Increasing evidences in animals and humans suggest that CD8(+) T cells contribute significantly to immune defenses against Mycobacterium tuberculosis (Mtb). In the present study, we found that without any stimulation, CD8(+) T cells in pleural fluid cells (PFCs) expressed significantly higher levels of CD69 than PBMCs from patients with tuberculous pleurisy (TBP). CD8(+)CD69(+) T cells expressed significantly higher levels of CD45RO and HLA-DR and lower levels of CD45RA than CD8(+)CD69(-) T cells, demonstrating that CD8(+)CD69(+) T cells were activated memory cells. Furthermore, we found highe

Many reports have associated tuberculosis with haematological abnormalities. These reports suggest that severe pulmonary tuberculosis, if associated with reduced tissue cellular reaction, may cause blood discrasias. Anemia was present in 32 percent of patients. Leucopenia with neutropenia and lymphopenia was observed in 15 percent in patients with very severe clinical tuberculosis. Active tuberculosis was associated with significant reductions in absolute numbers of total T, T4 and B lymphocytes, but there were no significant differences in total T8 counts. T4 lymphopenia causes reversal of T4

In the past decade, the incidence of tuberculosis morbidity has been halved. Nonetheless, an estimated 15 million Americans are infected with tubercle bacilli. Care of the tuberculous patient has moved from sanatoriums to general hospitals and homes. The declining incidence makes the diagnosis less often considered, especially since the classic signs are not always present.

INTRODUCTION: Empyema necessitans (EN) is an uncommon condition where an intrathoracic empyema extends into surrounding extra-thoracic tissues. This case report presents a rare instance of tuberculous EN in an immunocompetent individual.
PRESENTATION OF CASE: We present a case of a healthy young male with complaints of weight loss and a chest wall swelling, initially treated as a subcutaneous abscess. He had a history of TB contact and initial laboratory tests showed elevated CRP and ESR, with no bacterial growth on initial culture from FNAC sample. Subsequent imaging revealed the presence of

INTRODUCTION AND IMPORTANCE: Lipoma is a benign tumor that arises from adipose tissue; subcutaneous fat is the most common site. It is the most common soft tissue tumor in adulthood. They are usually curable with simple excision. They are slow growing and can rarely become cancerous. Lipomatosis is when there are multiple lipomas with different pedicles. Lipomas arising from the thoracic pleura are rare and intrathoracic lipomatosis is exceptionally rare. Here we report a case of huge lipomatosis removed from the right pleural cavity in a 65 years old man.

Sarcoidosis is an idiopathic multisystem disorder with several clinical and roentgenographic features suggestive of respiratory infection. In the absence of infection, it is characterized by the microscopic presence of noncaseating epithelioid granuloma in affected tissues. When present, constitutional symptoms, fever, coughing, and exertional dyspnea usually develop insidiously, although occasionally Lofgren's syndrome--the triad of bilateral hilar adenopathy, erythema nodosum and polyarticular arthritis--may herald the onset of acute disease. Pulmonary involvement is the roentgenographic hal

Middle lobe syndrome (MLS) is characterized by recurrent or chronic collapse of the right middle lobe of the lung. While non-vascular inflammatory causes dominate, endobronchial rupture of mediastinal tuberculous lymphadenitis causing obstructive MLS is an under-recognized entity that presents significant diagnostic challenges. In this context, we report the case of a 24-year-old male immigrant with no past medical history who presented with cough and right-sided chest pain. Chest radiography revealed right middle lobe alveolar opacity. He received empirical levofloxacin without radiological f

Tuberculosis (TB) remains a global health problem, and attention is focused on host immunometabolic pathways influencing antimicrobial immunity, tissue injury, and treatment response. This comprehensive review integrates evidence on the kynurenine pathway (KP) of tryptophan (TRP) metabolism in TB and evaluates its potential as a host-directed therapeutic target. Evidence from human cohorts, in vitro systems, animal models, and non-human primates indicates that Mycobacterium tuberculosis activates the KP across myeloid, antigen-presenting, non-hematopoietic lung, granulomatous, pleural, and cen

BACKGROUND: This study aims to investigate drug resistance patterns in pulmonary and extra-pulmonary tuberculosis (TB), including central nervous system (CNS)-TB, using the largest reference multidrug resistance laboratory in Punjab, Pakistan.
METHODS: Retrospective data from 2012 to 2022 were collected from 1410 patients referred to the multidrug resistant (MDR)-TB laboratory of Mayo Hospital, Lahore, Pakistan. The drug resistant (DR) testing was performed on sputum or pleural fluid in patients with pulmonary tuberculosis, and on cerebrospinal fluid in patients with CNS-TB.
RESULTS: The mea

BACKGROUND: Although pulmonary abnormalities have been recognized in patients with inflammatory bowel diseases (IBD), their prevalence and clinical significance are not known.
AIM: To study the prevalence and clinical significance of pulmonary abnormalities in patients with IBD.
METHODS: Ninety-five non-consecutive patients with IBD (12 Crohn's disease, 83 ulcerative colitis; mean age 41.9 [SD 13] years; 47 women) were prospectively studied from January 2007 to March 2010. Pulmonary function tests (PFT) and high-resolution CT (HRCT) chest were performed in them. PFT were compared to those in

BACKGROUND: The impact of pulmonary tuberculosis persists despite successful treatment and poses a significant burden on healthcare. Post-tuberculosis lung disease (PTLD) is defined as "evidence of a chronic respiratory abnormality, with or without symptoms, attributable at least in part to prior tuberculosis". The abnormalities include residual lung lesions on imaging, lung function abnormalities, and complications such as haemoptysis, relapse, bronchiectasis and destroyed lung among others. These lung disorders have been studied independently but a comprehensive approach to PTLD has not yet

The present study was designed to ascertain whether or not the pleural effusion and serum cytokine levels (granulocyte-macrophage colony-stimulating factor [GM-CSF], interleukin-10 [IL-10], and interferon-gamma [IFN gamma]) in lung cancer patients differ from tuberculous (TB) pleural effusion, in which a strong cellular immune reaction is found; and, whether cytokine levels are a prognostic factor in lung cancer patients with malignant effusion. A total of 202 lung cancer patients with malignant pleural effusion and 26 patients with TB pleural effusion were studied consecutively between 1995 a

BACKGROUND: Empyema necessitatis is a relatively rare entity. Two instances of mastitis secondary to empyema necessitatis, diagnosed by fine needle aspiration biopsy are reported.
CASES: One case was tuberculous in etiology and was initially recognized by cytologic findings of epithelioid and granulomatous cellular reactions and the presence of acid-fast bacilli, which were subsequently cultured and speciated as Mycobacterium tuberculosis. The other case was due to coexisting Actinomyces and Actinobacillus. These organisms were cytologically suggested by "sulfur" granules of filamentous, gram

The articulovisceral form of rheumatoid arthritis is severe, which creates difficulties in its diagnosis as it is characterized by polymorphism and atypical features. This type concurrent with pulmonary manifestations should be differentiated from inflammatory lung diseases, including tuberculosis.

The characteristics and function of lymphocytes in both pleural exudate and peripheral blood in 18 patients with tuberculous pleurisy were studied. The pleural fluid of 13 of 16 patients with tuberculous pleurisy had more T-lymphocytes than the peripheral blood. The difference between them was significant by the t test (p less than 0.05). When lymphocytes in peripheral blood were cocultured with purified protein derivative (PPD), lymphocytes from only 4 of 18 patients produced immune interferon on the fifth day. The titer was only 4 or 8 units/ml. On the contrary, lymphocytes in pleural fluid

Automated detection of Tuberculosis (TB) using chest radiographs (CXRs) is gaining popularity due to the lack of trained human readers in resource limited countries with a high TB burden. The majority of the computer-aided detection (CAD) systems for TB focus on detection of parenchymal abnormalities and ignore other important manifestations such as pleural effusion (PE). The costophrenic angle is a commonly used measure for detecting PE, but has limitations. In this work, an automatic method to detect PE in the left and right hemithoraces is proposed and evaluated on a database of 638 CXRs. W

Medullary thyroid carcinoma (MTC) with tumour-associated granulomas presents a significant diagnostic challenge in tuberculosis (TB)-endemic regions, particularly when thyroid nodules are non-palpable. We report a 75-year-old woman with treated pulmonary TB who presented with dyspnoea and pleural effusion, leading to initial clinical suspicion of TB reactivation. An incidental non-palpable thyroid nodule discovered on ultrasound was revealed to be MTC with epithelioid granulomas on fine-needle aspiration cytology. The absence of a palpable neck mass diverted clinical attention towards pulmonar

Miliary tuberculosis (TB) presents significant diagnostic challenges in resource-limited, high-burden regions when microbiological confirmation is unattainable. We describe a 65-year-old post-menopausal Nepalese woman with prolonged fever, anemia, and constitutional symptoms. Initial chest radiography was inconclusive, but contrast-enhanced computed tomography (CECT) revealed diffuse miliary nodules, calcified lymphadenopathy, and pleural effusion. Microbiological confirmation was precluded by the inability to expectorate and refusal of bronchoscopy. Empirical anti-TB therapy (isoniazid, rifam
