Axillary nerve repair by triceps motor branch transfer through an axillary access: anatomical basis and clinical results.

OBJECT: Grafting or nerve transfers to the axillary nerve have been performed using a deltopectoral approach and/or a posterior arm approach. In this report, the surgical anatomy of the axillary nerve was studied with the goal of repairing the nerve through an axillary access. METHODS: The axillary nerve was bilaterally dissected in 10 embalmed cadavers to study its variations. Three patients with axillary nerve injuries then underwent surgical repair through an axillary access; the axillary nerve was repaired by transfer of the triceps long head motor branch. RESULTS: At the lateral margin

Shoulder Reconstruction for Brachial Plexus Birth Injuries: An In-Depth Review and Case-Based Update.

Brachial plexus birth injuries can result in significant shoulder dysfunction with limitations in range of motion, decreased strength, and r … arthroscopic/open release and reduction, and (3) contracture release/joint reduction combined with tendon transfers (latissimus-teres major most common). The lower trapezius transfer is increasingly used for active external rotation as it spares internal rotation strength and has an excellent line of pull reproducing that of the infraspinatus. For advanced joint …

[Open surgical therapy of the rotator cuff].

Almost 100 years have passed since the first report of rotator cuff repair in 1898 by W. Müller. Even now new operative techniques are being developed. Various solutions for difficult extended forms of rotator cuff lesions are available besides the closed and semi-closed arthroscopic techniques. Anatomical reconstructive procedures such as local or distant tendon transfer compete with extra-anatomical procedures such as simple debridement, equatorial partial reconstruction or replacement with a delta flap. Hemiarthroplasty of the shoulder has proven to be more and more the treatment of choice …

Forgoing Upper-Extremity Reconstructive Surgery After Cervical Spinal Cord Injury: A Qualitative Prospective Cohort Study.

PURPOSE: Individuals with cervical spinal cord injury (SCI) rate restoration of upper extremity (UE) function as their top priority. Despite positive outcomes, rates of tendon transfer (TT) surgery remain low, particularly in the United States. The purpose of this study was to better understand why eligible individuals with SCI forgo surgery in an era of expanded treatment options using nerve transfers (NT). METHODS: Data were collected from adults with midcervical SCI who chose not to undergo UE surgery. A semistructured interview guide was developed and used to elicit information about the …

Bipolar Latissimus Dorsi Transfer through a Single Incision: First Key-Step in Poland Syndrome Chest Deformity.

Poland syndrome is a rare congenital anomaly characterized by a unilateral congenital absence of the sternocostal head of the pectoralis major muscle. The absence of the pectoralis major does not only result in chest asymmetry but also in a missing anterior axillary fold, which is essential for natural anatomical appearance in both male and female patients. In Poland syndrome patients, we perform bipolar latissimus dorsi flap transfer, which can be associated with a sublatissimus implant in women. All procedures are performed through a single short midaxillary incision, and tendon translocatio

Midcervical tetraplegia with strong wrist extension: a two-stage synergistic reconstruction of the hand.

Reconstructive surgery can restore appreciable function of the upper limb in tetraplegic patients. The magnitude of the function regained depends on several factors: (1) the level of the spinal cord lesion, (2) careful patient selection, (3) thoughtful application of the basic principles of tendon transfer, (4) absence of severe spasticity, (5) the remaining sensory function of the hand, and (6) the surgical program and the surgeon's expertise with this type of patient. Reconstructive surgery offers patients not only greater physical independence but also psychological benefits. This article r…

Restoration of elbow flexion using the latissimus dorsi musculo-cutaneous flap.

The latissimus dorsi myo-cutaneous flap has been used for restoration of elbow flexion in 8 cases. The indication for this transposition were brachial plexus injuries, significant trauma to the upper limb and restoration of arm contour. The surgical technique involved unipolar muscular transposition (4 cases) with tendon plication (one case) and bipolar transposition (three cases). The evaluation of results is based on the force and degree of elbow flexion, active pronation and supination, as cortical integration. We consider that we have obtained good results in 5 cases and moderate results i

Flexor Tendon Injuries in Zone 3: A Comprehensive Review and Case Report on Flexor Digitorum Profundus Rupture of the Middle Finger.

INTRODUCTION: Isolated rupture of the flexor digitorum profundus tendon within flexor zone 3 is exceptionally rare and easily mistaken for the far more common zone 1 "Jersey finger" lesion. To the best of our knowledge, only two such middle-finger cases have been documented over the past six decades. Reporting this case, together with a literature synthesis, highlights the diagnostic pitfalls and supports timely, tendon-preserving intervention. CASE REPORT: A 65-year-old right-hand-dominant White male felt a sudden snap in his right palm while restraining a dog leash, followed by an inability

Computational analysis of subscapularis tears and pectoralis major transfers on muscular activity.

BACKGROUND: Pectoralis major is the most common muscle transfer procedure to restore joint function after subscapularis tears. Limited information is available on how the neuromuscular system adjusts to the new configuration, which could explain the mixed outcomes of the procedure. The purpose of this study is to assess how muscles activation patterns change after pectoralis major transfers and report their biomechanical implications. METHODS: We compare how muscle activation change with subscapularis tears and after its treatment by pectoralis major transfers of the clavicular, sternal, or b

A Review on Functional Electrical Stimulation in Tetraplegic Patients to Restore Hand Function.

The purpose of this collective review is to describe a new form of functional electrical stimulation called neuroprosthesis. This unique technology has been devised to produce lateral pinch and palmar grasp in persons with C5 and C6 motor level spinal cord injuries. This neuroprosthesis includes external as well as implanted components. First, a receiver is surgically implanted into the patient's chest above a pectoralis major muscle. The receiver stimulator is then connected to 8 surgically implanted epimysial or intramuscular electrodes. Restoration of upper extremity function can greatly im

Patellar tendon reconstruction with semitendinosus-gracilis autograft.

We present a case of a 24-year-old, otherwise healthy, man who sustained a right knee injury after a fall. A small, comminuted inferior pole patella fracture with medial and lateral retinacular tears was encountered that required a small, nonarticular partial patellectomy and patellar tendon repair. An uneventful postoperative course was complicated by a fall onto a flexed knee and rerupture of the patellar tendon at 3 months following surgery. Intraoperatively, a significant tissue void was seen in the area of the prior repair. The patellar tendon was reconstructed with semitendinosus and gra

Surgical technique for thumb-in-palm deformity in cerebral palsy.

The most common surgical procedure performed by hand surgeons in cerebral palsy for thumb-in-palm deformity is release of the adductor pollicis muscle from the middle metacarpal origin, with additional release of the thenar muscles or flexor pollicis longus, as indicated, to decrease the flexion adduction forces across the first ray. Tendon transfer to augment extension and abduction of the thumb metacarpal will help avoid recurrence, and it commonly includes rerouting of the extensor pollicis longus. Stabilization of the metacarpophalangeal joint might be necessary if hyperextension deformity…

Treatment of dynamic scaphoid instability.

BACKGROUND: Chronic wrist pain attributable to scaphoid instability without fixed deformity (dynamic instability) presents a treatment dilemma. The diagnosis usually is delayed, missing the opportunity for direct repair. The intercarpal fusion, tenodesis, and capsulodesis procedures create a fixed deformity. This report presents an outcome study of a new treatment for dynamic scaphoid instability that spanned more than 5 years. METHODS: Arthroscopic evaluation was used to confirm the diagnosis for 105 wrists of 102 patients who had experienced wrist pain for an average of 24.4 months. The tre
