BACKGROUND: The prefrontal cortex (PFC) has been reported to be related to memory function. Especially, the dorsolateral PFC (DLPFC) is a substantial neural structure in short-term memory. In this study, using diffusion tensor tractography (DTT), we investigated the relationship between short-term memory impairment and the DLPFC injury in patients with mild traumatic brain injury (TBI).
METHODS: We recruited 46 consecutive chronic patients with mild TBI and 42 normal control subjects. Fractional anisotropy (FA) and fiber number (FN) of the prefronto-thalamic tracts were determined for both he

The dental pulp is a favorable model for studies of interactions between nociceptive sensory neurons and their peripheral target tissues. In the present study, we retrogradely labeled pulpal afferent neurons with an improved method that permits monitoring of changes in neuronal phenotype in response to controlled tooth injuries. The capacity of retrograde neuronal tracers to diffuse through dentinal tubules was exploited, thereby avoiding the severe injury to the pulp associated with previous tracer application methods. The strategy was to apply the durable fluorescent tracer, Fluoro-gold (FG)

The general classification of head injury proposed by Marshall et al., based on admission CT scan findings, might mask a group of patients who have Diffuse Brain Injury (DI) in addition to intracranial haematomas. The aim of this study was to assess possible differences in outcome with respect to the level of intracranial pressure (ICP) and cerebral perfusion pressure (CPP) between a group of patients with DI: III-IV (Marshall's classification) after the evacuation of an intracranial haematoma (group A) and another group with DI: III-IV in the absence of a mass lesion (group B). We prospective

Gram-negative sepsis has dramatically increased in frequency throughout the twentieth century in the United States. Currently, approximately 200,000 patients develop gram-negative sepsis each year in this country. Of these, about one-quarter develop the adult respiratory distress syndrome (ARDS). Among these critically ill patients, mortality is estimated at 60%-90%. In the complex series of events leading to acute lung injury in gram-negative sepsis, endotoxin is the proximal mediator. Although endotoxin may be capable of causing direct injury to the pulmonary endothelium, its primary role is

Cerebral small vessel disease (SVD) involves ischemic white matter damage and choroid plexus (CP) dysfunction for cerebrospinal fluid (CSF) production. Given the vascular and CSF links between the eye and brain, this study explored whether retinal vascular morphology can indicate cerebrovascular injury and CP dysfunction in SVD. We assessed SVD burden using imaging phenotypes like white matter hyperintensities (WMH), perivascular spaces, lacunes, and microbleeds. Cerebrovascular injury was quantified by WMH volume and peak width of skeletonized mean diffusivity (PSMD), while CP volume measured

In about one third of patients with violent spasticity due to spinal trauma, multiple sclerosis, and diffuse brain injury adequate control with oral antispastic medication cannot be achieved and successful rehabilitation is severely handicapped. In the past these patients were subjected to destructive chemical procedures or extensive surgery. The authors present the results of management of uncontrollable spasticity by means of continuous intrathecal administration of baclofen with a totally implantable gas driven pump system (Infusaid). 30 patients were treated between June 1985 and January.

Local cerebral blood volume (CBV) was mapped in 10 normal subjects and in 30 patients after head injury. In normal subjects, the mean CBV was 4.34 ml/100 gm. The coefficient of variation was 12% for the group mean, and 3% for values obtained in duplicate studies of individuals. Patients with head injuries were divided into three groups according to the stage of their illness: acute illness, early recovery, and later recovery. Averages of mean CBV were within one standard deviation of normal. In contrast, consistent changes were found in serial studies of the mean CBV in the same patient. Compa

Gamma aminobutyric acid (GABA) is an inhibitor neurotransmitter that plays many important roles in the central nervous system. Because the half-life time of GABA is very short in vivo, GABA itself is not used for clinical practice. An analogue of GABA, baclofen, is an agonist of GABA-B receptor, and has very strong antispastic effect by acting to the posterior horn of the spinal cord. However, baclofen poorly crosses through the blood brain barrier, and the antispastic effect is modest when administered orally. Therefore, direct continuous infusion of small doses of baclofen into the cerebrosp

OBJECTIVES: We compared the integrity of white matter (WM) microstructure to the course of recovery in athletes who sustained one sports-related concussion (SRC), assessing individual longitudinal changes in WM fiber tracts following SRC using pre- and post-injury measurements.
MATERIALS AND METHODS: Baseline diffusion tensor imaging (DTI) scans and neuropsychological tests were collected on 53 varsity contact-sport college athletes. Participants (n = 13) who subsequently sustained an SRC underwent DTI scans and neuropsychological testing at 2 days, 2 weeks, and 2 months following injury.

INTRODUCTION: Although the "conventional" autopsy is still considered the "gold standard" for the definition of the cause of death, an emerging interest in noninvasive cadaveric investigations is spreading. Among all these, the application of post-mortem magnetic resonance imaging of the heart is increasingly gaining ground in the study of sudden cardiac death.
METHODS: Using the diffusion tensor imaging sequence, the present study aimed to demonstrate how through the fractional anisotropy value it is possible to qualitatively and quantitatively define sudden cardiac death, particularly in ca

BACKGROUND: In patients with moderate and severe paediatric traumatic brain injury (TBI), we investigated the presence and severity of white matter (WM) tract damage, cortical lobar and deep grey matter (GM) atrophies, their interplay and their correlation with outcome rating scales.
METHODS: Diffusion tensor (DT) and 3D T1-weighted MRI scans were obtained from 22 TBI children (13 boys; mean age at insult = 11.6 years; 72.7% in chronic condition) and 31 age-matched healthy children. Patients were tested with outcome rating scales and the Wechsler Intelligence Scale for Children (WISC). DT MRI

BACKGROUND: A quantitative biomarker for clinical staging is essential for amyotrophic lateral sclerosis (ALS) stratification. This study evaluated microstructural impairment in brain short association fibers (SAFs) across ALS stages via neurite orientation dispersion and density imaging (NODDI) and assessed correlations with disease severity.
METHODS: Diffusion-weighted imaging data were collected from 87 ALS patients (categorized into four groups King's stages) and 37 healthy controls. Whole-brain SAF mapping was performed via a spherical deconvolution-driven probabilistic tractography appr

BACKGROUND: Intracerebral hemorrhage (ICH) triggers a dynamic immune response involving macrophages, However, the functional heterogeneity of these cells and the mechanisms through which they promote repair remain unclear. Although the neuropeptide CGRP has been shown to modulate macrophage phenotypes in other pathological contexts, its role in ICH recovery and white matter repair remains unexplored. METHODS: Single-cell RNA sequencing (scRNA-seq) of CD45 + cells from ICH mice (GSE167593 and GSE230414 datasets) identified macrophage subsets. Flow cytometry, diffusion tensor imaging (DTI), beha

INTRODUCTION: Traumatic brain injury (TBI), caused by external force to the head, leads to anatomical or functional damage to cranial structures. It is a leading cause of morbidity and mortality in adults worldwide, with substantial economic burden. Post-traumatic epilepsy (PTE) is a significant complication of TBI, posing immense challenges to rehabilitation and exacerbating socioeconomic burdens. The incidence of PTE varies widely, underscoring the need for early detection and treatment.
OBJECTIVE: Through prospective electroencephalography (EEG) evaluations over a two-year period, our stud

Trauma to the brain can induce a contusion characterized by a discrete intracerebral or diffuse interstitial hemorrhage. In humans, "computed tomography-positive," that is, hemorrhagic, temporal lobe contusions (tlCont) have unique sequelae. TlCont confers significantly increased odds for moderate or worse disability and the inability to return to baseline work capacity compared to intra-axial injuries in other locations. Patients with tlCont are at elevated risks of memory dysfunction, anxiety, and post-traumatic epilepsy due to involvement of neuroanatomical structures unique to the temporal

BACKGROUND: Historically neurosurgeons have accepted head injured patients only in the presence of a mass lesion requiring surgical decompression. Underpinning this is an assumption that these patients have a better outcome than patients without a surgical lesion. This has meant that many patients without a surgical lesion have been managed locally in the referring hospital. However, there is now evidence that treatment of all head injured patients in a specialist centre leads to improved outcomes. Therefore, we have asked the question: does the presence of a surgical lesion imply better outco

BACKGROUND: Cerebral Palsy (CP) is the most common physical pediatric neurodevelopmental disorder and spastic diplegic injury is its most frequent subtype. CP results in substantial neuromotor and cognitive impairments that have significant socioeconomic impact. Despite this, its underlying pathophysiological mechanisms and etiology remain incompletely understood. Furthermore, there is a need for clinically relevant injury models, which a) reflect the heterogeneity of the condition and b) can be used to evaluate new translational therapies. To address these key knowledge gaps, we characterized

Radiomics is an emerging clinical application in medicine that extracts quantitative features from diagnostic imaging modalities to reveal patterns beyond human qualitative perception. While radiomic applications have been well developed in neuro-oncology, their relevance to other neurosurgical subspecialties and broader utility remain unexplored. Particularly, traumatic brain and spinal cord injuries demand rapid, high-stakes decisions often based on subtle or evolving imaging findings. Radiomics offers the potential to enhance real-time decision-making through predictive analytics and risk s

BACKGROUND: Glutaric aciduria type 1 (GA-1) is an organic acidemia with potentially severe neurological sequelae. In Taiwan, newborn screening (NBS) for GA-1 began in 2001, but large-scale reporting is lacking. This study describes Taiwan's largest newborn screening population to date.
METHODS: Between 2001 and 2015, 1,490,636 newborns were screened for GA-1. Confirmatory examinations included the carnitine loading test. Confirmed patients were treated with a low lysine diet, carnitine, and high-energy intake during illness. Clinical, laboratory, and neuroimaging data were analyzed.
RESULTS:
