Successful treatment of a ruptured dissecting basilar artery aneurysm. Case report.

Dissecting basilar artery (BA) aneurysms in patients presenting with subarachnoid hemorrhage are life threatening, especially in those who experience subsequent bleeding or progressive dissection, and immediate surgical or endovascular intervention may be necessary. The authors report on a 52-year-old woman whose dissecting BA aneurysm was treated successfully with proximal occlusion and flow reversal. Clipping of the proximal BA above the level of the anterior inferior cerebellar arteries facilitated retrograde flow from a radial artery graft placed between the left vertebral artery and the l

[Clinical features and outcomes of 10 asymptomatic adult patients with moyamoya disease].

Recent development of non-invasive diagnostic technology, such as magnetic resonance imaging (MRI) and angiography (MRA), is believed to have made possible on increase in the diagnoses of asymptomatic moyamoya disease. However, no criteria have been established for the management of such cases. The present study aimed to clarify the natural history of asymptomatic moyamoya disease retrospectively. Ten patients were included in this study. None of them had experienced any episode due to moyamoya disease and were only incidentally diagnosed as having moyamoya disease. There were 4 males and 6 fe

[A case of traumatic extracranial internal carotid artery dissecting aneurysm treated by proximal ligation and STA-MCA bypass].

We present a case with the traumatic extracranial internal carotid artery dissecting aneurysm. A 21-year-old man was involved in a motorcycle accident, resulting in multiple injuries but no apparent head and neck injuries. Head CT was normal on his admission. He was discharged from his local hospital 3 weeks after the accident without any neurological deficits. Five weeks after the accident, he suddenly presented with a motor aphasia and a right hemiparesis. CT and MRI showed infarctions in the left para-Sylvian and the left angular areas. Angiography showed a left extracranial carotid artery

Hemodynamic evaluation of vascular reconstructive surgery for childhood moyamoya disease using single photon emission computed tomography.

To evaluate the efficacy of vascular reconstructive surgery for childhood moyamoya disease, the cerebral blood flow (CBF) in 31 hemispheres of 16 patients was examined by single photon emission computed tomography (SPECT) using the 133Xe inhalation method. Results were divided into two groups; 17 hemispheres with superficial temporal artery-middle cerebral artery (STA-MCA) anastomosis [A(+) group] and 14 hemispheres without anastomosis [A(-) group]. The mean hemispheric CBF (mCBF) and regional CBF (rCBF) in the frontal, temporal, occipital, and basal ganglia regions were calculated. Pre- and p

[Extracranial-intracranial arterial bypass in the management of symptomatic vasospasm].

Delayed neurologic deterioration from vasospasm remains the greatest cause of morbidity and mortality following subarachnoid hemorrhage. The authors performed superficial temporal artery-middle cerebral artery bypass in three patients with symptomatic vasospasm and studied its effects on cerebral hemodynamics. All three patients responded neurologically to the bypass procedure within 24 hours. The average cerebral blood flow in the region supplied by the spastic middle cerebral artery increased from 40 ml/100 g/min to 49 ml/100 g/min after bypass. Angiography disclosed dilatation of donor vess

[EC-IC bypass surgery using a long vein graft--reconstructive procedures for the occluded long vein grafts].

EC-IC bypass using a long vein graft has an advantage creating a large amount of blood flow immediately after the anastomosis, but on the other hand, disadvantage of relatively frequent incidence of the vein graft occlusion. In this report, we present three kinds of reconstructive operative procedures for the stenotic or occluded long vein grafts. Type A: A long vein graft bypass between external carotid and posterior cerebral artery was found occluded intraoperatively by the thrombosis occurred where the vein graft was injured during harvesting. Reconstruction was made simply by resecting the

[Recurrent aneurysmal hemorrhage following establishment of an extra-intracranial microanastomosis].

A report is given on the recurrent haemorrhage of a cerebral arterial aneurysm occurring three days after the establishment of an extra-intracranial microanastomosis by means of a venous transplant. The carotid ligature should be carried out in a second session. The short time interval after the haemorrhage and also the larger flow via the venous bypass is discussed as a possible cause.

Medical versus surgical treatment of patients with cerebrovascular insufficiency. A retrospective comparative study.

Report on a series of patients suffering from cerebrovascular insufficiency caused by stenosis and/or occlusion of one or several major cerebral vessels. The patients were divided in two groups, one being medically treated with anticoagulants, the other operated on either a carotid artery thrombosis by thromboendarterectomy or occlusion treated with an extracranial-intracranial arterial bypass. Results after a follow-up period of 1-3 years are compared and discussed in view of the recent literature.

ADAPT technique in ischemic stroke treatment of M2 middle cerebral artery occlusions in comparison to M1 occlusions: Post hoc analysis of the PROMISE study.

BACKGROUND/PURPOSE: The benefit of endovascular thrombectomy in acute ischemic stroke (AIS) therapy of proximal large vessel occlusions (LVO) is established. However, there are few prospective studies evaluating the use of a direct aspiration first pass technique in distal vessel occlusions. This post hoc analysis of the PROMISE study examines the safety and effectiveness of the Penumbra System with the ACE68 and ACE64 Reperfusion Catheters for aspiration thrombectomy in the M1 and M2 segments of the middle cerebral artery. METHODS: PROMISE is a prospective, multicenter study that enrolled 20

Cortical Venous Reddening Predicts Remote Cerebral Infarction Post Superficial Temporal Artery-Middle Cerebral Artery Bypass in Atherosclerotic Occlusive Cerebrovascular Disease.

BACKGROUND: The superficial temporal artery (STA)-middle cerebral artery (MCA) anastomosis (STA-MCA bypass) currently is performed to prevent atherosclerotic occlusive cerebrovascular disease. However, the benefits of the bypass surgery remain controversial. To ensure consistent surgical benefits, understanding the mechanisms of perioperative cerebral infarction (CI) is required. Moreover, appropriate patient selection procedures must be determined to decrease the rate of perioperative stroke. We retrospectively investigated patients who underwent bypass surgery at our institution and determin

Moyamoya disease in pregnancy: a single institute experience.

Moyamoya disease is a rare chronic, occlusive cerebrovascular disease characterized by bilateral steno-occlusive changes at the terminal portion of the internal carotid artery and an abnormal vascular network at the base of the brain. Moyamoya disease particularly affects children and young adults with female predominance, thus pregnant patients with moyamoya disease are not uncommon. Among 4,400 patients with consecutive deliveries in our hospital, 6 patients (0.14%) aged from 24 to 40 years (mean 32.7 years) were found to have moyamoya disease, all of whom underwent cesarean section. Four pa

Changes in cortical CBF and vascular response after vascular reconstruction in patients with adult onset moyamoya disease.

Using 133Xe single photon emission tomography, we evaluated the cortical region cerebral blood flow (rCBF) and vascular response to acetazolamide loading before and after vascular reconstructive surgery in 9 hemispheres in 6 patients with adult-onset moyamoya disease. Before the operation, the resting rCBF was not markedly impaired except that several low rCBF regions were present in the frontal and temporal lobes, and after the operation, the number of regions showing normal rCBF was significantly increased. Regional vascular response was more diffusely impaired in the regions with both norma

Giant cell arteritis: Role of surgery in the management of vascular complications.

Apart from life-threatening and/or functional emergencies, treatment of vascular lesions in giant cell arteritis (GCA) is medical. Revascularization may be considered if the lesion remains symptomatic or progressive despite optimal medical treatment, provided that there is no disease-related inflammation, and always managed by a team of trained experts. The main risk associated with aortic involvement (aortitis) is the development of an aneurysm, most often in the thoracic aorta, after several years of progression. Indications and surgical techniques used to manage these aneurysms follow the r

From Brain to Heart: Possible Role of Amyloid-β in Ischemic Heart Disease and Ischemia-Reperfusion Injury.

Ischemic heart disease (IHD) is among the leading causes of death in developed countries. Its pathological origin is traced back to coronary atherosclerosis, a lipid-driven immuno-inflammatory disease of the arteries that leads to multifocal plaque development. The primary clinical manifestation of IHD is acute myocardial infarction (AMI),) whose prognosis is ameliorated with optimal timing of revascularization. Paradoxically, myocardium re-perfusion can be detrimental because of ischemia-reperfusion injury (IRI), an oxidative-driven process that damages other organs. Amyloid-β (Aβ) plays a ph

Intra-arterial thrombolysis of embolic middle cerebral artery using collateral pathways.

BACKGROUND AND PURPOSE: Cervical internal carotid artery (ICA) occlusion associated with middle cerebral artery (MCA) embolic occlusion requires prompt revascularization to prevent devastating stroke. With the advent of endovascular techniques for chemical and mechanical thrombolysis, the clinical outcome of patients with major arterial occlusions will improve. Finding the most expedient pathway to the site of end organ occlusion for thrombolysis is important. METHODS: We present two cases of acute stroke secondary to thrombotic occlusion of the cervical ICA associated with MCA embolic occlus

MRI hypoperfusion as a determinant of cognitive impairment in adults with Moyamoya angiopathy.

INTRODUCTION: In Moyamoya angiopathy (MMA), mechanisms underlying cognitive impairment remain debated. We aimed to assess the association of cognitive impairment with the degree and the topography of cerebral hypoperfusion in MMA. METHODS: A retrospective analysis of neuropsychological and perfusion MRI data from adults with MMA was performed. Ischemic and haemorrhagic lesion masks were created to account for cerebral lesions in the analysis of cerebral perfusion. Whole brain volume of hypoperfused parenchyma was outlined on perfusion maps using different Tmax thresholds from 4 to 12 s. Regio

Emergency superficial temporal artery to middle cerebral artery bypass after intravenous recombinant tissue plasminogen activator administration for acute cerebral ischemia in a patient with moyamoya disease.

There are few study data to help in the decision whether to perform aggressive surgical revascularization, such as emergency bypass, after intravenous recombinant tissue plasminogen activator (rt-PA) administration in patients with progressive symptoms due to acute cerebral ischemia. A 33-year-old healthy male with no known previous medical history developed right hemiparesis and motor aphasia. No acute lesion was observed on admission computed tomography. According to the treatment protocol, emergency intravenous rt-PA administration was indicated within 3 h. After rt-PA administration, sympt
