Effects of a head-up position on stone retrieval efficiency and intrarenal pressure using a flexible navigable suction access sheath: an in vitro study.

Although flexible navigable suction ureteral access sheaths (FANS) combined with head-up positioning has shown promise in retrograde intrarenal surgery, the effects of the head-up position during the stone retrieval phase have yet to be quantified. In this study, we evaluated the effects of a 20° head-up position on stone retrieval time and intrarenal pressure (IRP) using FANS in a kidney phantom model. Using a supramolecular hydrogel kidney phantom, we performed 44 trials (22 paired comparisons). Five artificial stone fragments (2-3 mm) were placed in the renal pelvis and retrieved via suctio

Computed tomography-based mapping of ureteral stone distributions by anatomical segment and vertebral level.

Classical anatomical teaching describes three ureteral narrowing points as the principal sites associated with ureteral stone localization; however, contemporary imaging studies suggest a more complex distribution. This retrospective study aimed to characterize ureteral stone distributions using detailed vertebral-level mapping complemented by conventional segmental classification and multiplanar stone measurements. Non-contrast computed tomography (CT) images of 398 patients, each contributing a single ureteral stone, were subjected to analysis. Stones were classified according to five anatom

Impact of enhanced preoperative education on postoperative pain and emergency department visits following ureteric stent insertion: a prospective, exploratory (pilot) randomized controlled trial.

To evaluate the effect of comprehensive preoperative patient education on postoperative pain and unplanned emergency department (ED) visits following ureteric stent insertion, we conducted a prospective, exploratory (pilot) randomized controlled trial. Patients were randomized 1:1 using a computer-generated sequence and sealed, sequentially numbered opaque envelopes prepared by an investigator independent of enrollment; the intervention group received an in-depth, multimodal educational session (an extended verbal explanation, a short video, and handling of a sample stent), while the control g

Incidence and context-specific risk patterns of ureteral avulsion and perforation during ureteroscopy: EAU endourology systematic review and meta-analysis.

Ureteral perforation and avulsion are rare but potentially severe complications of ureteroscopy (URS), and their reported incidence varies substantially because of heterogeneous populations, techniques, and denominators. The objective of presented meta-analysis was to estimate the procedure-level incidence of ureteral perforation and avulsion during URS for ureteral stones and explore study-level factors associated with these complications. This systematic review and meta-analysis followed the PRISMA 2020 guidelines and was prospectively registered in PROSPERO (CRD420261441125). PubMed/MEDLINE

Current status of primary hyperoxaluria type 1 in Japan.

Primary hyperoxaluria (PH) is a rare autosomal recessive disorder characterized by an abnormal increase in urinary oxalate excretion. Primary hyperoxaluria type 1 (PH1), the most common PH subtype, is caused by a deficiency of the liver-specific enzyme alanine:glyoxylate aminotransferase, which is localized to peroxisomes. In Japan, the status of PH1 has not been studied in two decades, and few longitudinal studies of PH1 patients have been conducted worldwide. This study aims to analyze the present situation regarding PH1 in Japan. We conducted a literature review of PH1 cases reported in Jap

Antibiotic prophylaxis before shock wave lithotripsy in patients with sterile urine: a systematic review and meta-analysis.

Whether routine antibiotic prophylaxis (AP) should be administered before shockwave lithotripsy (SWL) in patients with sterile urine remains controversial. We aimed to evaluate the effect of AP in this population. We systematically searched Web of Science, Embase, the Cochrane Library, and PubMed through November 20, 2025. The primary outcome was the composite overall infection risk, and secondary outcomes included symptomatic urinary tract infection (UTI), asymptomatic bacteriuria (AB), pyelonephritis, and sepsis. Ten randomized controlled trials involving 3,298 patients were included. AP did

Infundibulopelvic angle assessment: non-contrast computed tomography vs. traditional methods.

This study aimed to assess the efficacy of a novel methodology for quantifying the infundibulopelvic angle using non-contrast computed tomography with conventional techniques involving intravenous urography or retrograde pyelography. We retrospectively analyzed 35 patients who underwent 64 simultaneous kidney multirow computed tomography scans and intravenous urography or retrograde pyelography. The infundibulopelvic angle values were measured for each patient. A paired t-test was conducted to compare the infundibulopelvic angle measurement values derived from the computed tomography scans wit

When should we stop operating? Multicenter analysis of operative time, outcomes, and complications in ureteroscopy with flexible navigable suction access sheath (FANS) by EAU Endourology.

Flexible ureteroscopy (FURS) is increasingly used for renal stones, with prolonged operative time (OT) linked to complications. Flexible and Navigable Suction Access Sheaths (FANS) may allow longer OTs due to better intrarenal pressure control. We aimed to evaluate risk factors for prolonged OT and associated outcomes using FANS. A retrospective analysis of a prospective multicentric database across 25 centres included 556 adults undergoing FURS with FANS between August 2023 and January 2024. Patients were stratified into three groups by OT: <60 min(group 1), 60–89 min(group 2), and ≥ 90 min(g

Evaluating the natural history of incidentally recognized clinically silent nephrolithiasis in healthy adults: an updated systematic review with meta-analysis.

The incidental detection of asymptomatic renal stones has risen with increased use of imaging and screening programs. While guidelines exist for treating kidney stones, managing asymptomatic stones remains controversial due to limited understanding of their natural history. A systematic search of the PubMed, Web of Science, Embase, and Scopus databases was conducted on 3rd June 2024 with a distinct search strategy and citation tracking to identify English language publications evaluating the natural history of asymptomatic renal stones in healthy adults. systematic report of the included studi

Analysis of microRNA expression in patients with uric acid calculi.

Uric acid calculi (UAC) account for about 10% of cases worldwide, although this varies among countries. The etiology of nephrolithiasis is multifactorial, involving both genetic and environmental factors. Kidney stones occur three times more frequently in individuals with a family history of the condition compared to those without such a history. Genetic factors also contribute to cases of UAC. Genes such as ZNF365, SLC2A9 and SLC22A12 may be associated with the development of uric acid stones. MicroRNAs (miRNAs) are small RNA molecules, that play a significant role in regulating gene expressi

Increased risk of upper tract urothelial carcinoma in patients with kidney stones: a large-scale analysis of the UK biobank.

Kidney stone disease is a common and increasingly prevalent condition, with its incidence rising by 70% over the past two decades in the U.S. Lodgment of stones within the ureteral wall can cause urothelial injury, triggering inflammation, edema, and obstruction. Chronic inflammation has been linked to cancer development, contributing to tumorigenesis through cellular transformation, proliferation, invasion, angiogenesis, and metastasis. Studies suggest that recurrent nephrolithiasis can lead to a range of complications, including an increased risk of urinary tract cancer. This study investiga

Understanding the clinical genetics of kidney stone disease using the Natera Renasight panel.

We aimed to characterize the underlying genetics of kidney stone disease (KSD) in an urban and diverse population using the Natera Renasight genetic panel. This was a single-center prospective study of high-risk KSD patients, defined as recurrent stone formers or those with a family history with KSD. Buccal saliva DNA samples were collected with the commercially available Natera Renasight genetic panel and were analyzed using next-generation sequencing. The panel assesses 385 kidney disease related genes, including 45 linked to KSD. One hundred eleven high-risk KSD patients were enrolled. The

Associations of obesity, sarcopenia, and sarcopenic obesity with the risk of kidney stones in the U.S. Adult: results from NHANES 2011-2018.

Kidney stones (KS), a globally prevalent urological disorder, are linked to metabolic abnormalities and inflammation. Sarcopenia, characterized by progressive skeletal muscle loss and functional decline, frequently coexists with metabolic dysregulation. Sarcopenic obesity (SO), defined by reduced muscle mass and/or function combined with excessive adiposity, may synergistically exacerbate health risks beyond isolated sarcopenia or obesity. However, current studies investigating the relationship between obesity, sarcopenia, SO and KS are limited to single-factor analyses, with limited explorati
