Clinical significance of slow paroxysmal atrial tachycardia.

This study examines the clinical setting, characteristics, and follow-up of 173 patients who had slow paroxysmal atrial tachycardia (SPAT) (greater than 4 beats, rate less than 150 bpm) during 24-hour Holter monitoring. These episodes were classified by probable mechanism according to recognized ECG criteria and included AV nodal reentry (AVNR), sinoatrial nodal reentry (SANR), and automatic (A). There were 76 males (44%) with a mean age of 72 years and 97 females (56%) with a mean age of 73 years. The indications for Holter recording revealed that the SANR and A subgroups had a higher frequen

The clinical and electrophysiologic characteristics of patients with accelerated atrioventricular nodal conduction.

The clinical and electrophysiologic features of 42 patients found to have rapid atrioventricular (AV) nodal conduction during electrophysiologic study are described. The distinctive clinical feature of these patients was the high incidence of supraventricular tachycardias, which were rapid and had been poorly responsive to conventional antidysrhythmic treatment. Abnormalities of both AV nodal conduction and AV nodal refractoriness were present. Medical management and late follow-up were characterized by inconsistent control of rhythm. In four patients, control of the tachycardia was finally ac

Electrophysiological diagnosis of participation of accessory pathway in patients with paroxysmal supraventricular tachycardia.

Electrophysiological studies were performed on 34 patients whose reentrant circuit of paroxysmal supraventricular tachycardia (PSVT) involved normal atrioventricular (AV) conduction system as the antegrade limb and either overt (25 patients) or concealed (9 patients) accessory AV pathway as the retrograde limb. The diagnosis of this mechanism was made by one or more of the following: 1) eccentric retrograde atrial activation sequence; 2) effect of bundle branch block on ventriculoatrial (VA) conduction time; 3) paradoxically premature atrial capture; 4) atrial capture by premature ventricular

Dissimilar atrial rhythms in a patient with the Wolff-Parkinson-White syndrome.

In this report, we present the findings in a patient who had paroxysmal supraventricular tachycardia and atrial flutter associated with the Wolff-Parkinson-White syndrome. During atrial flutter, localized atrial fibrillation was recorded in the coronary sinus (dissimilar atrial rhythms), near the accessory pathway; and during 90 minutes of observation, ventricular activation occurred solely over the normal pathway. We postulate that localized atrial fibrillation repetitively invaded the accessory pathway and rendered it refractory to conduction by flutter impulses.

[Clinico-electrophysiologic effects of magnesium, especially in supraventricular tachycardia].

Clinical electrophysiological effects of magnesium (Mg2+) are known for more than 60 years. Mg2+ is a cation to be found ubiquitously in the human body and is involved in more than 300 different enzymatic reactions. However, so far this ion has not been established as a standard therapeutic tool for the treatment of supraventricular tachyarrhythmia. This may be explained by the inconsistent efficacy of Mg2+, partly in relationship to a given plasma Mg(2+)-concentration, partly caused by the uncertainty regarding the dosage and injection rate or the unawareness of the clinical effects of the ca

Self-administered intranasal etripamil using a symptom-prompted, repeat-dose regimen for atrioventricular-nodal-dependent supraventricular tachycardia (RAPID): a multicentre, randomised trial.

BACKGROUND: Etripamil is a fast-acting, intranasally administered calcium-channel blocker in development for on-demand therapy outside a health-care setting for paroxysmal supraventricular tachycardia. We aimed to evaluate the efficacy and safety of etripamil 70 mg nasal spray using a symptom-prompted, repeat-dose regimen for acute conversion of atrioventricular-nodal-dependent paroxysmal supraventricular tachycardia to sinus rhythm within 30 min. METHODS: RAPID was a multicentre, randomised, placebo-controlled, event-driven trial, conducted at 160 sites in North America and Europe as part 2 of

Association of high serum soluble interleukin 2 receptor levels with risk of adverse events in cardiac sarcoidosis.

AIMS: Although soluble interleukin 2 receptor (sIL-2R) is a potentially useful biomarker in the diagnosis and evaluation of disease severity in patients with sarcoidosis, its prognostic implication in patients with cardiac sarcoidosis (CS) is unclear. We sought to investigate whether sIL-2R was associated with clinical outcomes and to clarify the relationship between sIL-2R levels and disease activity in patients with CS. METHODS AND RESULTS: We examined 83 consecutive patients with CS in our hospital who had available serum sIL-2R data between May 2003 and February 2020. The primary outcome

Mechanisms for spontaneous termination of monomorphic, sustained ventricular tachycardia: results of activation mapping of reentrant circuits in the epicardial border zone of subacute canine infarcts.

OBJECTIVES: The objective of this study was to determine why sustained ventricular tachycardias (VT) sometimes stop without outside intervention. BACKGROUND: Sustained, monomorphic VT in patients with ischemic heart disease is often caused by reentrant excitation. These tachycardias can degenerate into rapid polymorphic rhythms or occasionally terminate spontaneously. METHODS: Sustained VT was induced by programmed stimulation in dog hearts 4 to 5 days after ligation of the left anterior descending coronary artery. Activation in reentrant circuits in the epicardial border zone of the infarct

Haemodynamic analysis of atrioventricular tachycardia.

The aim of the study was to delineate the influence of the ventriculo-atrial interval (VAI) in tachycardia with regard to the underlying heart disease. Haemodynamic studies were performed by the conductance catheter technique during paced tachycardia with a HR of 140, 160 and 180 beat.min-1 at various VAI in 10 patients; three with coronary heart disease (CHD), three with hypertensive heart disease (HHD) and four serving as controls. The influence of the VAI accounted for an overall change in cardiac index (CI) of 30 +/- 14%. Alterations in left ventricular peak systolic pressure (LVPSP) depen

Prolongation of atrio-ventricular node conduction in a rabbit model of ischaemic cardiomyopathy: Role of fibrosis and connexin remodelling.

Conduction abnormalities are frequently associated with cardiac disease, though the mechanisms underlying the commonly associated increases in PQ interval are not known. This study uses a chronic left ventricular (LV) apex myocardial infarction (MI) model in the rabbit to create significant left ventricular dysfunction (LVD) 8weeks post-MI. In vivo studies established that the PQ interval increases by approximately 7ms (10%) with no significant change in average heart rate. Optical mapping of isolated Langendorff perfused rabbit hearts recapitulated this result: time to earliest activation of

Direct His-Bundle Pacing Improved Left Ventricular Function and Remodelling in a Biventricular Pacing Nonresponder.

The optimal pacing modality after atrioventricular junction (AVJ) ablation remains unclear. Herein, we describe the case of a heart failure patient who had AVJ ablation for chronic atrial fibrillation and received a cardiac resynchronization therapy defibrillator device. Because of the lack of clinical response to biventricular pacing, the device was revised with the addition of direct His bundle pacing, which resulted in significant improvement in functional status and left ventricular indices. This case illustrated direct His bundle pacing as an alternative for conventional biventricular pac

Effect of cardiac resynchronization therapy on left atrial reverse remodeling: role of echocardiographic AV delay optimization.

BACKGROUND/OBJECTIVES: Cardiac resynchronization therapy (CRT) improves left ventricular (LV) function in patients with advanced heart failure (HF) and there are some evidences about beneficial effects also on left atrial (LA) dimension and function. The contribution of atrioventricular delay (AVD) optimization on LA changes has not been evaluated. The purpose of the present study was to further investigate the effect of CRT on LA reverse remodelling and to evaluate the contribution of AVD optimization. METHODS AND RESULTS: From the Cardiology Department of Piacenza Hospital and Modena Univer

Development of an ovine model of pediatric complete heart block.

BACKGROUND: Complete heart block is a significant clinical problem that can limit the quality of life in affected children. To understand the pathophysiology of this condition and provide for development of novel therapies, we sought to establish a large animal model of permanent, pacemaker-dependent atrioventricular block (AVB) that mimics the size and growth characteristics of pediatric patients. MATERIALS AND METHODS: We utilized nine immature lambs weighing 10.5 ± 1.4 kg. After implantation of dual-chamber pacemaker devices with fixed leads, AVB was produced by interrupting His-bundle con
