH). Forty-eight caries-free maxillary central incisors were instrumented and randomly assigned to a two-factor design (two medicaments × two sealers), yielding four experimental groups (n = 12/group): CH + resin-based sealer, CH + bioceramic-based sealer, nano-HA + resin-based sealer, and nano-HA + bioceramic-based sealer. The medicaments were applied and stored in simulated body fluid at 37 °C and 100% humidity for 10 days. Afte

Influence of obturation technique and sealer type on the fracture resistance of maxillary central incisors with simulated traumatic fractures.

BACKGROUND: This study evaluated the effects of obturation technique and sealer type on the fracture resistance of endodontically treated maxillary central incisors with simulated traumatic crown fractures under clinically relevant loading conditions. METHODS: Forty extracted human maxillary central incisors were randomly assigned to either the single-cone technique or warm vertical compaction technique. Each group was further divided according to the sealer used: AH Plus or TotalFill BC. After obturation, the crowns were restored with a short fiber-reinforced composite and a nanohybrid compo

Effect of Cavity Designs on Instrumentation, Obturation, and Fracture Resistance of Mandibular First Premolars With Vertucci V Canal.

INTRODUCTION: This study aimed to assess the effect of cavity designs on instrumentation, obturation and fracture resistance for mandibular first premolars with Vertucci V canal. METHODS: Mandibular first premolars with Vertucci V canal were scanned with microcomputed tomography. A total of 20 teeth with moderately curved canal were prepared with conservative endodontic cavity (CEC/M) or traditional endodontic cavity (TEC/M), and 30 with severely curved canal were prepared with CEC (CEC/S), modified CEC (MCEC/S) or TEC (TEC/S). CECs and MCEC were prepared using dynamic navigation systems. TEC

Changes in admission laboratory tests in patients with maxillofacial fractures and the influence of dento-alveolar trauma.

BACKGROUND/AIM: There are few studies in the literature evaluating possible alterations in laboratory tests in patients with maxillofacial fractures. The aim of this study was to analyze the changes in admission laboratory tests of patients with maxillofacial fractures with indications for surgical treatment, including the influence of dento-alveolar trauma. MATERIAL AND METHODS: Data from complete blood counts, blood coagulation tests, blood chemistries, and urinalysis results were obtained. The occurrence of concomitant dento-alveolar trauma was noted. The medical records were also re-evalu

Effect of Causative Tooth Extraction on Clinical and Biological Parameters of Odontogenic Infection: A Prospective Clinical Trial.

PURPOSE: To prospectively compare changes of body temperature, white blood cell count, fibrinogen, and C-reactive protein between odontogenic infections in which the responsible tooth was removed and odontogenic infections in which the treatment included no extraction. MATERIALS AND METHODS: The sample was composed of patients admitted to the authors' maxillofacial unit for odontogenic infection from 2010 through 2013. One hundred seventy-nine patients were categorized into an extraction or a non-extraction group based on whether the causative tooth was non-restorable or restorable, respectiv

Urinary catecholamine levels and incidence of dentofacial injuries in children: a 2-year prospective study.

Urinary catecholamines have been used to measure emotionally stressful states which may have an impact on dentofacial injuries. This prospective study investigated the hypothesis that urinary catecholamines such as epinephrine, norepinephrine and dopamine do not affect significantly the incidence of dentofacial injuries in children. As a test of this hypothesis 204 children, males and females, aged 8 to 10 years, were included in the study. After a first examination, each subject was re-examined, at one-year intervals, over a period of two years. Dentofacial injuries were recorded by a clinica

Evaluation of fracture behavior in short fiber-reinforced direct and indirect overlay restorations.

OBJECTIVES: The aim was to assess how incorporating a short-fiber composite (SFC) core would affect the fracture behavior of direct and indirect overlays. Furthermore, to examine the relationship between the thickness ratio of SFC core to particulate-filled composite (PFC) veneering and the fracture-behavior of bilayered-structured restorations. MATERIALS AND METHODS: A total of 120 molars were used to create MOD cavities, with palatal cusps removed. Four different groups of direct overlays were then made (n = 15/group), all of which featured a SFC core (everX Flow) with varying thicknesses (

Cervical-thoracic necrotizing fasciitis of odontogenic origin in a diabetic patient: a case report.

Necrotizing fasciitis (NF) is a severe bacterial infection with rapid and aggressive progression. The infection generally affects individuals with comorbid conditions that lead to immunologic and microvascular deficiencies. It is characterized by necrosis of tissues, mainly in the extremities, trunk, and perineum, and is rarely found in the head and neck. This case report describes the course of NF in a 55-year-old man, highlighting diagnosis, surgical treatment, drug therapy, and supportive measures. The patient, who had chronic alcoholism, systemic arterial hypertension, a smoking habit, and

[Facial trauma and injury in children].

Facial traumas are common in children but often unconsidered. Facial injury is responsible of impressive bleeding because of the rich vascularization of the face; this bleeding is often underestimated because of the immediate arterial vasoconstriction that is very strong for children. The blood volume is 80ml/kg for a newborn, with a total of 250ml, reaching 70ml/kg at one year of age. The evaluation must be rigorously performed due to the risk of a sudden decompensation. Regarding the wounds, the primary repair must be performed directly neat or optimal in case of damaged tissues. The rule is

Spontaneous eruption of an occult incisor fragment from the lip after eight months: report of a case.

Large majorities of dental traumas are associated with soft-tissue injury and tooth fragments. They are often found embedded in the oral soft tissues, most commonly the lip. Early detection and complete removal of such fragments is critical in order to prevent infection and scarring. Adequate communication between medical and dental professionals involved is of considerable value in the management of such cases. This report presents a case of spontaneous extrusion of an occult incisor fragment from the lower lip, eight months after trauma.

Dentoalveolar trauma.

Dentoalveolar injuries represent a serious dental problem for the patient. Time from injury to treatment is a major factor in the outcome of many of these injuries. To decrease dental pain and suffering and enhance the prognosis, medical personnel must recognize the presence of dentoalveolar injuries and consult with, or refer the patient to, a dentist as soon as possible.

Management overview of osteogenesis imperfecta.

The treatment of osteogenesis imperfecta varies with the age of the patients and the severity of disease. Patients with mild disease rarely develop significant deformities; therefore, the care required is mainly limited to periodic fractures or to associated problems, such as erosion of the teeth, decreased hearing, and difficulties in adjustment. It is the patient with severe disease who often requires an unusual amount of attention. Regardless of the immediate problem, the treatment program should be geared toward the long-term function of the patient; in most cases this will be best served

The Richmond Crown Returns: A Laser-Assisted Approach to Restoring a Grossly Fractured Maxillary Central Incisor.

Extensive coronal destruction following trauma or caries presents a recurring challenge in prosthodontic rehabilitation, particularly in the esthetic zone. A multidisciplinary approach combining traditional post-core systems with contemporary surgical adjuncts ensures optimal long-term outcomes. This paper reports the management of a grossly fractured maxillary central incisor using a Richmond crown following diode laser-assisted crown lengthening. A 21-year-old male patient presented with pulpal necrosis and apical periodontitis in tooth 21. After nonsurgical endodontic therapy, post space pr

Evaluation of the fracture resistance of reattached incisal fragments using different materials and techniques.

OBJECTIVES: When coronal fracture occurs in anterior teeth, fragment reattachment can be a valid alternative to a direct restoration. The aim of this study was to evaluate the influence of the material and the technique used to reattach the fragment. MATERIALS AND METHODS: Eighty sound maxillary and mandibular incisors were selected and randomly divided into eight groups (n = 10). The incisal third of each tooth was removed using a saw machine. The fragments in groups 1-4 were reattached using resin-based materials: group 1 adhesive, group 2 flow, group 3 composite, group 4 cement; in groups
